Tier 16 is a 5-piece set obtained with tokens from 决战奥格瑞玛.
There are three different colors: 528 gear from LFR, 540 from Flex shares colors with 553 from Normal, and 566 from Heroics. This is the LFR color.
These tokens are turned into tier from vendors at your faction's shrine:
Alliance: 薇尔贝茨·欢哨 (LFR)
Horde: 布里兹克·火刀 (LFR)

Tier drops are as follows:

Helm: LFR tokens from 嗜血的索克 (Wing 3) in 决战奥格瑞玛.
Shoulders: LFR tokens from 攻城匠师黑索 (Wing 4) in 决战奥格瑞玛.
Chest: LFR tokens from 傲之煞 (Wing 1) in 决战奥格瑞玛.
Gloves: LFR tokens from 纳兹戈林将军 (Wing 2) in 决战奥格瑞玛.
Legs: LFR tokens from Klaxxi Paragons (Wing 4) in 决战奥格瑞玛.

Matching accessories:

Waist: 迦拉卡斯 or 加尔鲁什·地狱咆哮
Feet: 伊墨苏斯, 迦拉卡斯, or 马尔考罗克.
