For clarification: The target of this spell needs to have a Rejuvenation or a Regrowth Buff. This Buff is removed once Swiftmend hit's the target. Therefore you can get the most out of this spell if you apply it to a target who's HoT has nearly expired.
The amount healed does not depend on how many ticks of the HoT are lost, the full 12 seconds are used when determinating the amount of life healed by swiftmend.
Of course, this only applies to non-critical situations. ;-)
How does Swiftmend choose which HoT to remove? After all, tanks with a Druid healing them typically have both on...
You can Swiftmend the rejuv or regrowth of another druid.
As of patch 2.0.1, this talent will now include bonus healing from the talents Improved Rejuvenation and Empowered Rejuvenation.
16% of base mana for a tauren druid is a bit more of 300. The reduced cost of ToL affects this spell.
Can Swiftmend crit?
Does this have some sort of graphic effect ?
Nice in critical situations, but 16% is quite a chunk of mana.
as i've seen swiftmend will consume first ur reju if regrowth and reju ar applied both
dunno if im wrong tested this on 66 level
Build 8820, I just respec'd resto for an Utgarde Keep run and Swiftmend doesn't seem to work. It keeps saying invalid target even though my target has regrowth and/or rejuv. Tested on myself and party members.

the best choice for this efect is to use regrowth which by my calculations will health for about 2k while the rejuvination will heal for around 1k or so

Makes this spell even better, not consuming the HoT's is amazing.
So, if you have Glyph of Regrowth, and double cast regrowth, the hot is now ticking for 20% more.
If you Swiftmend the hot, will Swiftmend hit for 20% because the hot was ticking for more?
A consumed Rejuvenation heals for more than a consumed Regrowth.
This is similar to a Warlock's Conflagrate in a way. Which MAYBE means some conflag tactics can apply to this

imo all druid healers should use the glyph it allows you to heal more w/o having to time it perfectly

The Swiftmend spell icon is not currently available in the list of macro icons. Put the following in the top line of a macro if you want its icon.
As of Mists of Pandaria:
-Efflorescence ticks are now named after Swiftmend, so the combat log will say "Swiftmend" instead of "Efflorescence", and if you have Floating Combat Text enabled (or an addon similar to that), then it will say "Swiftmend fades" after the AoE HoT expires instead of "Efflorescence fades".
-Swiftmend's AoE heal no longer appears to be linked with the amount of healing done by Swiftmend, and the tooltip now states a static number per tick instead of using percentages. However, it is possible that it is still % based, and the tooltip could just be showing the actual numbers that will occur instead of only telling us that it will heal for a percentage.
-The AoE HoT will no longer heal more if the direct Swiftmend crits, as you read in the last point, but it now has its own chance to crit. For example, an Efflo tick can crit heal 2 out of the 3 targets it healed that tick, even if the original Swiftmend was not critical.
If the HoT
still based on the direct heal, then it is a good idea to use it on an ally with a +X% healing received talent (for those moments of raid wide damage)so everyone can benefit more from it.
You can cast Swiftmend if your target have Genesis buff.
As of Shadowlands Alpha (9.0):
Swiftmend will return to its historic functionality of consuming a heal-over-time effect on the target, but with a substantially reduced cooldown and cost.
–Swiftmend will try to consume Regrowth, then Wild Growth, then Rejuvenation, so that its behavior is predictable.
A few notes about using
1) You can only cast
on a target if
has an active HoT from
. You cannot use a HoT on yourself and then consume it by healing another target, nor can you consume an active HoT on a target and apply it to a different target or yourself.
2) Per Nivius' comment below,

in that order. Note that this is basically a "smart" process because it is consuming the HoT with the
amount of healing-over-time remaining. (See #3 below.)
"consumes" a HoT, it does NOT also heal for the remaining healing left on that HoT; it just gives the
heal amount. So "consume" in this case means that the HoT is gone -- it is wasted; the target will never receive that healing.
In other words, if you just want to benefit from the quick heal that
provides, just cast
-- you get the
higher initial heal
with less healing over time, and that
HoT is consumed, with less healing "consumed" (wasted), to give you the
