Probably the easiest non-capital city Elder to get to: just get a portal from any city to Blasted Lands and walk towards the Dark Portal.

Just take the portal at every mage trainer to get you teleported to the portal.

How to create a route for the Eastern Kingdoms Elders in Carbonite.

Click on the torch icon (Favorites) in your carbonite minimap

This brings up the notes window.

Click on the word Root on the left hand side.
Right click on the word Root.
Click on Add Folder
Name it Lunar Festival
Press Enter or click on Accept to create the new folder
Click on the new folder then right click on the new folder you just created.
Click on Add Favorite
Name this one Eastern Kingdoms Elders then press enter or click on accept.
Click on the Record button in the upper left corner of the Favorites window.
Make sure that it is red.

Enter the following lines into your chat window: (you will have to copy and paste these one at a time)

Click on Record to stop recording.

You can now Right click the Targets and rename them if you would like.

MAKE SURE THAT YOU HAVE STOPPED RECORDING!

Now you can click on the first one and Carbonite will automatically create a route for you to follow. If you are unsatisfied with the route, right click on your carbonite mini map window and chose Route then Current Goto Targets. This will redo the path from where you are currently.

Starting from 2015, you need to speak to 希多尔米 (located @ 48.2, 7.2) to get the buff 时间旅行 to find this elder.
Makes me wonder what the Iron Horde had done to this poor elder.

As of Warlords of Draenor, there is an NPC called Zidormi at the entrance to Blasted Lands from the Swamp of Sorrows, if you speak with her she will show you the Blasted Lands before the Iron Horde Invasion and allow you to speak with Elder Bellowrage.

Speaking to her again can bring you back to the Horde Invasion phase.

Since WoD, go to the entrance of the Blasted Lands, talk to the NPC 希多尔米 to return to the past, you can now interact with Elder Bellowrage.

I made this video for the Hallow's End Achievement, but it works also to be able to see this elder. In it, I show you how to get to Zidormi and go to the old blasted lands.
As a side note, if you cross the Dark Portal from the "Old Blasted Lands", you go to Draenor too, instead of Outlands.

This dude is pretty hard to find. To get to him you have to travel back in time and see the old dark portal. You can do this by going to the entrance to blasted lands and talk to the woman there.

This elder is not in blasted lands he is on the other side of portal now so technically it outland just outside portal

All this phasing is just stupid. Blizzard really needs to stop being lazy and stop confusing people. Imagine all the new players who will never complete this achievement, because of this.

This was the last elder for me to honor before achieving "To Honor One's Elders"
When arriving at the Dark Portal in Blasred Lands i noticed Elder Bellowrage was phased to a different moment in time a short fly and a chat with Zidormi fixed it all up and the achievement was mine.

I was a bit surprised to learn that Blasted Lands was phased to the past when I flew directly from STV. I got Elder Bellowrage and moved on towards the Sunken Temple. Then I noticed all the WoD flight points and such off to the east... turned around, the the Portal was phased to WoD status. It was really odd. I never had to speak to Zidormi.

If you have the quest 督军扎伊拉的回响 Anachronos 阿纳克洛斯 will stand in front of the portal instead of the elder. Even 希多尔米 Zidormi won't fix that problem.
After completing 督军扎伊拉的回响 and talking to 希多尔米 again 风怒长者 was there.
