In order to get the 酒瓶伺候（第2赛季） achievement, you need to use this item and target a hostile spectator with the green reticle; landing the throw turns the NPC friendly again, so only one person can do it at a time.
It is up to RNG for the most part whether a spectator will turn hostile - they get a debuff that indicates they have lost a bet, etc.
Keep in mind that you must aim carefully as there is a 30 second CD! There is a minimum range, so the NPC has to be roughly 5-10 yards away.
