Since it's a unique item, you can buy and carry one at the time.
Yes, it WILL be consumed on use.
It teleports you to SE inner segment of Theater of Pain at this exact location: 51.26; 49.05.
It teleports you right next to 哀伤克星, making this item very useful at retrieving it.
It has 1 minute cooldown and no other usage restrictions beside that.

made sure to have it in the inventory together with my potions. logged out for the night since the edible redcaps were crowded to log in before the WQ resets.

put myself in a raid group with an alt and went to bed. next morning i defended the ballistae, ran to the flightmaster, ran to the caps, still crowded, some crossrealm zoning later i had the buffs and wanted to click the scroll - it was gone.

now i am 100% sure i did buy it, but i will have to test if it really disappeared. dumb as i am i didn't bind myself to the theater and the sword is gone like the wq which seemed to complete even in raid.
