I used mine, killed the boss and he despawned when he died. I died to the berserk fire.. Card is gone now. Seems to bug out.

Edit: WARNING!

If you die, or the boss bugs out, the ONLY way to get a new challenge card is by abandoning your quest and going back to Area 52 in Netherstorm, taking the quest again.
