This turns you into a spectral pirate, like it says. More interestingly, it also turns your mounts spectral.
Your best chance at getting this is farming
because the other NPC (
) is tied only to an event and has a limited number of mobs. The other problem is that if you have looted the chest on the character you are farming on, they the pirates will be friendly. I used an alt who cleared the cave over and over and ignored the chest, mailed the grog to my main and never had a problem.
This item is consumed when used. So it's not a permanent transformation item.
After you loot the chest in the Blackguard's cave the Spectral Pirates become friendly, so if you want to stock up on this item, do so before looting the chest or you'll have to farm them on an alt. Don't do like I did and find this out the hard way.
Update: Looks like Evenuel is right - they appear to be on a timer that resets them to hostile after a while.
Just as a note for Druids, if you shift into any of your forms the buff will be removed. Sadly.
Just like some colour enhancing buffs, it can be used with
, to drastically change the appearance and colour of the two mounts. However, it does look slightly glitchy :P
Interestingly, this stacks with the Brazier of Dancing Flames buff, resulting in a blue, fiery Draenei.
This + Ashes of Al'ar/Spectral Tiger looks so good. It intensifies the sparkle by like x100

So demon locks, shamans, druids, so on so on. even cooldowns that make u look diffrent will make this disapear!

Druid shapeshifting no longer removes the buff since 6.0.2. Gives your forms a spectral appearance.
Also, the buff stacks with other transformation items, like Iron Boot Flask and Bones of Transformation, giving you a spectral version of that form.

If you use this as a druid, when you prowl you remain in spectral pirate form. Your buffs will show prowl and cat form, but when you walk you just look like a spectral pirate sneaking around. Guessing this might be useful for role playing?
These drop from Spectral Pirates on Timeless Isle in Pandaria (the isle to the extreme right of the Pandaria map). The Pirates spawn in Blackguard's Forgotten Cove, which is a cave with an UNDERWATER entrance on the left coast of the isle. The entrance is exactly between Huojin Landing and Tushui Landing. The best way to find it is to just ride along the coast until you see a silver treasure box appear on your minimap marked Blackguard's Jetsam. If you see that, swim out to sea a little bit and dive and the entrance will be there.
