About Horrific Visions: What Are They and Why Are They Important?
Horrific Visions are a major gameplay feature coming out in
Patch 8.3 Visions of N'zoth
. These visions are scenarios where players explore corrupted visions of Stormwind and Orgrimmar and complete objectives to disrupt the vision and prevent it becoming a reality.
All players will explore the same Horrific Vision throughout the week, either Orgrimmar or Stormwind. It will then alternate between the two cities every week.
Horrific Visions will be the primary way players:
, improving its stats, effects and corruption resistance.
, used to purchase research upgrades, socket upgrades, and other vanity items.
Obtain gear up to 470 item level, rivaling 475-485 gear from Mythic raid and 475 gear from Mythic+ 15s.
These scenarios can be attempted in group sizes of 1-5 Players and the health of enemies will scale accordingly.
While there are cases that doing these solo may be easier and may allow you to get further, failure is punishing. You can lose your run and hard earned
to something unfortunate. To avoid the risk of missing out on a cloak upgrade and other rewards,
I suggest that you bring friends with you to help cover for you in case something bad happens.
A friend can save you from an untimely disconnect or revive you if you underestimated the enemies you just pulled.

Queuing for Horrific Visions
The party leader can queue for the Horrific Vision by interacting with the gateway located in the

Every member of the party must have a
in inventory. This is consumed once the vision begins.
These are obtained weekly from completing the Black Empire assault on

Horrific Visions User Interface
Horrific Visions use a few specific elements of the User Interface. This overview uses the in-game default UI. If you are unable to see some of the features pointed out below, it may likely be due to an addon. If you are using a UI addon and can't see the Sanity Bar or the Sanity Restoration Orb Button, check your addon's settings and make sure that another element isn't covering them.
- Both your party and raid frames will show how much Sanity your fellow group members are currently at. Raid frames use a simple bar at the bottom of the player's frame instead of the full graphic. If your friends die or reach 0 Sanity, their Sanity bar will glow after which they can be revived by using a Sanity Restoration Orb near them or donating half your Sanity to a downed member.
- This shows your current and maximum Sanity capacities. If this bar depletes to 0, you die. If you are soloing the Horrific Vision, it will end and you will port to the exit to collect your reward. Note: there is passive sanity loss in the visions so don't AFK or spend too much time standing around.
Sanity Restoration Orbs
- If unlocked, this extra action button appears for all members in the vision. Pressing this button will summon a Sanity Restoration Orb that will fully restore the Sanity and Health of all nearby players over 10 seconds and provide immunity to all Madness effects for its duration. Only 3 of these may be used throughout the Horrific Visions, the remaining amount of uses available is shown on the button as well.
- Sanity Damage happens when you lose Sanity by being hit by certain abilities or mechanics. The amount of Sanity lost is shown on-screen near your character when it occurs.
- These are the current Madness effects that you are subject to related to the area you are located in. These effects are always active until you move to a new area which will often bring a new set of Madness effects. You can mouse-over these for details as to what exactly they do.
Main Objective Tracker
- This will show the main objective of the Horrific Vision. Completing this objective will
the Horrific Vision. Here you will also see the total amount of
you have earned so far. These are awarded in full to the player after you leave the Horrific Vision.
Bonus Objective Tracker
- When you are near a new area of the city, that area's objective will appear as a Bonus Objective. Bonus Objectives are additional objectives the player can complete to earn extra rewards at the end of the Vision. There are a total of 4 Bonus Objectives in a Horrific Vision.
Horrific Visions Mechanics and Objectives
Your time inside the Horrific Vision is limited and your
is consumed once the vision actually begins when you talk to Wrathion inside the vision. Make sure you are fully prepared and aware of what you are getting into.
Horrific Visions of Stormwind and Orgrimmar
Players will be queuing into an instanced version of Stormwind or Orgrimmar.
The active city changes weekly and is consistent throughout the week.
The mechanics for both Stormwind and Orgrimmar function similarly, but each has its own unique objectives, layouts, secrets, and special rewards.
Interacting with the gateway in the Chamber of Heart will let you know ahead of time which Horrific Vision is available this week.
You can also view the city map by talking to Wrathion when you first enter the vision. This shows you the city you will be entering and the layout of the corrupted areas. The layout is consistant for each city so once you are familiar with it there is no need to check the map.
Sanity in Horrific Visions
Sanity is a player resource bar that starts at 1000 by default. This is drained passively overtime and drained in larger amounts if you are hit by certain abilities. If your Sanity hits 0, you die. The reverse is also true; if you die, you'll be reduced to 0 Sanity. Note: If you are soloing the Horrific Vision, it will end when you reach 0 sanity and you will port to the exit to collect your reward.
Passive Sanity Drain
- While in any Vision of N'zoth, your Sanity is passively drained just by being inside the vision. In Horrific Visions, the passive Sanity drain varies depending on which area of the Vision you are standing in.
Sanity Damage from Creatures
- Some enemies inside Visions of N'zoth cast abilities that drain your Sanity if you are afflicted by them. In Horrific Visions, any enemy that has an ability that can drain your Sanity will have a buff on it called:
Sanity Damage from Mechanics
- Don't stand in the fire! There are many different patches of bad that will cause sanity loss if you stand in them. Some are caused by enemies, others by Madnesses, and others are just part on the environment (eg. the glowy blue walls around and between areas).
If a player dies and there is another player alive, a living player can use a Sanity Restoration Orb or give half of their remaining Sanity to revive a dead player back to life with the amount they gave. If all players are dead, the Horrific Vision ends and MOTHER evacuates the players to the rewards chamber.
Sanity Loss Resistance and Restoration
There are several ways to mitigate sanity loss.
- The equip effect of
reduces all Sanity draining effects while in the Visions of N'zoth.
is the first Titanic Research upgrade players will have. When purchased, your party will have access to 3 Sanity Restoration Orbs that you can deploy by clicking the extra action button on your screen while inside a Horrific Vision. This will fully restore your Sanity over its duration as well as revive any dead players with 100% Sanity. Further research also opens up other sanity helpers.
The "Good" Potion
- There are Vials of Mysterious Liquid scattered throughout the vision. One color vial will restore 100 sanity if you find it and use it. The color of this "Good" potion changes every time you enter a vision. Check out our guide to figure out how to tell which is the correct color for your run. There are also addons that will provide you with an interface to let you know which potion does what.

Horrific Visions Objectives / Gameplay
Your goal in Horrific Visions is to complete certain objectives while inside the vision. There are a total of 5 objectives per vision that you should aim to complete. There is a "Main Objective" of the vision, and then there are 4 "Bonus Objectives".
It's important to know that your Sanity is your time limit and that you won't be able to complete all objectives until you have some upgrades that provide you resistance against Sanity draining effects or provide opportunities to restore sanity.
It's suggested you only try to do objectives suited for your cloak level and those areas will provide you with what you need to upgrade your cloak.
Objectives that may be too challenging for your cloak level have red warning text when you mouse over them on the map.
Main Objective - Tainted Areas
The "main objective" is the objective located in the first area (also known as the "Tainted Area"), and is the objective players on their first visits will aim to complete every time. This is the objective seen in the objective/scenario tracker. This also appears as a SKULL on the map and minimap.
Completing this objective will END the Horrific Vision immediately, regardless of how many other objectives you have completed and how much remaining Sanity you have.
Completing this objective successfully will yield Tier 1 cloak upgrades (Flesh of Stone) and any rewards tied to Thrall's (Orgrimmar) or Alleria's (Stormwind) Corrupted Chest.
Bonus Objectives - Corrupted and Lost Areas
Players are free to venture out beyond the starting area and into the other sections of the city. There are 4 additional sections to the city, each with an objective. However, each of these areas has a higher passive Sanity drain, more deadly enemies with higher health, and Madnesses that are more relevant.
2 Bonus Objectives are "Medium" tier difficulty located in the "Corrupted Area" of the Vision.
2 Bonus Objectives are "Hard" tier difficulty located in the "Lost Area" of the Vision.
Madness Effects in Horrific Visions
Each area of the Vision will have a Madness that is applied to the player. These are secondary effects the player will experience while being active in these areas.
These Madness affixes will be consistent for subsequent visits within the week, but will change weekly. Each type of area has its own pool of Madness effects. These can be viewed ahead of time by opening up the map and mousing over the objectives.
You can see any Madnesses currently applied to you on the top of the screen. Players deal with 1 affix per area every week, while players running with
deal with 2 affixes per area.
Possible Madness Affixes in Twisted Areas
Your spells and abilities cause you to shift between fast time and slow time. Fast time causes time to pass 30% faster, speeding up buff and debuff timers and causing you to cast, attack, and move faster.
Periodically you will damage all allies and enemies nearby and heal yourself for a small amount based on how many targets you hit.
Periodically summons a zone of power. Standing in it will increase damage dealt by 25%. Failing to stand in it will reduce your damage dealt by 10%.
Possible Madness Affixes in Corrupted Areas
While in combat, you gain stacks of a debuff representing the amount of bugs on you. After reaching 5 stacks, you will be forced to run forward for 5 seconds. Jumping removes stacks.
Occasionally summons a shadowy vision of yourself that will pursue you. If it touches you, you will take shadow damage and be stunned for 4 seconds.
Occasionally you feel hot, causing your movement to force you to leap over a pool of fire.
Possible Madness Affixes in Lost Areas
Occasionally you are encircled by images of yourself, with only 1 section of the circle free for you to escape. Touching one of the images will disorient you for 6 seconds.
Moving in combat will quickly slow your movement speed until you are immobile. You regain the ability to move as you stay stationary.
Occasionally players create a shadow image of themselves at their current location. After 4 seconds, that image explodes, forcing all nearby players to run in fear.
Horrific Visions Potions
In both the Horrific Vision of Orgrimmar and Stormwind, 5 different colored potions are scattered across the zones. Clicking a potion will cause you to drink it, giving you one of the 5 potion effects. Within a Horrific Vision run, all of the same color potions will give the same effect, BUT each color will be randomly assigned a new effect every run.
The 5 effects are sanity related or a 5 minute buff that has a negative effect when it expires. If the buff hasn't expired the second time you drink the same potion, it will extend the buff by 15 minutes.
-100 Sanity (reduced by Cloak level)
- Frequently breath fire dealing damage to nearby enemies. On expiration or removal you will catch fire yourself for a short time.
- Heals 2% of maximum health every 5 seconds. On expiration or removal will slow movement speed for a short time.
- Take 5% less damage from all sources. On expiration or removal you will Vomit uncontrollably for a short time.
Since there is only 1 bad potion (-100 Sanity Potion), once you find out what color that potion is, you can automatically click the rest of the potions for the rest of the run.
Faceless Masks in Horrific Visions
Faceless Masks are the Horrific Visions end-game system. These allow you to (by choice) make your run through Horrific Visions more challenging but also more rewarding. Through exploring, you'll eventually obtain these masks and be able to activate them prior to runs if you choose.
Masks can be activated and deactivated in the starting area of the vision, prior to starting it. Shrines that are glowing red indicate masks that have been collected by a player in the party. There are a total of 5 Faceless Masks.
All 5 Faceless Masks share this same effect. If multiple masks are active, their effects are added.
+25% Health of all enemies
+25% Damage of all enemies
In addition to the above, each mask has a unique effect.
- Reduces Maximum Sanity by 50%.
- Moving causes void zones to spawn in your path and drain sanity if standing within them.
- All Sanity damage caused by creatures is increased by 400%.
- Each area will consist of 2 Madnesses (instead of 1).
- While below 50% sanity, occasionally an enemy of the void will spawn and attack you.
Faceless Masks need to be earned by at least 1 player in the group to be used.

You can earn anywhere from 100 to 6000
from a single Horrific Vision run based on your performance.

Improving the strength of

. This 3 minute cooldown will remove and protect you from all Corruption side-effects from gear for a short time.

Completing the main objective of a Horrific Vision will reward 1 gear item from the "pre-raid" set. The item level of this piece depends on how many other activities you did in the Horrific Vision, as well as how many Faceless Masks were active.

How many cloak upgrade is possible on a single week? Is it limited by how many stuff are you able to retrieve from the visions, or is there a hard limit, for example you can upgrade once on a week regardless you farming 0-24? If there are no hard limit, is it possible to calculate a theoretical roadmap which can be achieved? Like first week cloak level 4, second week level 6, etc?

Faceless Mask of the Burned Bridge With 1 mask active, successfully complete the Valley of Wisdom objective in the Horrific Vision of Orgrimmar.
Am I just reading this wrong or not getting the idea? How to use the mask to earn a mask if I need to earn the mask in order to use it?
Lets say that if your character is about 450 ilvl on live server, are you capable of completing the visions with all objectives done before the Sanity Bar expires? Or rather do safe and just kill Thrall/Alleria and be done with it for the week?
For the people who have already pre-ordered Shadowlands do we still get early access?
That Mad World achievement will keep me at it for a while, I guess. Curious if I can go at it with my tank spec, or if I need a DPS spec. Either way, that's probably gonna be quite some fun.
The drop of the azerite essences is based on my Loot Specialization?
It isn't mentioned on this page but the Sanity Restoration Orb cannot be cast while in combat. Make sure you use it before starting combat.
It feels as a

. I thought content scales depending on the class as well. At least I've read something about it ages ago. I am a resto druid and do about 15-35k damage depending on single target or mobs to aoe down, yet I couldn't do Thrall on my own. I've got a 1.7k rio, 449 ilvl decent gear and I am not the most inexperience player. What am I doing wrong here?
P.S. I've done the visions in partys without problem. I am talking about the
solo options for achievements and better rewards
Just did a couple of runs(Shaman) and the last I sadly managed to die, but to my suprise I wasn't able to use my reincarnation. Which Is a bummer as I was planning on using it as a worst case scenario. Take it as a warning that Shaman's will be restricted here so don't expect to be able to use it.
Wrathion's Cloak quest chain - Week 1
These are the minimum steps you need to take. If you feel confident, you can tackle some of the extra adds (or objectives when your cloak is stronger), but be careful you don't run out of Sanity. If you don't complete in Week 1 you can simply catch up in a following week (you're just limited by how many Corrupted's you can collect from dailies, rares, events and chests in the Assault areas).
Before you start each quest/vision, use any any Corrupted Mementos you have to carry out
. Each trait costs more each time (100, 150, 250, 400 etc), so spend wisely. Bring flask and consumables if you can.
Remnants of a Shattered World
Enter Vision & defeat Thrall
from the chest after mother teleports you
Hand in to receive
& use to upgrade your cloak to Level 1
Use any essence upgrade you get (I got
for Guardian off-spec)
Reconstructing "The Curse of Stone" x 3
Repeat steps from above quest, looting the

Stepping Through the Darkness
By now your cloak should be Rank 5
You need to recover the book
which comes from a Corrupted Area so may want to consider doing this with a group
Wrathion's Cloak - Week 2
You'll need to collect a further 10,000
for another Vessel. If you're short on time, do the mini-vision then prioritise the
quests in Uldum & Vale (two are designed to be short ones, the third takes longer).
You can get up to Rank 7 in week 2, but you will need to do at least one of the two Corrupted areas for rank 6 and BOTH for 7.
This week it's
. You start off to the south of Cathedral. Alleria (the end boss) is inside.
In the corrupted zones, you need to:
Jump over the
that spawn under your feet
Jump to remove stacks of
Run away from the

The small black circles give you a damage buff
One colour of potions scattered around the city restores sanity (pink for my run) the other damages you (green for my run)
Once you've done the corrupted zone(s), if you have enough sanity left, head back to Cathedral and defeat Alleria for extra momentos, but this is optional if you are just after the cloak upgrade and have already got the armor item reward for this week.
Head North East towards Dwarven and rescue Kelsey from 3 Brainwashed Gnomes
Plant 8 bombs at the locations on the minimap, avoiding
near the Blacksmith - avoid his Entropic Leap and move from Void-Tainted Blades (heavy damage and slow)
Use the detonator at the South East exit
(heavy damage & Sanity loss) and move away from
(near the flight master). Interrupt
and watch out for
(turns a player hostile)
(paths from Stormwind Gates to AH). Run away when he casts
(stuns, damages and silences if you get too close)
Free 8 captives from cages
Move out of line of sight when she casts
Move out of the area marked for shower of arrows in
The books and pages are in the chests at the end, after you exit the vision, but before you leave the gateway. You can do this solo (if you are a strong class) or in a party. Trade is hard on melee, so best to bring one ranged.
This is still not very clear. It appears I am in the week for cloak level 8. I am currently level 5. So I entered the Horrific Vision in Ogrimmar and the main objective tracker said I should kill Thrall (this btw should be posted BEFORE I enter the vision and waste the 10k coalescing visions vessel). So I did, the vision ended, and all I got were 150 corrupted mementos. I have the "Stepping through Darkness objective or quest but that item is only in the tier 2 area. So why did the tracker say I should kill Thrall?
Man, I suck at these Visions. First 5 were easy enough but, I can't even sniff getting through the next Wing. Ilvl 442 BM Hunter and I get rolled and fail before I get to the second guy I need to free. Hopefully, Blizz eases up on these once the good players are finished with it. 4 visions wasted this week. Sadness.
For the rare
inside the stormwind version, you will need to drop a
toy train set
to distract him and separate him from his bots. Kill bots, kill him, and pray to RNG gods for
overly sensitive void spectacles
Try these as a healer! Make sure to equip Cyclotronic Blast in yout Pocket-Sized Computation Device trinket and you can almost global the elites. Only thing to be wary about is mobs seem to hit harder. I pulled smaller packs in this case. Very easy!
There's no guesswork required for the bad/sanity drain potion, "HalfandHalfIsWhole" on reddit pointed out:
"The BAD potion color in Stormwind is ALWAYS in the courtyard next to the corpse with the note next to it
The BAD potion color in Orgrimmar is ALWAYS in the hut next to the blacksmith hut obscured by the corpse"
Welcome news from Blizz:
"We’re working on a set of hotfixes to the game that should streamline upgrading your Legendary cloak via Horrific Visions.
- Coalescing Visions now drop in higher numbers from Assaults and Daily Quests.
- The Unique(5) inventory cap on Vessel of Horrific Visions has been removed, allowing you to carry as many Vessels as you like.
- Up to 4 Torn Page of "Fear and Flesh"
quest items can now drop in Horrific Visions from objectives on the rank 7-11 Legendary cloak upgrade quests (was a max of 2).
You’ll need to clear more than two sides areas in one run to get more than two pages. The rank 13-15 quests will still drop a max of 2 pages, only from Lost Area objectives."
These updates are supposed to go into effect with weekly maintenance Tuesday, February 25 in North American region.
For new players - don't rush additional quests until lvl 5 of cloak, you need just the last boss.

For pure dps specs like mage and warlock or even rogue is there a certain spec that is better then others for the visions or does that fall on preference and playstyle. I know in raids a certain spec can be better but wasnt sure about these.
Does anyone have any information on how the visions scale with masks and party size? Is there an optimum group size?

Do yourself and your group an
While doing the quests to upgrade your cloak, confirm that all group members are on that quest, and also that they at least have the minimum upgrade level for that quest.
I got into a group the other night for two pages, and lo and behold, the healer had a level 2 cloak and wasnt even on the quest.
Needless to say, he blew through all three of our orbs within a few minutes and we wasted our Vessels.
I'm convinced Horrific Visions are rigged. On my paladin I can do a 5 mask run without even using a single orb. On my DH? I have failed 5 times now, I can't even get to the last boss with full sanity/insanity restoration. Every ability when I run on my Demon Hunter happens at a 100% frequency. Split personality? happens twice as often, burned bridge? twice as often, EVERY MECHANIC is happening at the same time. There's no way they're designed the same for every class/spec. This was seen in Legion's artifact challenges where certain encounters would change depending on which toon you were on. Really frustrating.
Opinions are mixed on this, but personally I DETEST the vision in Org. I find it so much harder than SW tho I've read people feel the opposite. I struggle with the the madness affix of scorched feet (omg the drag is a drag!), Rexar is a nightmare, and Thrall, for me, is much harder to kill as it is a smaller space to avoid aoe and move. I usually skip Org unless I only need a lower amount of pages. I've cleared SW with masks on an alt that I struggle to squeak thru Org without.
I probably know the answer to this, but is dark delusion affected by slowing traps? It's not as bad as scorched feet but it is still a pain to run away from it and avoid pulling another set of mobs.
Gosh how I hate HV... I am able to finish them, but it's so stressful and I really despise that I am not in control. These debuffs are only annoying and require more luck than skill to avoid.
Got a vision today were I found only potions that restored sanity... Never got that many in one run. Come on... So that basically means that in some runs I might not get a single one?
If it is as it seems and it's completely based on RNG, it's an even bigger fail.
Really disappointed in that content and I am happy when I am over with it, meaning when I have got the mount and my cloak fully upgraded. I won't even bother trying
to get any achievements from there.
Just stop with that RNG already. It's unfun.
Edit 1: got the mount and cloak is now rank 14. Only 1 rank left and I am done with this BS. What I mentioned about the potions actually happened. Didn't get more than 1 sanity gain potion in my last run. :)
Edit 2: Well I finished the main quest and did a 3 masks run solo, was quite easy (no daredevil or long night masks).
This content could actually be enjoyable if, and that's a big if, they would allow us players to time the
and if they would build the HV in manner that would be more depending of skill than RNG.
Edit 3: So here we are... 5 masks visions. Let me put it into words: You spend close to 30 minutes completely stressed out, trying to finish the run, trying to find that one potion that will keep you from getting insane (IG), you freaking hope that the next split personality doesn't proc exactly while feared or confused, and after enduring and surviving all this stressful BS (I call what it is) you get rekt by either Rexxar or Alleria. BRAVO blizzard. You surpassed yourself on that. OH and if you somehow survive you get freaking armguards every. freaking. damn. time! I really hate you for that blizzard.
That is really my biggest issue: I am not in control and that's the most frustrating thing about this. The
procs exactly when I don't need it and forced me to rush into mobs in order to make it valuable. Madness affixes like
will place the open spot for you to leave the area toward a wall so that your trapped.
Something like the Proving Grounds would have been far more enjoyable since it was entirely up to you and your skill how far you would get.
Again, if someone from the dev team reads this, please do a community poll and ask the player base what they think about horrific visions. Based of what I read, saw and experienced, I can't be the only one to really really dislike what you have created here. I trully hope that you will never introduce this kind of mechanics ever again. Let content be challenging due to actuall difficulty and not RNG.
I am super lost, and I'm having a really hard time even finishing the first quest to upgrade my cloak to rank 2. How many areas do I need to clear before I can even get into where thrall is to get the book? I haven't found any guides that get specific on that because you can't just go in and try to kill him, the blue shield is up blocking the entrance. I just don't understand how people are able to do this so easily when I'm having a really hard time.

FYI have just discovered today, after hitting rank 15 cloak and doing, I don't know, 12+ runs ? I was almost certain the mementos were just either added to your score or given to you as a bonus at the end of the run. But nope, the Etheral has to do a little RP first.

Sure with the amount of mementos from the 5 masks I do now with the last couple of runs, I could probably make up that difference in two runs or something, but still, freaking hurts because literally nobody told me about it and nothing online warned me about it other than a reddit comment on an unrelated topic.
Im very late to the party, 16/7/2020 but big shout out to this guide, got my 5 mask run done last night so thats both SW and ORG done, YAY! - 2 things that made the diffrance from struggle street 4 mask to easy 5 mask was:
pick up all the potions, other than the bad one, those buffs all add up (2% healing every 5, combine with the food buff of 2% every sec is extremely useful)
Purification Protocol Rank 3. the shift from rank 2 - rank 3 made a huge diffrance, learn your titan procs, combine with Protocol rank and boom, OP god mode.
currently have Org 5 mask on Farm, easy mode, SW can still be a little tricky but you can do it!
How does this even count as a Guide? It doesn't mention any of the bosses? It doesn't mention the 10% buffs you can get?
Was intimidated to attempt a 5 mask run, solo.I was able to do as a blood DK, (465 ilvl) and ate kbab food to restore 100 insanity. I notice you can avoid some mobs in which i felt like it was very diablo esque to kill everything when i was frost or unholy. I did die when i pulled too many. I was hoping to save my cheat death on rexxar, but cut it very close. I had 1 sanity orb left in which i used in thrall room. Looking forward to next week to try and do stormwind.
I took my time since this was my first time getting a high piece of ilvl gear in any expansion dating back to wrath. I've read a lot of bad reviews of bfa, but i think overall this xpac did very well in regards to item distribution. LFR, Mythics+, and Visions, I slowly worked my way up in getting pieces of gear. I did have to put on hold pvp, because that's how i always obtained my gear, but am not that great in terms of rating to obtain higher pieces.
Visions are fun although i am horde, i like the doing stormwind, less scary :p
