Unable to complete this quest today, because I was unable to enter the rift - the instance was full. Really frustrating. I hope that it will carry over to the next rift.

If you are like me and want to farm these on all alts (for bonus and Soridormi rep) here is the macro to check what you have completed so far on the current character:

This is stupidly imbalanced. We had 20 people in there trying to kill the Lich King and the adds were taking a quarter of HP off our health. We tried killing them but it did nothing.

Basically off a DK target when you're asked to investigate an area, shows up during the Scourge rifts.

Hi, Can anyone provide some hints to get this done. It's my last one for the acheive and ive been trying now for over a month, at least 20 scourge rifts. I keep checking every time rift and once I see a scourge in phase 1 I fly around looking for the mini boss but have not seen it once. What am I missing? Thanks.

This quest does not appear to ever pop up during part 1 of the rifts!

By now I am convinced that it's bugged. I have done every single scourge time rift in the last week and the rare that is supposed to drop the item just NEVER spawns. Needlessly to say I am pretty frustrated, since it is the last item I need for the achievement.

Just got it, spawned from a GIANT gold/blue portal in front of the stairs, Happened during the last phase just before the final boss portal opened, the stage where it flags multiple areas to just kill whatever you want.

This was the final item I needed to complete the meta-achieve. I kept trying to get it from the Scourge/Azmourne rift, but it was not dropping and I kinda wanted to punch a baby...
Instead, I got it from the Murloc/Azmerloth rift, which is Murloc/Scourge.

The Hand of the Lich King mob spawned with about 1min remaining in p1. A special portal spawned on the path @ /way 39 57 and out popped the mob with the name of one of the other players in the vicinity. Quest item on the ground once the mob died.

Finally got this today. This is related to the tasks that Soridormi gives you to complete. One of those tasks is called "In Service of the King" and you must investigate a frozen time rift. From there the NPC you need to kill (a copy of yourself) will spawn. Once you kill the NPC, the quest item will appear on the ground near by. So, your best chance at getting this to pop is to complete as many of Soridormi's tasks as possible during the Scourge time rit

After nearly going insane trying to follow the directions in all of these comments, I finally got this today and wanted to leave additional details in case anyone else is still struggling:

The most important thing I noticed is that the coordinates seem to change during the event. The ~39, 57 that kept being mentioned took me to Valdrakken, but during the event, it took me right to the spot. Outside of the event, if you were trying to find it to be prepared like I was, it seems like the coordinates are ~48, 59. I don't know why they're different - I only noticed during the event that it was actually 39, 57 in the same spot.

If you don't have coordinates, if you fly straight from Soridormi across the central circle where the portal spawns, the entrance to the Titan chambers in the mountain wall is where people are referring to

The larger rift that spawns the npc has the same sandy effect as the smaller rift portals, but it also has some swirling blue in it, and most notably, there was a giant patch of ice on the ground underneath. After a few moments, the npc came out and we could kill it. After it died, the quest item appeared and I could pick it up.

Other notes:

Seems like you have to tag the npc to get the item, or maybe at least be there when it dies - I found the corpse of this npc during one of the events, but the quest item was not there, unlike some of the other items
There wasn't anyone around at the time that the rift spawned, but it did copy a player that flew up during that time
I didn't do any quests, I just hung out in front of the chambers and killed things around the area
Timing seems random - both times I saw the corpse and the spawn were about halfway through the event with about 2-3 additional quests still after it, unlike other comments mentioning it happened during the last free for all
It took a couple of seconds for the quest item to drop after it died, during which time I nearly had a heart attack thinking I didn't get it again
