I'm pretty much done with everything on Forbidden Reach and Vaults, this being one of the few things I'm missing. I thought I was just unlucky, but as it turns out I just find the skeleton really hard to see. I've cleared the relevant room probably 5 times without seeing the actual Skeleton. It wasn't until i watched one of the youtube videos linked here on wowhead, that I recognized which room the Skeleton is supposed to be in. Even with that knowledge, it took me 5 seconds to see the Skeleton after entering the correct room.

I strongly recommend looking at for example https://www.youtube.com/watch?v=2ZTnNoyuPG4 to showcase the room and exactly what the Skeleton looks like.
