I have somewhat always felt that they should get rid of the major two factions (Horde and Alliance) and allow us to gain or lose reputation with any and all of the factions.
The Horde and Alliance would still exist, but our acceptance or rejection by them would be determined by our association with other factions.
For instance, if you started with a human, you'd automatically start out friendly with Darnassus, Ironforge, Exodar, etc. But through your actions, all of those reputations could be driven up or down.
If you were forsaken or human, for example, and started accepting quests from the Syndicate, your home faction would be driven down, making you unwanted even by your own people. You'd be seen just as traitorous as the Syndicate.
Because of this, there would need to be at least two lower level neutral cities that could be reached before you get to a place like Gadgetzan.
As a human, it would be great to be able to wear a Scarlet Crusade or Scarlet Onslaught tabard. The downside to this is that these factions have the potential of being written out of existence by Blizzard through questing or expansions.
Of course, there would have to be some lasting benefit to all of this. Gear rewards are okay but I think this mostly appeals to those of us who enjoy RP.
Another problem is the game mechanic of faction-only communication. Characters would need to be able to speak to all factions, which is the way it actually is. I've never really liked the game-mechanic of the common language only being understood by your own faction? Ideally, I guess, this was implemented to prevent griefing, but that's what reporting people is for.

That language issue was another part i was considering (but forgot to post). When you acheive friendly status with a faction you can learn their language in addition to your own. The "griefers" Blizzard were worried about would be few and far between because those individuals will probably stick to the pvp style of gameplay.
Common is spoke by all races in Lore. It is not in-game for game mechanics. It was primarily for trade between the different races.

A server like this would be perfect for a Goblin race, or the Pandaren for that matter. Very good idea, I don't see it happening, but I like it a lot.
