Drops from heroic mode Nefarian in Blackwind Descent or can also be bought from Faldren Tillstale (A) / Jamuz'Vaz (H) with the 绝望精华 which drops from heroic mode Al'Akir.

PALADINS, NEED on this man! wether your not a main healer or not, NEED this (now I know there's peepz out there who says : oh but the locks gear sucks, or well he needs it more than me.... ) and know that often we (as Plate healers) we DONT get to raid often, unless your in a non-fail raiding guild. or you just happen to raid every WEEK *no lifer* in other cases, its OKAY to roll need man, even if your roll is like 35 or lower, if your a plate Healer , FRAKING /roll for it. I cant stress that enough.

I don't know if it's just me or what, but I have killed Nefarian on Heroic multiple times in BfA and never seen the helm token drop... any of them... the token is, however, dropping off of Al'Akir in Throne of Four Winds (in Uldum)... but the Essence of the Forlon no longer drops. Might be some change from BfA.
