this trinket is actually much better than it looks.
It does -901 threat. Which is just a little bit more than a Fade 6. But its a permanent threat reduction.
Also note the 2 minute cooldown. IE, you can use this at least twice just about every kara fight.

This item is mainly useful for AoE situations, 901 threat is nothing in single target DPS terms, however in AoE situations it totally negates one or two of your Arcane Explosions or 3-4 seconds worth of Hellfre...
Still I don't like this trinket, Resilience is a PvP stat, yet the "Use" is purely for PvE...

This trinket is Beautifull for a warrior, who has no aggro reducing skill.
Often you grab just a little too much aggro with a double windfury crit. The stam and resilience helps your defences survive the hit.
After activating the trinket we loose aggro again to the tank (hopefully), and can be more carefull with nuking ;) A soulstone for a DPS warrior.

this is a very nice arena trinket. im using this on my mage. with all my stam gear, i have well over 11k life unbuffed. 14k buffed.
250 life + 23 resil means its a very nice trinket.

As a feral druid, I will get this trinket for tanking because +def is not worth to have as much as warriors but I still need to reach the immune crit cap. 23 resilience is about %0,6 less chance to be critted upon which equals to about 15 defense skill (or 57 defense rating).
The use ability can come in handy for tanks too where the tank needs to temporarily drop aggro. Temporus & Doomwalker comes into mind.

I consider this kind of trinket very useful in instances with may pulls composed of more than 3-4 mobs, like Shattered Halls.
In these cases (especially if you are on heroic), you'll have a hard time keeping the tank alive without pulling aggro and since threat generated by heals is divided by the number of mobs (a 6000hp heal is considered 500threat on each mob on a 6 mobs pull, for example) this kind of trinket will come in handy and save your skin plenty of times :D

This is very useful for enhancement shammies and ret paladins. Both classes, IIRC, are the only ones in the game who have no innate threat reduction (most 'natural' melee DPS classes generate -10% less threat; ranged DPS has to generate 130% more threat than the tank to pull aggro rather than 110% for melee) or any kind of threat reduction ability whatsoever (fade, feint, vanish, FD, etc)

+27 stam and +24 resilience make this item fairly solid for PvPers of any class working to get Medallion of the Horde and Talisman of the Horde (effective 88 to 97 stamina).
Easily obtainable by doing a few BM runs.

Having used this trinket in pve and pvp I'm much happier with the pvp applications.
So far noone here have mentioned the real pve uses for this trinket and exactly why Fetish of the Sand Reaver is ultimately better than this garbage Use: for pve situations.
For 5-mans and players learning to manage their aggro granted these are some good training wheels, but for serious pve encounters Fetish is incredible.
On a Void Reaver fight (an encounter where threat matters a LOT) you can use this at most 4 times. That's 3600 threat out of hundreds of thousands. Even when combined with shrouding potions this will barely let you dps for a few extra seconds and for me personally it comes out to less than 2% of my threat. With Fetish you get 5-10% more dps before you pull aggro and die on Void Reaver. That's either 100% more dps (your attacks do 50% threat, letting you dps harder) for 60 seconds in the 10 minute fight (10% improvement) during which time you trinket and blow all your cooldowns or on the low end 50% less threat (your contant dps only counts for half) for 60 seconds of the 10 minute fight (5% deduction against total threat).

I thing that this is one of the Best Druid Tanking trinkets.
It have all we need, we have more Use for rezi instead of Defence to reach cap -5,6% crt and its Use is usefull when we R offtanking at several ocasions like offtanking Supremus where we need to get agro to tank him so If I see that I am much higher than main tank I can just use it and give him a chance to reach agro back to him.

I just responded to a post on the wowhead forums where somebody asked what the best tanking pet was. I posted the following relaying my turtle experiences. You may find this interesting.
I don't know the definitive answer to this question but i'd like to relay my turtle experiences here.
With maxed natural armor, and my toon wearing mostly mail, my turtle has 14143 armor at 70 which amounts to 57.26% dmg reduction. On top of this he has shell shield which reduces dmg taken by 50%. It has a 3 min. cooldown unfortunately. It should come as no surprise then that there is no pet that can take more of a beating than a turtle. This has been stated previously.
The problem is that unlike owls and gorillas they have no aoe attacks and have trouble holding agro of large mobs.
The only way however to get the best of both worlds is to use the turtle. There's nothing you can do to allow gorillas and owls to take more of a beating, but there are a number of ways to make your turtle better at controlling agro.
These include:
Intimidation (BM)
rotating growl around to different targets
Misdirection once you get it
Also if you are alone fighting mobs with your turtle and you pull agro, there are some things you can do to reduce your agro.
These include:
feign death
Trinkets like The hypnotist's watch, The jewel of charismatic mystique, and the Timelapse shard which cast basically cower and reduce your agro.
There's a cloak enchant that reduces your agro from all attacks by 2%
You're probably thinking why would I want to use these trinkets and give up my bloodlust brooch.
These trinkets are used for killing large mobs quickly and that's it.
If you're trying to farm obsidian war beads or motes of water in nagrand for example, it's faster to use volley and the super explodey trap on large mobs rather than killing things one at a time, since their damage scales linearly with the number of enemies they're hitting. (double the number of enemies = double the damage etc.) There's a very real benefit to using these but hunter's generally don't because because they pull agro off their pet, which before 2.3 was an even bigger problem because of the dead zone. When this happens you use the trinkets. In a way it's like having three feign deaths instead of one.
On a slightly different note, there are enemies that hit too hard for you to solo even with a turtle. That's where cower comes in. To my knowledge this is the only situation where cower would be useful.
Instead of sending in your pet and praying he lives you can send in your pet and before he's going to die, you pull the agro off of him and kite while you heal your pet and it continues to do dmg, then you have the pet pick him up again. and repeat. To get agro from your pet you can use distracting shot to help and your pet can use cower. Also turn growl off and on at the appropriate times.
My turtle's name is pizzaface. and just for laughs in shatt i frequently turn on bestial wrath and shell shield, big red sparkling turtle. hilarious.
Hope this helps you to die less.

For a starter druid tank, I'd say this would certainly warrant a trinket slot. Stamina and Resilience? I'm farming rep for the enchanting recipe anyway, this is a pretty decent side benefit.

Summary: this trinket gives 15 DPS for a threat-capped shadowpriest and any similar class.
This trinket gives:
reduction of 900 threat per use (2 minute cooldown)
reduction of 450 threat per minute
reduction of 7.5 threat per second
The average raid-buffed shadowpriest has roughly 50% normal threat reduction (talent, salvation, etc.) but still tends to have his or her DPS capped by threat if well geared. That shadowpriest COULD do more damage/healing on a given fight but would pull agro. So, during an extended single-target boss fight (Void Reaver, Gruul, etc.), a shadowpriest using this trinket can reduce threat by 7.5 threat per second, thus, increase damage by 15 damage per second (or 15 damage + healing per second).
Not much? Maybe not. But a threat-capped shadowpriest using a +spelldamage trinket gets NOTHING out of the +spelldamage trinket because DPS is already capped by threat. So get this trinket and do more damage than you did last week.
(FYI: under similar math, the shadowpriest can do an extra 7 DPS with

An odd combination of stats vs. on-use, but if - like me - you're trying to rapidly get together a PVP off-set on an otherwise PVE character it could definitely come in handy. The fact I can get it with rep I already had from helping kara key people is a definite bonus.

2) taking the previous into consideration, this one clears A LOT more threat than the blue one. In 10 mins, you clear 2150 threat with the blue. You clear 4505 with this one.
To some people saying the stats are confusing :
1) it has clear pvp utility.
2) it is really good for tanks to get crit immunity when they gimp their gear for resistance.
3) its better than AP/Spell damage trinkets in fights where you have to gimp your dps due to threat issues.

Anyway.. This trinket isn't supposed to be used when you are being smacked by mobs. it's supposed to be used whenever it's up. Just do the maths. If it does -7.5 TPS (like Arodi said) on a 5 min fight it will allow you to do an extra 2250 damage/healing without threat reducing buffs/talents. It's obviously a bad trinket to someone who can't double the tank's damage, but that's a different issue and those people shouldn't be here talking trash.

This trinket is good for resistance tanks in order to become crit immune. Resist gear has no defense so this trinket can help make the defense in your other slots count for more. And since the only trinkets worth using with def are the hydross one (
Scarab of Displacement
) and the bloodboil one (
Shadowmoon Insignia
), not counting
Styleen's Impeding Scarab
, which is still good imo, anything that you can put in the trinket slot to help get you uncritable helps. This trinket is one of those things.
It can be helpful for FR Tanks on Illidan if you want an idea of how useful it is (as well as the FrR and NR tanks on Hydross).

This Trinket is best used for shadow priest as it gives resilience and stamina that can be used in PvP situations and an extra fade for PvE.

This trinket is only good pvpers, who havn't bothered to get a kind of Medalion of the alliance/horde, and hasn't done a single trinket giving quest in whole tbc.

I haven't seen this mentioned yet, so here goes:
This trinket is also useful to miners on PvE servers. If you aren't a hunter or a rogue, you might find it extremely satisfying.
Have you ever been mining a node and had a hunter or rogue drag a bunch of guys to you and then feign death or vanish, only to reappear seconds later and ninja the node from you? I know I hate it when that happens to me.
Well, with this trinket, the joke's on them. Wait until they start mining the node (don't attack any of the mobs that are hitting you in the meantime), and then pop this thing as soon as they do. Suddenly the aggro will be back on them, and you can finish mining your node in peace. Wonderful stuff. :)
