According to the data provided for this items spell effect, this has a 20% chance to proc with no internal cooldown. If accurate, that could make this pretty good.
It may not do a ton of damage on its own, but consider its in your NECK slot, a slot that generally contributes ZERO damage. Even if it hit once a minute for 6, it'd be a pure gain (assuming it has your proper stat allocation).

After obtaining this item, I've been able to test it out a lot. In my situation it was an item level loss (it dropped with no upgrade procs, and the necks for my tanking set are 870 and 875) but the stats on it arent horrible since as a DH, Mastery is good and Crit adds to avoidance.
On a single target fight, this item's bonus was generally not worthwhile. While it was still certainly extra damage, the gains were so minor that the flat-out stat gain from a higher item level piece will outweigh it. While I would certainly go for the bonus if your alternative is of similar strength, I would NOT replace a piece with any significantly higher stats, even just agility/stamina, for the proc alone.
However... when you move to any type of multi-mob tanking, things change. If you can reliably refer to your tanking as "AOE", generally meaning a minimum of 3+ enemies most of the time and 5+ sometimes, this neck really shines. On some trash pulls, it was my
third
damaging ability. The more enemies you have the more damage it will do, as it reacts to being struck and has no internal cooldown. Basically, the value of this neck goes up based on your number of targets. Your damage intake does NOT matter, simply the number of attacks against you; it doesn't matter if you're being hit for 1m or 1k, it's quantity over quality in this situation.
Also keep in mind that only attacks that LAND can proc this. If you are a spec that relies heavily on avoidance such as dodge/parry, the proc will lose value. Tanking classes that rely more on soaking and mitigating damage will gain much more out of this over classes that rely heavily on avoiding damage entirely. Keep that in mind when deciding to use this as a tank; when your job is to avoid damage, using an item that only works when you TAKE damage means that item will deprecate in value as you get better at your tanking role!

Sadly the damage effect doesn't scale with the ilvl if you get war/titan forged still very promising piece.

I've tested it a bit today as Bear, does about 2-4% total dps depending on boss. Answering to previous comments - apparently it will only proc from white attacks (autoattacks), not from direct magic damage or AoE abilities, which makes it useless for either melee or ranged damage dealers. On Shadow of Medivh where boss always casts something - neck procced only once.

I've been using this on my Vengeance Demon Hunter for tanking. I replaced a higher iLevel neck and have no regrets so far. This neck's passive proc seems pretty ridiculous. It won't do much on boss fights, but on Mythic+ trash it's often doing 5-10% of my total DPS, which seems crazy to me.
Maybe BiS for Mythic+ tanking due to the damage? I'm not seeing myself replacing it any time soon, seems very strong.

Does anyone know if this neck's effect can be procced by stagger? That would make it pretty good for brewmasters.
I currently have it equipped on my brewmaster and it's around 5 % of my single target DPS. Not sure if that's because of stagger or if it's that good on its own.

The damage of this piece scales quadratically with the number of targets.
For example, if 5 targets are hitting you, this neck will proc 5 times more often, and each proc will deal 5 times the total damage, therefore, it would be 5^2 = 25 times more powerful than against a single target.
Worth taking note since very few things scale this well.

I equipped this trinket and it was 3% of my damage in a heroic dungeon. Therefore, I value this proc as 10 item levels, when comparing with other items.

This proc currently doesn't scale with timewalking, making trash packs trivial and providing excellent speedrun capabilities. Most likely a bug.

This necklace is incredible for tanks looking for more damage when speedrunning Heroics, Mythics, low Mythic+, and Timewalking(!). Slap Hidden Satyr on this and you've got a ton of damage from your neck slot. It won't suddenly carry damage in your groups, but it will make a difference, especially in AoE tanking and pulling large packs. It'll still proc a healthy amount on bosses/single target.
While this isn't amazing for raid tanking, it'd be strong for trash. I definitely recommend having this for a tank, even if you bank it until you start speedtanking lower content.

As an Ilvl 860 Demon Hunter, this deals 8.5% of my damage in aoe situations (this is an average, as stated by Wizmike, the damage dealt scales massively on amount of enemies.) like dungeon packs.
On single target, it is unnoticable at less than 1%.
I would imagine a Brewmaster's dps could skyrocket with this, though.

It can proc in low level raids/dungeons so if you are planning to pull a large group probably unequip. It seems to deal AoE damage too as I was farming Naxx and it would kill trash for me. No trinkets or other spells as a Paladin considered damaging via Recount addon.

As a prot Warrior doing essentially / exclusively mythic+ dungeons, I decided to go with this neckpiece, titanforged at 870ilevel.
Of course, it has some issues like a subpar primary stat ( crit ), a damage component that does not scale with ilevel, and, since Kharazan MM+ isn't available yet, a very low base ilevel meaning it would be harder to get it to proc to the same level as MM+ 10/12 potential gear ( 880-885 ).
However it very consistently generates 4 to 5% of my dps, effectively helping in maintaining threat, especially during Skittish weeks which are an absolute nightmare for us Warriors. Paired with a mythic

trinket, which amounts to 10% of my dps, it's a easy, "I don' even have to care", 15% total free dps and threat.The next thing would be to weight it against some optimal, haste/mastery/versatility piece of gear but I'm afraid I'm not knowledgeable enough to do the maths.

like Barbed Reins is a better deal, as long as its ilevel is not too low - I would actually give a 10ilevel value to the damage component itself.
A word of advice though : be wary of Necrotic weeks ! The damage CAN proc from the dot and WILL make neutral ennemies hostile when getting too close to them. This is especially true for Wrath of Azshara and the various neutral monsters there : snails, seagulls, droplets... And more often than not, I find them to be much more of a nuisance than the regular foes, either because of high healthpool, disorient, or slow effects.

Wowhead shows the procs damage scaling with ilvl, which is not the case! It only scales with passive weapon trait damage bonus (27% at lvl 54), versatility, and temporary damage buffs such as

or
沉睡者之怒
; but the base damage is always the same (15k), regardless of whether it's 860 or 925.

I've got a sneaking suspicion that this trinket is affected by AoE damage caps: I tested this neck by myself on N Skorpyron with all of his scorpion adds, and I saw a minimum damage value of 8.7k, significantly below the stated 23.2k damage value on an ilevel 845 version of the neck (though the average damage was 22.6k, which is close to the stated damage value. Hmm.). The fact that the proc is Physical damage would explain some of the missing damage, since Physical damage is affected by armor.
In any case, to put it in perspective: My brambles did more damage than the neck.
Still, the neck is no slouch in the dps department on a mass aoe fight like Skorpyron's fun strat -- it pulled 308k dps
by itself
on the best skada log I had.

Randomly Procs on neutral/hostile mobs which causes combat to initiate, so not just when taking damage. I've watched it kill critters too. Procs off of damaging auras. TAKE OFF WHILE FIGHTING

in BWL. I did not appreciate the repair bill I incurred before I figured out the choker was one shotting this boss trigger his insta-kill mechanic. The boss would just run up and die before any attack animation was performed, no damage to me was listed in the logs.

For Shadowlands Season 4, the effect on this necklace wasn't increased whatsoever. At 288 item level, It did 0.0% of my overall damage or 7.1k total in a roughly 30-minute dungeon. Is this a huge oversight or is it intentional?
