The Devilsaur of Un'goro
- Players in their
early level 50s
may or may not encounter this deadly monster, the
Devilsaur
has a well-known habit of tip-toeing up on players who do not look behind them and wiping them badly. The Devilsaur was originally needed for a quest for druids.
Fel Reaver of Hell
- Anyone
starting in Outlands
will always be seeing this mechanical demon, FR's target range is massive. Like the Devilsaur, they have a habit of rogue-jumping and instantly destroying players, before they do, they unleash a disturbing cry. A
Fel Reaver
's defeat was required for the acquiring of the
Shattered Halls key
, but because of keyring and locked door nerf; Fel Reaver has no purpose but to squish unsuspecting pre-70s.
The Whale Shark and the deep ocean
- If you have Thalassophobia (fear of the ocean and certain bodies of water) then
Vashj'ir
is inevitably a large adjustment if you don't want to do
Hyjal
. Swimming through the passages of this ocean world is the
Whale Shark
, the most frightening mob to date that is the subject of an achievement, if you are going for this achievement then kiting the swimming monster is necessary. If you loathe the underwater zone for it's deep blue and void-like reach then turn your view distance up and always stay to the ground. Anyone who dislikes Vashj'ir was generally subject to
Snacker the shark
from Banjo Kazooie or
Unagi the purple Eel from Super Mario 64
and those games' respective areas.
The Nerubians and Qiraji
- If you really hate bugs, then you're not going to like these buggy folk. The
Nerubians
live in the spidery
Azjol-Nerub
and
Ahn'kahet
, they are essentially spiders and unpleasant for people with Arachnophobia. The
Qiraji
are an array of different but similar-looking bugs that thrive in the desserts of
Silithus
where their home
Ahn'
Qiraj
lies. If you happen to be doing either of these dungeons, do not pay attention to environment if you dislike gross things. The Nerubians & Qiraji originate from a bug race called the
Silithid
and were known as
Aqir
.

Whale Shark and the deep ocean should be Devil Shark and the Deep Dark Ocean, imo.
Kudos to those that get the reference.

You left out one of the best scary things in early WoW: Stitches! It's still one of my fondest WoW memories, being ambushed by him in the little shack behind Raven Hill Cemetery (it was my first character, so I had no experience with Undercity abominations) and beaten like a rented goalie.
Other potential candidates for this guide, some of which might have been nerfed or removed by now:
- The Sons of Arugal (are they even still there, wandering around Silverpine Forest? If so, are they still elite?)
- Mor'ladim in Duskwood (another one known for jumping the unaware and stealing their lunch money)
- The Room of Upside Down Sinners, in the crypts under Karazhan (although I've heard it's a bannin' to get there anymore, so maybe it's best just not to mention it at all)
- The Spooky Goldshire Children.
- The Storm Giant in Howling Fjord (who functions similarly to the Fel Reaver, just so you don't feel lonely when you swap from Outland to Northrend).

@above: It's fine to go there, they'll just port you out at most, it's not like its game-breaking.
Cool idea, could use a lot of fleshing out though. There's a ton more scary things like the Karazhan Crypts, the voices in Naxx, a Deathwing strafing, and many more.

Don't forget to mention the blinking portrait in Gilneas area.

Not all that scary, but things that are kinda creepy looking in-game:
-Some of the rugs found in Orgrimmar and other Orc inhabited places, which consist of a skinned centaur, with eyeless sockets looking ahead. Pretty gruesome in it's own way.
-The frames with leather drying on them found near many Ogre settlements and caves, which could easily be a humanoid skin. :O
-The Goblin wig stands near any Barber look pretty demented to me with their creeper smiles.
Granted these aren't really what I would call scary, but they do tend to have a sort of general creepy factor to them. :P

The voices in throne hallway in Undercity when you set volume to max you hear Arthas, Medivh and Arthas's dad voices

You seemed to have left out the truly scariest thing in WoW
...
Goldshire on the Moon Guard US server.

the only thing that kind of gave me the creeps in the whole game is a section under undercity where they cut to pieces those pink giants with spikes on them and you can see their guts D:

I agree that Son of Arugal should be noted, if only for historical reference.
The crypt at Raven Hill (notably the root tunnel) is also scary.
Going north from Redridge Mountains into Burning Steppes and getting banked by about 20 skull-level demons was also a scare.
Fun guide.

The quest
Discretion is Key
in Hillsbrad was particularly eerie if you turned the volume up and listened to the buried men screaming in the field.
