If the 30 stacks already listed is not a placeholder number and the tooltips for Necrotic Plague that Bareno linked to remain unchanged, it looks as though you'll have to wait until that DoT kills at least 29 different people before you kill Arthas.
It also looks as though most raid members would die to just a few stacks of this (assuming each stack is worth 10k per tick). So if it jumps to a raid member anywhere between 3 and 30 stacks, it will need to be dispelled IMMEDIATELY, which will drop a stack and it will need to kill another NPC to make up the difference.
This achievement is going to be a beast.

Assuming the tooltip for Necrotic Plague goes unchanged when the new wing opens and Arthas himself is actually live (there is a good chance of it changing), there will most likely be a damage reducing buff of some sort, as we've seen in both the Festergut and Blood Prince encounters.

Purely speculation but...
Now that the encounter is actually live, let's take a look at the mechanics of both the fight and
Necrotic Plague
:
1. We know that its effect ticks every 5 seconds.
2. It can affect both players and npcs (unknown if it affects The Lich King himself but for hypothetical purposes lets just say it does)
3. The Lich King summons adds (Drudge Ghouls and Shambling Horrors in phase 1, Valkyr Shadowguards in phase 2, and Vile Spirits in phase 3)
4. If its effect ends via death or the duration running out, it gains a stack and jumps.
5. If it is dispelled, it will lose a stack and jump.
Here is how this achievement might possibly be done:
During his first Necrotic Plague use, if one of the tanks are hit by it then it MUST be dispelled and you must wait for it to be used again. If a dps is hit by it, they MUST use anything to reduce damage every time it ticks and the raid healer MUST be ready for it. It has to gain one stack so that when it jumps, it will be dispellable but still able to jump (instead of being dispelled completely from having no stacks).
When it does stack to 2 and jump, one of two things will happen.
-If it jumps to a player then it's at 2 or more stacks, you MUST dispel, otherwise you will have to either tough it out at a single stack or dispel and wait for him to use it again.
-If it jumps to an add, wait until there's another add and then ALL DPS BURN THE ADD WITH NECROTIC PLAGUE. It will then gain a stack and hopefully jump to another add.
-The ideal time to go for this achievement would either be during phases 1 or 3, since those are the times when he summons the most adds, thus giving more chances to stack the plague. It is of course advised to STOP DPS ON THE LICH KING and focus on bringing Necrotic Plague's stacks up in this way during the phase that you attempt the achievement. (Of course if there is a berserk timer then you must either choose to wipe the attempt or just kill him without the achievement if this will take too long).
Of course, as I write this I am writing this under the assumption that the raid won't have the zonewide buffs that are, to my limited knowledge, not available to players yet (though I could be wrong). This buff that was stated to be put into effect in Icecrown will possibly either trivialize the achievement or be necessary to gain said achievement (there is also the possibility that it will do both).

Facts
#1: Necrotic Plague is only casted by Lich King during P1.
#2: Necrotic Plague can jump to either npc or player.
#3: Stacking to 30 means it can only be done on 2 tanks after a stack of 20.
A true test for
DPS(take down adds as fast as possible to let the tanks have room to breathe)
Tank(not only the Necrotic Plague stack is doing huge damage on a tank, Lich King gains enough +dmg to give tanks a big kiss)
Healer(well, just stressful. One of the healers in my group is a girl, you'd expect to hear girl's screaming from voice-chat all the night.)

The way we've done the achievement is:
Off-tank gets all adds tricked by a rogue without anyone else dealing damage to them.
First plague runs out of the raid and gets dispelled somewhere so the first plague disappears. We've found this to be the most effective way as we've lost our stacks before on not having enough ghouls at around 25-28 stacks.
After the first plague, everyone else just runs to the OT just like normal and then basically you wait. You wait pretty long and the only important part in this is to make sure the ghouls get tricked to the OT immediately in the end since it will be fairly close once you get 30 stacks.
After P1 just make sure your DPS are giving everything they got and finish him off. A log of the kill including this achievement can be found

Basically: DPS Lich King to 78% --> all DPS and healers back away from the Lich King, all adds are tanked on LK --> Necrotic plague hits 30 stacks after killing tons of adds --> Pull adds away and kill boss as usual.

Its isnt soo hard, just let first plague be dispeled out of raid, dps Lk down to 72% and be carefull with enrages on the adds. We had hunter + rogue on that. Enrage is fine when all ppls stays alive.

Yeah it wasn't so bad, we found even if someone mucked up running out if we let 1 or 2 following go out before resuming dispels near OT we were right, so there's a bit of flexibility.
Managed to get this on my 1st LK kill (4 attempts) as well so I was happy.

There's one issue with this achievement my 10 man group ran into tonight. Unfortunately it was the last achievement many of us needed for our frostwyrms.

(this is a total of 1,450,000, or 29 stacks)

So if the Necrotic Plauge *SKIPS* 30 (i.e. combines with a second stack) the achievement is not triggered.
I suspect that the code for the achievement trigger is listed for (stacks = 30) rather than (stacks >= 30)
For anyone attempting to do this achievement, beware. It looks as though the stack count must equal 30 in order for the achievement to be triggered. This has been posted on the BUG forums and I've opened up a ticket with Blizzard about it.

unless some guy on Wowhead explicitly warns you otherwise.
That being said, the strategy for obtaining this achievement can be done a variety of ways, but the primary thing you need to know is this:
Get the
Necrotic Plague
onto the adds as late in as possible during phase 1, without pushing the Lich King into phase 2 (meaning, without getting him to 70% or lower). This means you're going to want to dispel
Necrotic Plague
on the first person it comes out on while they're standing away from the raid (and away from adds), thereby eradicating the first plague. You can treat the rest of the plagues like normal at that point, since the plagues--while they might initially be separate stacks--will eventually jump onto the same add and
fuse their stacks
. Anyway, what you have to have happen is have the plague jump from add to add (as adds continually spawn during phase 1), and avoid having to dispel it off of friendly players as much as possible, BECAUSE! each dispel removes a stack of
Necrotic Plague
. Additionally, each dispel will add a jump, thereby increasing the damage the Lich King does through
Plague Siphon
, so you definitely have motive to avoid that scenario as much as possible.
One last thing--the ranks of
Necrotic Plague
listed elsewhere on this page for this achievement are
WRONG
. The rank I listed is the one for 10 man regular and if you're pursuing the 10 man meta and aren't doing it while fighting Lich King Heroic 10,
this
is the accurate rank. Just my two cents.

Edit: Needless to say, this was done at level 80 when Lich King was endgame. I suppose at level 85 it's pretty trivial since phase 1 was easily pushed with 25m hc gear and

We usually have a hunter and rogue with us, but their internet was down so we missed
Misdirection
and
Tricks of the Trade
.
In order to work around this, I as the retribution paladin, taunted/Divene Stormed the ghouls of Lich King while standing between our OT and MT running back and forth between my spot and Lich King as needed, like this:
OT------------ me <--kiting ghouls--> MT
(depends on how you position your raid ofc)
Our OT paladin then used, among others,
Righteous Defense
on me to get the ghouls.
It turned out to be a great workaround, we oneshot the achivement, thanks to the 20% zone buff and ICC25HM gear, my dps was not needed at all on LK.
Infact we got LK to 72% very fast and our ranged were basicly having a coffee break untill we got 30 stacks.
We did not delay the plague, we were able to keep the plague "alive" with me kiting the ghouls to our OT.
Remember, as soon as one you see 30 stacks, the achivement is active and you just proceed to kill Lich King as usual.

Our Strategy: During phase one, we had our tanks cross Vigilance to reduce the high amount of tank damage as well as allow our add tank to machine-gun taunt the ghouls/shamblers.
We had everyone stack up about 20 yards away from the add tank, which was about 30 yards away from the LK tank. The end result was a pretty tight triangle.
Our first plague was run out about 10-15 yards behind the group. All consecutive plagues were cleansed onto the adds as usual. Our ranged DPS just kept DoTs on King and casually DPS'd him. Make sure to stop DPS at around 72%. A holy paladin+crossing Vig made healing both tanks pretty easy. We also had our Destro lock Shadowfury whenever there were two enraged Shamblers.
Once you see 30 stacks on any of the adds, push the phase and continue on to make a normal Lich King kill. With the ICC buff at 20% enrage won't ever be an issue, we were a whole 2:30 ahead of it despite the long first phase/3 healing.

The stack doesn't disappear when it gets to 30, keeps on ticking.
Reason i say that is because it looked like it disappeared when we hit 30 (someone was fast with dispelling)
Now you know!
Edit: It is really recommended that you dispell the first two plagues outside the raid (nothing gets it) if you want this achievement.

This is simple to get. Here is how:
Raid Comp:
This actually does not matter a whole lot. You want a disc priest per normal LK mechanics, but really any normal raid comp will work. Some groups may want a DK Tank for easier add control, however a blocking tank will take less damage and thus require less healing. I highly recommend running a 3 healer comp. Obviously you want a group of people with a decent amount of Lich King experience so you are not dying in later phases after the time investment in phase 1.
Process:
1. Stack ranged and healers together in a tight clump. Have the OT tank the adds roughly 20-25 yards away from the raid.
2. In our example, the OT was to the left of the raid. The first person with the plague runs to the RIGHT and is dispelled.
3. The second plague goes out, this person runs behind the adds being tanked by the OT, is dispelled, and returns.
4. Every other plague receiver runs RIGHT to be dispelled away from the adds and raid. You DO NOT want 2 plagues in the adds if possible, it produces a chance of failing the achievement.
5. Burn LK down to about 72% then have DPS hard stop.
6. The plague continues to bounce after LK is pushed. You want to allow the plague to bounce to 30 less the number of adds you have up then push, maybe giving an extra 1-2 just in case. Normally, you can push LK at about 25-28 stacks depending on how many adds you have up.
7. The rest of the fight is exactly as normal.
What this does:
- Ensures that you do not merge plagues and accidentally go from 29-31 stacks (you must hit 30 exactly or you will not receive credit).
- Simple execution. Your raid members only need to be able to pay attention to DBM (or whatever they are using), count, and discern left from right. If members of your raid struggle with this, odds are you will struggle with basic fight mechanics later on.
- Ensures that you will not run out of adds before that plague has stacked high enough.
Your aim is to stack
Necrotic Plague
to 30 stacks, this has nothing to do with the
Plague Siphon
debuff on the Lich King. Simply stated, you want to track the debuff that is bouncing around, not the buff on the Lich King.

I got this today on my Death Knight, the plague was at at least 33 stacks, and seeing the number of adds up it was probably at about 40+ by the time he entered Phase 1.5.
I'm willing to bet the coding for the achievement's been changed to >=30 instead of =30 now.
Also, is there some kinda reference I'm not getting in the name of this achievement?

Knocked this out today with an all 85 group. We didn't dps adds at all and let the OT just pick them all up. Once the plague gets to around 20 stacks, it's killing the adds pretty quick. After 30 stacks, it's gravy.

Finally got this one and my drake.
It's not that easy even at 85 with full DS Gear. As gear does mean anything only how fast u will finish LK actually after the hardest part of the achievement.
Done in a pug, spamming trade channel. After about 5-6 fails, we changed tacts a bit. We were always run out of adds and basically loose stacks. Decided to change tacts a bit: we've skipped 1st
four
necrotic plagues, dispelling it out of group. This achievement really requires much concentration from all members - to track the Necrotic Plague and also its stacks. At lower stacks, pretty hard to find the mob with stacks in all that mess. At high 20+, much easier, as small mobs got one-shotted by this desease and big adds come 2-shotted.
The reason our group needed to skip just as much as 1st four plagues is that our add tank was druid, and for gaining enough aggro, needed to do AOE dmg (Swipe,Thrash) sometimes, which eventually kill the small adds fast enough.
Also very important thing is really to dispell this from people, as in first tries our dudu healer got one-shotted from 285K hit.
Maybe this will help someone.

Did this last night and despite all the inormation above about hitting exactly "30 stacks" of Necrotic Plague, we completed this achievement with 34 stacks and then burned down the ol' LK. Idk why this worked for us and did not work for other groups, but we followed a similar method as stated above. I notified Blizz on behalf of all the folks on here that did not get the achievement in hopes that whatever bug allows some groups to nail it and others not, to be fixed. Completed this last night, got it on our second try.
-Make sure you have a good tank picking up the adds who picks them up fast. If you have 29 stacks on the mobs, and there's no mob to let it jump to, it'll reset. (that's what ours did)
1) Have MT tanking LK (with the whole raid with him, except OT of course) on the left side (facing the LK) of the stairs.
2) Have OT tanking adds on the right side (facing the LK) of the stairs.
3) The FIRST person to get the plague runs to the OT, behind the adds, and gets dispelled.
4) Everyone else runs to the CENTER of the platform (away from both the raid and the OT), and is dispelled there, making the plague disappear.
5) The one stack in the adds will keep bouncing around. It took us around 4-5 minutes to get it stacked up to 30. It really goes fast once it starts getting to higher numbers and one-shots the mobs.
6) Once it stacks to 30, you may kill the adds, whatever you want to do. We just burned him afterwards, and even got Neck Deep in Vile. (We did this on NORMAL mode! Please do not try to do this achievement and Heroic at the same time, unless your raid members are extremely geared and extremely raid aware.)

At level 85 there is a way to get this achieve first time, every time, as long as your raid is competent.
You will need, a pally or a priest healer and one tank.
Have your raid in the formation in the link.

You must start the encounter with only the tank doing any damage, he should wait at the center (red) and gather at least 10 adds, (this is to avoid the issue of the adds running out of adds that seems to occur at around 25 stacks).
All this while necrotic plague victims should run away from the group (purple) and only then should they be dispelled. There should be no rush in dispelling one stack necrotic plagues as it only does 50,000 damage.
Once the tank is ready, he should run to purple and pick up the dispelled necrotic plague, once the adds have the debuff he should return to his position and keep picking up the drudge ghouls and shambling horrors, the plague will easily increase to 30 stacks, when that happens, kill him as usual.

this is a fairly simple achievement that we five manned in a guild group to cut down on pugs who run around saying "derp dispel meh!". make sure someone can dispel diseases and that you have 2 tanks to make things easier. also a healer of course.
now our strategy was having 4 markers in a square shape almost. we had 1 tank with the LK in the center of the room only have them use white swings if you are doing this achieve on normal. we had our other tank (we used a DK for the DnD) stand where you teleport in at tanking the adds. we had the rest of the raid standing near the pillar to the right of teleporting in. our last marker was the dispel area that people with necrotic plague would run to we had it about 10-15 yards from the raid group and the tank tanking the LK. the first necrotic plague goes to the dispel area and the second goes to the adds. all after that are going to the dispel area. we did this so that we didnt run out of adds because necrotic plague started killing them too quick. make sure to dispel the OT if they get necrotic plague so they don't die.
it takes a while but its worth it. after the plague gets it's 30 stacks. kill all adds and dispel necrotic plagues away from everyone and start burning boss. then its just LK as usual.

Just completed this achievement in patch 5.0.5
After quite a few attempts and only just missing it by 5 stacks we were able to do it using this strat.
MT tanks LK as normal with OT between MT and stacked raid grp. OT picks up adds with one AOE ability while taking care to not damage adds currently on OT. Raid run dispels to a designated spot until OT has 4-5 sets of adds (12-15 drudge's).
Once OT has enough adds, one raid member gets dispelled on OT to start the stacks bouncing. Rest of the dispels after that are done as before on the designated location. This ensures that there are not multiple debuffs being bounced. Once 30 stacks has been reached kill boss as per normal.
We did this with a DK OT and Pally MT so that DnD could be used to pick up adds without spreading plagues or over killing the adds before the debuff could work its magic.
With the CD on dispel we had to communicate on vent so as to ensure everyone could be dispelled. We also did this with the 30% raid buff turned off to ensure the adds didnt die to quick as everyone in raid was in HM DS gear.

Got the stack to 32, blood dk died because healer didn't dispel plague fast enough, then healer died from the plague as well, but the rest still managed to do the rest of the fight with ease. After LK died no one got the achievement... Guess they didn't fix that bug where the stack has to be exactly 30 after all. See you next week arthas...

Here's my strategy for level 90's. You will need 2 tanks, at least 1 healer that can cleanse diseases, and as many DPS as you can get. (My group only had one healer who was also the only person who could cleanse necrotic plague)
You will need to setup four locations, a main-tank location (MT), off-tank location (OT), raid group location (RG), and dispel location (D). I recommend the setup to be like this:

In case you don't know how necrotic plague works: It is a disease that is periodically cast on a random raid member throughout the duration of phase 1. If necrotic plague is dispelled or the entity (player or non-player) with necrotic plague dies, necrotic plague will attempt to jump to the closest entity (player or non-player). If there was an entity close enough, they will gain the necrotic plague with an extra stack, otherwise the necrotic plague will have nowhere to go, and it will simply vanish. When the necrotic plague gets enough stacks, it will start doing damage well over anybody's maximum health.

Have nobody DPS the Lich King at all, only have the main-tank gather some initial threat and auto-attack from there. The off-tank should pick up every single add that spawns and attempt to do as little damage to them as possible. All raid members besides the main-tank and off-tank should be grouped at (RG) with all DPS sitting idle and ready to move to (D) to be cleansed.
For the first 2-3 necrotic plagues that go out, have the afflicted raid group member run out to (D), be dispelled there, then run back to (RG). For the 3rd-4th necrotic plague, have the afflicted raid group member run to (OT), be dispelled near the adds, then run back to (RG). This will allow a decent number of adds to spawn before starting the necrotic plague stacks. After a stack of necrotic plague has been started in the add group, have any new necrotic plague be dispelled at (D) like before (this will ensure that only one necrotic plague stack is building up). At this point, raid members capable of cleansing necrotic plague should keep an eye on the off-tank to dispel him in the case that the necrotic plague stack jumps to him. Your off-tank should continue to pick up any adds that spawn until necrotic plague reaches 30 stacks.
Once necrotic plague has reached 30 stacks, have your main-tank pick up any new adds that spawn from this point forward, and have your off-tank and all ranged DPS kill the adds that your off-tank already has all while having everybody remain at (RG) (Melee DPS will continue to do nothing; future stacks of necrotic plague must still be dealt with as usual). Necrotic plague may jump to your off-tank multiple times as the adds are killed, make sure to cleanse him quickly. Once all of your off-tank's adds are dead, your off-tank will be left with the final amount of necrotic plague that WILL kill him if it ticks, simply dispel your off-tank (who should have no adds or players near him) one last time to remove the high stack necrotic plague from the encounter. From here you should have your ranged DPS push the Lich King into phase 2 (you can have your melee DPS join in as well, as long as they continue to deal with new necrotic plague).
From this point you simply fight the Lich King as you normally would. Oh, and make sure you don't do this on heroic mode, as it makes the achievement much harder (or nearly impossible) and this strategy would not work.

Just completed this achievement for the mount. I believe the easiest way is to follow the steps below:
1) Tank Lich King on the right side of the stairs. The rest of the group stack on the tank and DPS LK to around 75%
2) An OT tanks the adds on the left side of the stairs. We used a warrior tank for tanking LK and a DK tank for tanking the adds. DK tank was able to get all the adds as soon as they spawn.
3) The first person who get Necrotic Plague goes next to OT and gets dispelled and then returns back to the group. This needs to be done only once. People who get the plague later during the fight goes in to the middle and gets dispelled there.
4) Keep doing this until the tank or one of the adds gets 30 stacks then DPS LK to the second phase.
5) The rest is easy, just kill him.

Two-manned this today on a level 90 Blood Death Knight tank (in DPS gear) and Mistweaver Monk healer, approximately ilevel 470 gear. Never been to this raid or read anything about the fights prior to tonight, and wanted to see how far we could get on the achievements for the mount.
Have the tank grab the boss and hold all of his adds near the base of the stairs. The healer can stand in the center area at nearly maximum range just to be safe. The boss will ONLY ever cast fresh stacks of Necrotic Plague on the healer, who stands far enough away so that the existing stacks on the adds will not bounce to her. The tank is only damaging the boss and the adds enough to keep aggro on them so they're nice and grouped at the stairs, and DO NOT damage the boss below 70% or he will transition into phase 2 and the adds will stop spawning.
Necrotic Plague
will bounce to a nearby friendly or enemy target when it is dispelled, expires, or kills its target. If it is dispelled, its stack is reduced by 1 (unless of course there is only one stack -- it will bounce as a single stack rather than being destroyed). If it expires or kills its target, the stack increases by 1. It deals 50,000 damage every 5 seconds for 15 seconds for
each stack
of the debuff that is present. This means that by the time it starts to approach the number of stacks needed for this achievement, it will be dealing over a million damage per tick and will one-shot any player after 5 seconds, even at 90. (There is a possibility that future expansions will give us enough health to survive this, but I'm not expecting it any time soon.)
The first time Necrotic Plague appears, have the healer dispel it right away while standing far from the adds, so that it disappears, or just let it tick while healing through it. It took us four or five attempts to figure this one out, because many times, the plague would kill all of the ghouls before the boss summoned more of them, and then the stacks wouldn't have any more targets to jump to and would disappear entirely as soon as it's dispelled (it can't bounce to the boss himself, and this is good, because otherwise it would deal a ton of damage and we'd trigger the phase transition right away). Giving him more time to summon adds at the start of the fight helps ensure that there are enough of them to last through the end of the phase, even when the plague starts one-shotting the ghouls every 5 seconds.
Each time the stack jumps, the boss gains a +2% stacking damage buff, and he'll be dealing 60% more damage at the very least when you're done, so there's no reason to start stacking the plague again if it falls. Better to just wipe the fight and try again.
On the second Necrotic Plague, the healer runs in to the group of adds, dispels the disease, then runs back to the center of the platform. It is
extremely important
to watch the tank very very carefully for the rest of the phase. If the plague bounces to the tank, dispel it off
immediately
or it will deal massive damage and possibly one-shot the tank. If the very first one bounces from the healer to the tank, it's okay to just let it tick because it's still low enough. But once it's in the double digits, you really can't let it tick at all.
For the third Necrotic Plague, and every other one that appears on the healer from this point on, just let it tick and heal through the damage. Due to the new dispel mechanics, dispels have an 8 second cooldown. If the healer dispels the easily-ignored plague off of themselves, and the plague on the adds happens to bounce to the tank soon afterwards, that is going to result in a wipe. Best not to risk it.
Continue watching for plague on the tank and keeping both players healed up, to prevent too much damage from
Infest
, which is removed when the target is above 90% health.
Keep tab-targeting through the adds to keep track of how many stacks there are, and as soon as it hits 30, have the tank start damaging the boss to bring him down to 70% and trigger the phase 2 transition. The healer can use whatever ranged attacks she has at this point to pick off the weaker ghouls, to reduce their numbers and give fewer targets for the plague to keep bouncing between. Continue to be very careful about dispelling diseases off of the tank during the transition, as there will still be some adds up. It will probably jump to the tank at some point during this transition, and need to be dispelled off in order to get rid of it completely. Make sure the healer continues standing a distance away from the tank until the disease is gone -- you don't want a 35-stack plague jumping from the tank to the healer while her dispel is still on cooldown.
The damage the boss is dealing to the entire platform during this time can easily be ignored, but watch for the
Ice Spheres
that spawn every few seconds. If they get close enough to their target, they will explode, knocking anyone nearby off of the platform to their death. They have very little health, so a single death coil or jade lightning tick will destroy them. Make sure you stay away from the outer ring of the platform, as he will cause it to fall at the end of this transition and you'd better not be standing on it when that happens (not hard to avoid).
During the second phase, the valkyr will grab the healer (I assume the tank won't ever get grabbed, every time we did it it was the healer) so the DK should be specced into various stuns in order to slow the valkyr enough to kill it before it carries the healer off the edge to her death. This should only happen once during phase 2, as the boss will be taking enough damage to go into the next phase transition soon afterwards, which works the same way as the previous one.
Phase 3 should be quick and easy -- we brought him down to 8% before any of the Vile Spirits even showed up and before he did this whole "transporting us into Frostmourne so we have to help rescue his father" thing I was reading so much about, and ended up getting the
Neck-Deep in Vile
achievement as well without even trying for it. I saw the Vile Spirits floating around during the RP part of the fight, but with both of us dead, they didn't have anything to attack, and eventually they just died and fell back down to the platform.

Just some tips for who is going to do this in MoP at level 90, since dispels have 8 seconds cooldown:
- Before dispelling Necrotic Plague near mobs to stack it, we waited 6 casts of the disease to gather up enough mobs to ensure 30 stacks were possible
- Dps must DO NOTHING and stand more than 20 yards from adds, until 30 stacks are done. You dont want to aggro a stacked mob away from the group
- You can do it with only one tank, just be sure to keep all mobs close because they will aggro the healer
After you get 30 stack, you are done, dps the Lich King, kill orbs at 70% to avoid knockback, you usually are so fast you even skip Valkyrie phase, and thats it, GZ! :)

Had a rogue stand around and do nothing until Lich King was down to 70%, just to make the rest of the fight a little easier. Healing priest stood about 25 yds away from both me and the rogue (forming a triangle,) dealing with the dispels.
We waited until there were three sets of adds spawned before we started worrying about letting the plague stacks add up. Took us nearly a dozen tries to figure out that we weren't having enough adds out to keep the stacks bouncing around on them. The more adds you can handle (without doing enough damage to them to kill them yourself) the less likely you are to have the plague jump onto the tank (the demo lock, in this case.) As soon as the stacks hit 30 we killed all the adds and took the Lich King down to 70% (he was usually around 75% by the time we got to 30 from trying to keep up my Demonic Fury to use my damage-absorbing ability that I can't remember the name of) and dealt with the rest of the fight as you normally would - rogue dealing with ghost adds, lock dealing with ice spheres, and having a dead priest cheering us on. (We lost him to a valkyr on our successful attempt - totally my fault because I suck at demo.)

So we tried to 3-man it (resto shammy, assa rogue and bm monk), lvl90 all of us. Everything went fine until we reached 20-21 stacks. At that point another necrotic plague has appeared and there were 2 plagues on 2 players, which means I was unable to dispell both cause of the 8 sec cooldown. I've checked this skill of the boss here and on other sites two, but couldn't find any information about a 2nd necrotic plague. Can anyone confirm this? Or was I blind and simply didn't find it? By the way any tipps for the above setup is appreciated :) Thanks in advance.

Trio'd this last night brewmaster monk, prot warrior, holy paladin. Brewmaster monk picked up all the adds with dizzying haze as not to damage them. After about 4 of the big adds (shambling horror?) had spawned we then dropped the necrotic plague into the add swarm.
To avoid dropping plague into the adds too early the monk would roll/teleport away from adds if LK put necrotic plague on him then detoxed himself. Prot war tanked LK down to about 78% for easy phase push. After all adds were dead (post 30 stacks) the paladin cleansed monk and fight proceeded as normal.

FOR PEOPLE WANTING TO DO THIS AT LEVEL 90 AND STRUGGLING!
It took us a lot of tweaking to get this at level 90, but it IS possible! We had a huge issue of having enough adds, even after completely eradicating the first Necrotic Plague - simply, this was due to the accumulated damage our OT was having to do in order to aggro the Adds!
We used two tanks (One Pala, One DK) and one Healer (Priest in our case) and only had a couple of DPS. The way we did it in the end was a variation on a tactic I found on here.
1.
Position your Off tank on the left of the platform, about 25 yds from your Main Tank who should tank LK in the centre of the platform.
2.
Have your DPS and Healer(s) stacked together, another 25 yds away from each of these and mark a 3rd area on the right side of the platform.
3.
Have the first FOUR people afflicted with Necrotic Plague run to the 3rd Marker (right side of platform) to be dispelled and get rid of the first 4 plagues completely. During this time, your Off Tank can gather up all adds that spawn (DPS should kill Shambling Horrors as per normal).
4.
The FIFTH person to be afflicted with the Plague should run to the Off Tank to be dispelled. Everyone after that should continue to be dispelled on the right side of the platform.
5.
Sit back, watch the stacks accumulate without killing any adds (other than Shambling Horrors) and when you hit 30+, nuke down Lich King as per normal :)

Pretty easy to solo this achieve as a holy priest. Necrotic Plague will not target you alone, so it was simple to wait until the second shambler, then pop shadowfiend+fade. As long as the LK wasn't in the middle of summoning ghouls he'd throw out the plague immediately. Get at least 30 stacks (I ended up with 40 and got credit, though that's quite a bit of a damage buff), flip to Chakra:Chastise and push the phase so he loses his damage buff
The rest of the fight was simple, Raging Spirits can silence but they have a hard time hitting level 90s, Val'kyr won't pick you up, you know exactly who's getting Defile, and you don't get sucked into Frostmourne while alone. There's no dps race at all.
Got so into it I panicked when Spirit of Redemption popped up, thought I left an Infest ticking again and screwed it up. :D

Was told last week by a GM that the stacks of the plague have to be exactly 30 to get the achievement. :/ Now reading wowhead and seeing people having 45 makes me want to go punch a kitten T_T adds died at 33 stacks and still no achievement for me.

So I just recently got this achievement with 3 people
1x Restoration Shaman
1x Frost Mage
1x Survival Hunter
Note we are all PvP geared and the strategy we developed is based entirely around the healer.
I as the restoration shaman would simply tank the Lich King and his Ghouls and let the plague jump between the ghouls and kill them off while gaining stacks, if the plague jumped to me and killed me I'd pop reincarnation, after that I never received the debuff again. So once the stacks get to 30 I would run as fast as possible and have my two dps nuke the infected mob down first and if a big enough gap was created the plague would fall off and if it was close enough to another ghoul they simply continue to nuke through the ghouls until the plague vanished. We then proceed through the fight as per normal and we win.

We waited til about 5 spawns of ghouls until we let the plague stack. Ran into a glitch where Tirion didnt spawn, so we were all stuck dead. I was able to release and come bk in. There were still adds left over, killed them fixed the glitch!

Just did this with 4 lvl 90's. Me as a 537 moonkin, a 505 Holy Pali a 480-ish lock and a 480-ish ele shaman. As other comments noted, we dispelled the first away from the group and had the second one bounce around the melee, as the holy pali heal-tanked. Worked like a charm, holy pali never went oom and once we hit 30, knocked LK down, and killed him before any phase transition was done. Easy peasy at 90.

For some time we were trying to get the stacks onto someone for the transition because we had miss-read the achievement! (This is two people at level 90 doing this)
Just in case anyone else is interpreting it the same way we did: you DO NOT need to have the stacks on a player for the transition.
So in other words:
Boss > 70% health - You always have the stacks on an add and dispell any time that the plague stacks are on a person
Stacks reach > 30 on an add - DPS boss below 70%
Boss < 70% health - adds die off and no more are spawned. Anyone getting the stacks should be dispelled and the plague will eventually have nowhere to jump to and disappear.

This can work with 7 people. Have your tank take all the mobs onto himself. Have the rest of the team stand a good distance away and try to avoid dps or healing or you will pull aggro. Also you don't need to do this on heroic to get the achievement! Good Luck!

Did this tonight dualboxing on a blood DK and fistweaving monk. I also had two DPS warriors and one resto druid on my team.
As my DK, I took all the phase 1 adds off to one side, while one of the warriors off-tanked Arthas to the other side. The druid and my monk stood a distance apart in the center, and the second warrior stood off in a corner while he did his laundry. (No, really.)
I simply had all of the others stand still in their locations until I'd cleansed three or four Plagues on them. On one attempt, my DK was either given an extra infection by Arthas or someone had moved too close (that was before we'd gotten the execution smoothed out) so I tabbed targets until I found the one with the lower stacks of plague, taunted it on my monk, beat it to death, then cleansed myself. At the fourth or fifth plague on the kill, I was lucky enough to have it cast on my monk, so I rolled her to my death knight and cleansed her, then rolled back. I waited on the adds, tabbing through them every time plague jumped from them, as well as cleansing every plague that went out on the party. When I saw one of the adds with a stack of
precisely
30, I called DPS time, and the raid jumped onto Arthas. The plague killed off the adds one by one (growing higher than 30 for a time, too) and I basically tabbed out of the DK entirely to be on plague duty.
When Phase 2 started, Val'kyr rushed in and grabbed one warrior and my monk, and we didn't get slows up in time to prevent both of them from dying. The next two Val'kyr took the other warrior and the druid. From there, I was soloing, thankfully immune to Val'kyr dragging.
My DK isn't set up to DPS, and the Lich King went berserk at 14% or so. I was expecting to die instantly but I had a huge buildup of Blood Shield and it took him some time to cut through it as I furiously jabbed at Soul Reaper, Rune Strike, and Death Strike. When it was finally torn down, he knocked me down to 62% in about one second and then started casting Spirits. Looking up at his frame, I saw 2 seconds left on my most recent Soul Reaper, and 11% HP. I let that Soul Reaper tick off, put up one more, used Blood Boil to refresh his diseases, then used my rocket belt and ran up the throne like I had the lord of undeath himself on my heels... (Oh, right). He ticked down to 10% just after coming out of his cast, and we got the achievement.

Completed this last night with:
1 540 assassination rogue
1 537 shadow priest
1 490 mistweaver monk
I tanked as the rogue. In order to do this without killing everything, I took my weapons off, and used FoK very lightly - only when needed to grab adds as they came up, or if healing aggro pulled them off of me.
The priest's job was to continuously tab-target through the adds to count the stacks of plague. The monk kept me up, and dispelled the plague whenever it landed on a player, with priest as backup dispeller in the event the monk was on CD.
We stood in a triangle so that dispels off players would not land on other players or the adds (except the one time we wanted it to land on the adds).
We let the first 3 plagues get dispelled off without landing on adds, to give the LK time to get plenty of adds out. When the 4th plague went out, whoever it landed on ran to me with the group of adds, and dispelled it off in the middle of the pack. Then back out to triangle formation. Future plagues were just dispelled off without jumping to adds.
When our stacks were at 25, I made sure the next group of adds/shambler came out, FoK'd them to grab them. Then turned my auto-swing off, put on my weapons, and poisoned them. Targeted LK to make sure I was ready to go.
Once the priest counted 30, she and I started dpsing LK. Single target, no AOE, and no cooldowns yet. Priest started heading for edge of platform to not get hit by upcoming AOE.
LK hit 70%, ran to center, and priest and I blew him up with all our cooldowns. I used cloak to prevent getting blasted off of the platform.
We did the normal death-in-the-middle-of-the-encounter, and the RP started. But then, it seemed to get stuck. The LK was just standing there with colorful ribbons coming out of his sword, and nothing was happening. We noticed that a shambler and 2 ghouls were still alive and standing at the edge of the platform, and thought maybe that had bugged it. We all released at the same time, having no idea if we would be able to get back in or not. Fortunately, we were able to. The LK was still standing there with his ribbons of light. We killed the ghouls, and the RP proceeded. We also got the achievement, despite the release/run back. Whew!
Things that failed:
We tried it first with bringing the 3rd plague to the add pack, and found we were running too low on adds by 30 stacks.. the adds were mostly dead and it was jumping to players too much, and killing us.
We tried just all standing together to let the plague jump off us to the adds. It stacked very quickly this way, but also very unpredictably. As others have noted, it has to hit exactly 30 stacks in order to get the achievement. If it jumps from 29 to 31, the achieve will fail. With us all standing together, it would jump too randomly, and the chances of getting an exact 30 were too low. Keeping all the players separate, it stacked nice and evenly, 1 at a time.
The reason we did not AOE after 30 stacks was because if all the adds died before LK hit 70%, then the plague would start jumping between the players and killing us. However, this did lead to the aforementioned bug. If I needed to do it again, I would say single target LK to bring him to 70, as soon as he runs to the center blow up all the adds real quick, THEN blow up LK.
While players of our ilvl can survive the AOE damage at 70% with no problem, the LK will periodically knock you in a random direction if you are standing in it. If you get knocked off the platform, you're done. So it is important for ranged to stand outside of it. If the ranged are strong enough, let them do all the killing (LK goes down really quick at these ilvls). Melee can stay in if they have mitigation (like cloak), or are just willing to take their chances.

Yesterday, I four-manned this with three other 90s. While it is certainly doable by less people, as demonstrated in other comments, we found this to be the easiest method. It only took us two tries and we only failed on the first because someone missed an ice sphere.
We had four players: a druid bear tank (myself), a druid cat DPS who switched between bear and cat form depending upon phase, a holy paladin healer and a retribution paladin DPS. So, one dedicated tank, one switch tank/DPS, one pure DPS and one healer.
The dedicated tank will, upon pulling the Lich King, take him to one side of the stairs, where they will hold aggro throughout the first phase. With a decently geared tank at 90, this should not be a problem. He hits like a kitten. The main tank should require little attention from the healer, who should be focusing mostly on the off-tank. The off-tank will pull ALL of the adds to the other side of the stairs and keep them there, running back now and then to gather the other adds as they spawn. The healer stood in the center of the platform, just close enough to heal and cure the plague when it jumped to the off-tank. The pure DPS had no role in phase one save to stand by the healer in case any adds aggro'd onto him. This was essential for us, as it kept the plague from jumping onto the main tank or healer and potentially causing a wipe.
With this configuration, the goal of phase one is to stack thirty necrotic plagues on the adds. Necrotic plague will jump (and multiply) when it kills a target or gets to the end of its tick, so it's really just a waiting game. Since he had nothing else to do, we had the pure DPS tabbing between the adds and counting the plagues. Do NOT kill the adds. Let them attack the off tank and multiply, as it will decrease the likelihood of a massive stack of plague jumping to the off-tank and killing them. Do not bring the Lich King below 70% health either, as this will trigger phase two and he will cease summoning adds and casting plague.
It should be noted that each plague upon each separate add counts as a separate plague. When they jump together, they will all stack. So if you have five adds up, one with ten plague stacks, one with five, one with seven, one with six and one with two,
you have thirty stacks of plague.
Kill the adds. When the adds die, all of these stacks will stack on one person (the off-tank as long as no one else is nearby). This will trigger the achievement at the end of the fight. Dispell the plagues immediately. Do not let them tick, as they hurt quite a bit, even at level 90. Make sure the off-tank is away from the other members of the group when it is dispelled, so that the plague cannot jump to another group member.
With the plague bit over, pull Arthas to the center of the platform and fight the rest of the battle there. At about 70%, phase two will pop. The edges of the platform will fall. Keep AWAY from them. If you fall, you'll die and you cannot be battle ressurrected. Your body will be out of LoS. At this point, ice spheres will begin to fall from the sky. Do not let these hit you. It's best if you don't let them hit the ground at all. We had the DPS paladin on the ice spheres for the entirety of phase two, rather than fighting the boss. Two druids, with the off-tank switching back to cat form, were able to burn the Lich King down rather quickly. If you have a ranged DPS, I'd put them on the spheres just to be safe.
At the end of phase two, a valkyr will appear and grab someone. The valkyr will head towards the edge of the platform and, if they get there, throw that person to their death. Some people have said this will always be the healer.
That is not the case.
The valkyr took our paladin DPS. As previous commentors have two-manned this and noted that the valkyr always took the one of them that was healing, I am assuming, but not certain, that the valkyr will take someone who is not the tank. I could be wrong though. Could be random. Regardless, stunlock the valkyr and bring it down quickly. Everyone should be focused on the valkyr. Even the main tank, if that's what it takes. We managed to save our DPS paladin.
Tank and spank the Lich King through phase three. We took him down before spirits spawned, so don't worry as long as your damage output is decent. Just before the Lich King dies, perhaps about 4-5% health, the black pool that spawns under him will spread over the entire platform. You will all die.
Do not release your spirit.
There will be a moderately long scene between Tirion and Arthas. Menethil will resurrect you at the end. Go back to hitting the Lich King. He won't be able to do anything, so just take him down. Your achievement should trigger. Goodluck!

This achievement requires a character who can cleanse diseases.
Duod with an 85 Disc Priest and 90 Enhance Shaman.
Disc Priest tanked the Lich King whilst the Shaman grabbed all the adds. With the first 2 Necrotic plagues, the shaman sprinted+body and soul'd away from the adds and the plague was cleansed in an isolated area where it couldn't bounce to any adds.
On the third necrotic plague and onwards, it was allowed to run its course (3 ticks of 50k, easily survivable at 90), and it was only cleansed should the big stack of plague jump to the Shaman. This must be cleansed within 5 seconds, as it would one shot the Shaman with each tick. Allowing the default necrotic plague cast by the Lich King to run its course also keeps the dispel disease ability off cooldown for when it was needed.
Using this technique the plague easily went up to 41 stacks, on which occasion we killed all the adds and killed the Lich King as normal.

After the 2nd Shambling Horror summon your treants, and he should cast Necrotic Plague immediately after.
Once Necrotic Plague is up, begin auto attacking to generate rage and begin adding

Finish off the remainder of P2 on the stairs so the orbs don't knock you off the platform.
From there it's just dps. The only thing to remember in P3 is to fight him off the stairs because you need them to be clear of

The question is, to WHAT does this plague need to stack onto? The description quite sucks and is completely hazy and ambiguous.
The answer: the Nectrotic Plague must stack past 30 onto any single member of YOUR PARTY. NOT to the LK and NOT to adds. We did it just having it on one tank and passing phase 1 and getting into phase 2 and 3 and living to the end.
To sum, you get the plague from one of the mobs that show after you engage LK (ghouls or big mobs). The goal is to move the plague around, either by dispelling the plague from a player onto an NPC (stack the tank and have a healer dispel). A dispel costs you one stack. Surviving the plague adds one stack onto whoever it latches onto but it will not be the first person.
If you get a plague, you can just have that player stand on the tank and heal through it... hopefully it transfers onto a mob not onto the tank himself because it may hurt. You need all mobs but not the LK on the tank so it can ideally latch on to as many NPC's as possible, not LK. You might not want it on the LK because it could push his dmg onto phase 2 though this, I am not sure about.
This plague bounces around and it increases in number rather haphazardly, and haphazardly increases the stack and goes around to different NPCs and players, so it's a real pain to babysit. Sometimes you see the number is 13 then it vanishes, so you need more mobs to let it accumulate, you get it to 27 and it vanishes, don't worry. Just be patient and do not push the LK down below 70% where ph. 2 begins or you must wipe.
Better to bounce from tank to NPC to NPC to NPC than onto squishies...so keep the tank separated from the party and have the players run to the tank to get dispels or to get heals.
If you allow the plague debuff to stack to 30 onto anyone in your party, and you survive to kill the LK, the achievement does NOT require you to have to end with the debuff on any NPC. And you don't have to end ph. 1 with the plague on LK either. You merely need to survive the stacking to 30+ on anyone. We were trying to end ph. 1 with the 30 stacks on something but it doesn't matter. You hit 30+ during ph. 1 at any one time on any one player, you win.
Naturally you can dispel the plague when you need to (it can bring a geared 90 down in 3 seconds if you aren't paying attn) but you are not guaranteed to get bounced those stacks back and you might have to restart them at 1. The stacks are really randomly bouncing and appear whenever, do not worry if they disappear and require a long wait to get more NPC's to start the ball rolling. Just be patient.
Make sure that a non-tank gets to where the tank and the bulk of the mobs being tanked are in the time allotted, which is I think 10 seconds. You will see the plague debuff tick off, and then transfer from the first player onto someone, and will increase.
In order to survive the stacks, that means you have to wait a long time to accumulate all the adds you need to grab the plague from, so heal up and do NO DPS. Take off your auras and your dps trinkets (like if you have a trinket that does damage if you are damaged, you need to take that off). Some random people may wipe so be prepared to battle rez or lay on some quick heals.
If you get into Phase 2 without a player hitting 30 or over, you need to wipe. My addon was showing the stacks on the players, that's Healbot with debuffs shown on its setting interface.
The stacks do not have to be presently on you when you go to phase 2. You just need to have had them on you for long enough to get up past 30. Doesn't have to be 30 exactly, it can be 38 or 32, what have you.
I hope this helps get you your mount and get through what I think is one of the most badly worded achievements in recent memory. Rate up if you like this.

We did this one at level 90 with 5 people. Druid, Paladin, DK, Hunter and a Warrior. We tried ALOT to figure this one out, even read the top comments on this page many times. We used the Druid to tank/heal, due to that the AoE aggro ability of the DK and the Warrior did too much damage to the adds, so they died too fast. We dispelled the plague 3-4 times (until 4 packs of adds had spawned, and the druid had aggro of them.) The next plague that was casted after that, the one go got infected ran over to the druid and the adds, and got dispelled by the paladin there, so the plague jumped to our tank, which again was dispelled, and then jumped over to the adds and started ticking upwards.
Every plague that was cast after that in the raid, everyone ran out to the side, and waited for the timer to run out, so the paladin could focus on dispelling the druid if the plague jumped over to him. We reached 33 stacks, and then we nuked the Bleech King down until there was nothing left.
NOTE: You will get the achievement AFTER the fight is finished.

Alright, I want to help out with an easy, updated and thorough method for completing this achievement. I just 2 manned this the other night in full pvp gear with my wife and after reading a lot of vague, incomplete guides I felt the need to do this in order to ease the confusion. I will also be explaining the simplest method for defeating the Lich King for those of you having trouble with that as well.
Before anything else I will state that it is no longer a requirement to have EXACTLY 30 stacks of Necrotic Plague in order to get the achievement, but you must have AT LEAST 30 stacks now.
First off, there are 3 requirements to make this as easy as possible on you:
1. The WoW addons RaidAchievement and RaidAchievement_Old Modules. Download them at
www.curse.com
2. Download Deadly Boss Mods and Deadly Boss Mods - Wrath of the Lich King Mods, also at curse.com.
3. A character who can dispel Necrotic Plague.
Your roles:
Tank
First off there is no need to move around the stage. Stay right in the middle, facing your teammate and DPS the Lich King to around 75% health while keeping aggro on the mobs. Once he's at 75% face away from him so as not to damage him further, while keeping aggro on the mobs. Try not to kill them, but don't worry too much if some of them die. There is nothing more to it until the RaidAchievement addon notifies you that the criteria for the achievement has been fulfilled. At that point, kill all the mobs BEFORE you resume DPSing the Lich King. If you don't kill them, he will begin phase 2 once he hits 70% health, your stacks of Necrotic Plague may not all stack together, and you will not get the achievement.
Dispeller (I realize this is not a real word, but in consideration of MMORPGS it should be)
All you really have to know for this is 5 things.
1. The initial Necrotic Plague favours the players who are NOT tanking, so you will get it first.
2. Dispel the first 3 Necrotic Plagues AWAY from the mobs. Walk to the edge of the stage.
3. Don't waste your dispel on single stacks of Necrotic Plague that are cast upon you AFTER the first 3 above dispels.
4. Walk INTO the mobs for every dispel cast on yourself after the first 3 dispels.
5. Don't forget to dispel your tank.
Deadly Boss Mods will notify you when Necrotic Plague has jumped to one of your teammates, and RaidAchievement will notify you when you have accumulated enough stacks of Necrotic Plague to kill the mobs and down the Lich King. Both addons will notify you with text and an audible warning. That's all there is to the Achievement. Now on to finishing off the Lich King.
Phase 2
This is easy. Very easy. The Lich King will raise Frostmourne to the skies and begin casting an AOE that encompasses the entire stage, HOWEVER, the edge of the stage which is a cliff to certain doom EXTENDS during this phase, giving you a safe place to stand during this phase. Again stay put in one spot and have the tank DPS the adds that spawn. They will look like copies of your characters. Have the other person dps the white orbs that come from the Lich King. When phase 2 is about to end the edge of the stage will crumble and return to its former size. There is a visual warning for this. That is all there is to phase 2.
Phase 3
When phase 3 is about to begin, return to the center of the stage, where the Lich King stands during phase 2. This is crucial, because in phase 3 the Lich King summons Val'kyrs who will pick up a teammate and carry them to the edge of the stage and drop them into the abyss. The person who doesn't get picked up must DPS the Val'kyr as quickly as possible, which is why it's best to tank the Lich King in the center, giving you the furthest possible traveling distance to the edge. That is all there is to phase 3.
The strategies for each phase remain the same. Rinse and repeat.
IMPORTANT: Once you burn down the Lich King to around just under 5% health, he will wipe your entire group, and you will see the "Release Spirit" message pop up. DO NOT RELEASE! This is SUPPOSED to happen. This is the beginning of the last phase, and you will know it has begun when the Lich King begins his speech. If everyone in your party releases you will have to do it all over again. However, as long as one person remains and does not release during this phase it will still count towards the achievements once the Lich King is dead.
Additionally, when this phase begins DO NOT RESURRECT AND ATTEMPT TO BURN HIM DOWN BEFORE HE IS DONE SPEAKING! You absolutely MUST allow the scene to complete so Tirion can resurrect you. In other words, you need to be dead, and REMAIN dead. This means warlocks, shamans etc. need to refrain from using their self rez. If not, there will be no achievements had. Blizzard calls this a "glitch", though I find it is more of a "Well, guess you shouldn'ta done that" kind of situation.
I hope this guide clears up some of the problems many people have been having in completing this achievement.

Don't kill the Lich King during the RP event or no loot will drop, the achievement will not be registered and you will have to redo the full raid again after the weekly lockout expires :-(
I am sorry for the incident with the Lich King. I understand your frustration, and I'll try to provide as many information as possible so you can be better prepared for what has to come.
In your previous ticket you said: "The Lich King was at 1 health point when we were resserected by him we ressed and hit him."
We have verified that if players kill the Lich King while he's still on his roleplay event, this will prevent the encounter from properly ending. That is only possible cause you're now dealing enough damage tu burst him down before he completes his speech. When this glitch takes place, no loot will be generated, and no achievement will be provided.
As for the achievement "I waited a long time for this", sadly there is no tech which allows us to see how many stacks were reached by the necrotic plague once the fight ends.
Since we cannot verify you ever reached the goal we cannot provide you with the achievement. If you don't obtain that achievement by fighting the Lich King, the Game Master team cannot help you.
Please next time you defeat the Lich King, on top of meeting the requirements for "I waited a long time for this" make sure to also let the boss reach the middle of the room and complete his speech, before downing him. This will let the encounter to reach its proper ending and you'll be able to finally acquire the achievement you've been waiting a long time for.
I hope my explanation was detailed enough to let you continue your adventures, but if you need any other question answered just write me back.

A quick, TL;DR version of how to do this in MoP.
We did this with 4 people, one person was tanking the Lich King, the other was tanking the adds. We brought Lich King down to 75% health, so when we get 30 stacks we can quickly begin the phase 2 with which starts at 70% health.
You get infected with plague every now and then, for the first 4/5 times, we would run away from everyone (25 yards) and be dispelled (cured) so the plague doesn't appear on anyone, and just gets removed. After we have enough adds so they survive 30 stacks, we started dispelling it on the adds. When they one of them reaches 30 stacks, nuke Lich and you're done.
You need a healer or someone who can dispel disease (like paladin).
Plague doesn't go anywhere if the target is dead or dispelled near a different target, it jumps two someone else.
The plague hurts you, and more there are stacks, quicker it will kill the target, so the adds will die if you don't have enough of them.
And, the stacks on Lich King have nothing to do with this achievement, just to clear that out.
Super easy to be done in MoP, as you can just stand there, while Lich hits the tank, and while the adds hit you, yet receiving little damage.

THIS CAN BE INCREDIBLY SIMPLE. IT TOOK ME AND ONE OTHER PERSON ABOUT 2 MINUTES TO GET 30 STACKS. HERE IS HOW:
Okay, enough with the caps. It was me (90 priest) and my bf (90 pally.) We engaged the lich king and did no further dps whatsoever to boss or adds. Which is good. We let all add's come out. This is so necrotic plague bounces around between THEM not YOUR TEAM. My pally bf cleansed it from himself or I if it did get on us. The necrotic plague simply bounced between the add's until 30 stacks. You MUST have
raid achievement filter
addon to notify you when there are 30 stacks on an add and you can kill the lich king. Otherwise you will have to keep clicking different add's to see how many stacks it's at when it bounces to next add. We kept the amount of add's at about 9-11 so they didn't become overwhelming or anything.
So this is the break-down:
Have a pally or someone who can cleanse disease off a friendly target with you.
Start fight and don't dps anything. If you get over 10-12 adds, do kill the extra if you need to.
Have cleanser cleanse necrotic plague off raid members ONLY who get the plague.
Wait for it to bounce between add's until 30 stacks and Raid achievement finder notifies you that it has.
Finish off those adds and kill lich king.
Remember not to res when he insta-kills everyone. You will be resurrected shortly after.
Things to note:
It doesn't need to be on you to get achiev. In fact, don't let it be on you at all. just adds.
It doesn't need to be on any one thing in particular when it hits 30 stacks to finish off boss. It can be on any add once it hits 30.
The plague bounced between the add's automatically, on it's own. We didn't do anything. We just stood there and cleansed ourselves and healed. It just bounced around on them until it was at 30 stacks on it's own accord. The more adds that were out the faster it was, but still 10-12 adds was enough to get 30 stacks in just a minute or two.
Just be patient and wait for it to bounce off the adds while cleansing the raid members and there should be no probs.
My pally bf and I got this on our second try when we realized that we can just let it bounce off adds and not us.

My friend and I completed this achievement tonight
two-man on 10N WITH NO dispels.
After reading a lot of the very detailed and well-thought out strategies here, but not having access to a healer, we decided to figure out our own strategy and a way to do it without ever needing to dispel the debuff.
We did this with my Blood DK and his Hunter, both between 530-540 ilevel, but it could absolutely be done with much lower ilevel. Strategy/setup were very very simple, although there is a bit of RNG involved.
I (DK) tanked The Lich King near the stairs, Hunter buddy stood safely on the other side of the platform and did nothing. To be safe, he also had his pet hold position away from us because we weren't sure if the plague could bounce between him/his pet. We wanted to keep the fight in phase 1, so I did not damage the LK, just used Death Strike on a high-health add as needed to keep my health topped off.
The LK should never apply the debuff to the tank
- he always cast it on my Hunter. With no targets near the Hunter to spread to, the debuff simply fell off and he healed back up with Spirit Bond. We let this process repeat until the LK had summoned ~35 total adds. Our plan was simply to let enough adds build up that the plague could easily bounce up to 30 stacks among the ghouls while having enough eligible targets that it was
unlikely to bounce to the tank more than once.
Not iron clad, but it worked for us first try.
Once ~35 adds built up, the Hunter walked close to the pack to spread the disease to one of the ghouls, then backed out to safety again.
The plague spread between them, and by the time it hit 30 stacks, we were probably up to ~45-50 adds. With decent gear (I swapped in stam trinkets also to be ~1.02 million health with the 30% ICC buff) a tank should, depending on available cooldowns/self-healing/gear, probably be able to survive an application of the debuff with as many as 10-20 stacks. Any higher than that and the damage is just insane.
As soon as we saw the plague hit 30 stacks on an add, the Hunter joined in, we threw down max AoE to start clearing out the adds and burned The Lich King below 70% to enter phase 2.
The plague never even spread to me, though I'm not sure if that was due to a luckily timed Anti-Magic Shield while burning the adds. Either way, if I'd died, Hunter could just finish the fight solo, or any class with a battle res could bring the tank up.
Your mileage may very, but after this, I firmly believe many class combos could 2-man this without any dispels. You may have bad luck and either have the disease bounce to the tank twice (with no available cooldowns the second time) or bounce to him in the 20+ stack range, but with 50+ available targets and only one stack of the disease bouncing around, the odds are pretty good you'll succeed, and the strat is nice and simple.

I found the posts here sort of confusing, so I tried to break it down into a simple, non class-specific guide. This post is focused on the achievement, not on the entire fight. I hope this helps some of you out!
You will need:
- someone who can dispel or heal diseases (ideally)
- someone who can gather the adds together without killing them
What to do:
Pull the Lich King. Keep his health above 70%.
As the Lich King summons mobs, gather them together without killing them. Other players should stand away from this group.
Watch for
死疽
to be cast on someone. (A healer add-on may be helpful for this!)
the first two times, have that person run
away
from everyone, then dispel the disease (or let it time out). This allows time for more enemies to be summoned before starting the next step.
the third time, have that person run
into
the pack of enemy mobs, then dispel the disease (or let it time out). This will get the disease jumping around on the enemies.
Wait and let the Plague jump around in the pack until it builds up to 30 stacks.
Everyone stay out of the pack except the person tanking and gathering them.
Do not dps anything.
If

jumps to the tank, dispel it quickly.
If a second

is cast on someone outside the pack, have them run away from others and let it time out. (Save your dispel in case the tank needs it.)

ROOKIE WARNINGS:
- watch out for

in phase 2 - they are easily killed, but they will knock you off the platform if they touch you.
- the edges of the platform fall off when phase 3 starts... I learned this one the hard way! Stay away from the edge.
- at the end the Lich King kills everyone; this is normal! Also, if you somehow survive this, do not attack him. He needs to finish talking for the event to properly complete.
Useful info:
- the addon "
RaidAchievement
" (with RaidAchievement_Old Modules) is useful for tracking the stacks of Necrotic Plague
- the disease will kill mobs quickly as the stack count gets higher; step 3a ensures more mobs are spawned so that you don't run out of jump targets towards the end. If you wait longer, more mobs will spawn and the chance that the disease will jump to the tank is reduced (handy if you don't have a healer).

Simple guide:
1) Have all healers and dps stand in one spot
2) Have tank hold LK and adds about 15-20 yards away
3) Tank spam aoe taunts and threat holding abilities to keep everything on them
4) DPS and Healers run to tank and adds when they have the debuff, get dispelled, run back
5) For speed, dispel the first round of the debuff away from the adds, so on the second round, it jumps up quickly (as otherwise it kills the adds too fast)
Since Mists of Pandaria this achievement is now extremely trivial, as gear and stats are way above worrying about damage.
Your tank may get one shotted if not dispelled though, so
dispel your tank
, and ensure no one is killing the adds until you see 30 or more stacks. The achievement will be completed once you pass 30, you don't need to hit exactly 30, however you won't see it trigger till after the silly RP bit.
Also note, if you have any adds up as he enters the RP phase, it will get stuck, and only someone with Resurrection or a Soulstone mechanic can get back up and fix it (by killing the adds which cause the RP bit to get very stuck), otherwise you have to retry the fight.
Tips:
- You are not looking at the stacking buff on the Lich King.
That buff has nothing to do with this achievement!
I almost guarantee there will be at least one poopmonkey in your group who insist's that that buff is the one. That person is wrong, and possibly has parents that were related before marriage
- The buff you want is bouncing around the adds, and you want to see that hit 30, again, the one on the Lich King himself is completely unrelated to this achievement
- Don't do any damage at all until you have 30 stacks confirmed, as he enters phase 2 quickly these days (again because of stats and gear)
- Disable pets, spike/thorns damage mechanics, turn off anything that does damage when you are hit, and any pulsing AoE's. This will one shot his adds and fail the achievement

What we did after squish on Normal mode.
We had 6 people but should work with 3.
Need 1 hunter, 1 tank(or someone that can taunt), 1 healer.
Group stand in small circle where you first port in. Have hunter's pet tank LK and adds close to stairs. Use distance to keep from pulling aggro. Pet can only use taunt and Thunderstomp(the aoe that generates aggro). Pet will attack LK, and do a small but steady amount of damage, so when the Shambling Horrors spawn make sure the pet is on one of them to minimize damage done to LK. If Shambling Horrors die it's no problem. No need for dps to kill them. You are waiting till about 20(+) adds spawn. Drop plague off in the corner, either dispel or let it fall off. Do not move forward toward the adds. Once enough adds have spawned, start dropping plague on them. If 1-4 adds are pulled let tank taunt as many as possible back to the pet and have the pet use aoe to aggro off tank. Small number of adds in your group can be killed without worry since u will have a large number anyway. If 5 or more adds get pulled back to group then the healer needs to start healing themselves to gain aggro. Healer will then need to stack on pet and just remain on pet to keep the adds stacked up. If healer dies no worries. After 30 stacks burn LK.

Did this today, patch 6.0.2 as a holy paladin with a hunter. It was very easy once we figured out the mechanics.
My hunter friend attacked the Lich King and we waited while the LK spawned his adds a few times. As adds die so quickly we didn't worry about aggro and the hunter did no damage to the adds. He only got the LK down to about 78% health. I just sat there in the middle doing nothing. I did not even dispel necrotic plague when I got it.
When a number of adds were up we started the process. We basically judged it good to go when the hunter was at 20%. I then healed him to full. Doing so gave me aggro on all the adds which is what I wanted. I then waited until I got the necrotic plague and dispelled it. Now here is where we ran into trouble a few times. Only dispel the necrotic plague that has 2 or more stacks. When the process is running and necrotic plague is killing the adds, you will still get the newly cast necrotic plague from the Lich King. Don't worry about this as it did absolutely no damage to me. I only dispelled it when I had it AND the stacked necrotic plague needed for the achievement (you can have more than one instance of necrotic plague). Otherwise I let it expire without any issue.
This helped me save my cleanse for when it was really needed. By the time necrotic plague got back to me it was at 43 stacks. I bubbled which got rid of necrotic plague and we burned the LK down in seconds for the achievement.
Hope this helps with anyone attempting this under the new patch.

Just soloed this post squish as a Frost Mage. My guide and tips are below, first the TL;DR.
TL;DR
Set Elemental to passive, kite LK till 30 creeps are spawned. (Count them manually.) Do not drop below 90% HP.
Set Elemental to assist and let him take aggro of LK. My method of doing this was Blink + Blazing Speed, let LK catch up on his own, then Cone of Cold for its low damage and short range.
LK now casts Necrotic Plague on you, dismiss Elemental. Pass Plague to adds, continue kiting.
Look at LK's buffs, once it reaches 30 stacks, clear adds then nuke him to 10%.
DO NOT KILL

Wait for LK to finish showing his literary skill and finish him.
Now, a more indepth version.
Skills
With my method, you absolutely need Ice Barrier and Blazing Speed in your talents. It just makes surviving Infest and kiting so much easier.
Method and Mechanics
The majority of this fight is going to focus on only Phase 1, which also poses most of (all of) the difficulty in this achievement.
Ring around the rosie
This is the period of time where you need to stack up 30 adds that LK spawns. When I did it, I counted them as he spawned into chat, just so I don't forget. Firstly, set your Elemental onto passive mode. You don't want him drawing agro of creeps and messing up the stacking of creeps. next, when kiting, I went around the four bone pillars, thus forming a rough square. Ice Barrier I used on cooldown, Blink/Blazing Speed I used when he caught up. There was no need to slow the creeps at all - Ice Barrier's CD should come up before it breaks. Above all, make sure you do not drop below 90% HP in this time, or else his infest WILL kill you.
Do not damage Lich King as you kite.
You'll basically keep running around that square, counting the creeps until 30 are spawned, which brings us to the hardest part - getting the plague to cast on you.
Getting sick is very hard
Lich King has an annoying mechanic (for soloers) in which he will not cast Plague on the target he has agro on. Thus, you need him to switch agro onto your Elemental - this being the reason you don't hit him when you kite. My method of doing this was to Blink + Blazing Speed away as fast as I could, and set my Elemental to Assist mode in this time. LK will catch up before his adds, and I used Cone of Cold on him - its stupidly short range and damage was perfect to get the Elemental to start hitting LK and grab agro quickly. Almost immediately, he should cast Plague on you - at least, it did for me. Now, you want to dismiss your Elemental so that LK will continue chasing you and the creeps won't have funky agro, and
avoid a second cast of Plague happening
.
Running seriously gets bored after the 7th try
Now, you want to pass your Plague onto an add. To do this, simply stand near an add when the timer on the debuff runs out, and it'll automatically hop over. After this you have to... that's right, keep running round that square again. To track the stacks, instead of spamming tab targetting like you would have to if a second cast of Plague happened, simply look at LK's damage buff. Keep kiting till that hits 30 stacks, after which you need to clear the adds. Everyone probably has their own method, I used Frozen Orb on cooldown, spammed Ice Lance while moving, and when Blink/Blazing Speed was up, I used that to clear some distance and drop some Blizzards. While clearing, hopefully you'll drop LK down to 70% as well, which brings you into Phase 2. If that happens, just clear up the creeps asap.
Finishing off an annoying achievement
In this transition phase, you're going to want to essentially end the encounter. If not, in my experience, the Val'kyrs will carry you to your death, with no hope of survival. Luckily, this is pretty easy at 90. (and obviously 100 when that is released) You can definitely tank Remorseless Winter - Just walk into it and nuke LK down to 10%.
MAKE SURE YOU STOP THERE.
After that clear the adds. He should go into a long monologue during which I took the opportunity to Alt+Tab into Hearthstone, and when he finally finishes spouting nonsense, kill him off. If you kill him before this monologue occurs,
you do not get achievement credit or loot.
After this, all that's left is to wait for next week to steamroll everything on Heroic mode, then fly around on that shiny mount.

This is mentioned in some of the comments, but I want to comment it on its own, because lots of people who have never seen this fight at all are now soloing this or running it in small groups.
Plague Siphon (the buff on the LK) is not the same as Necrotic Plague. The boss gains a stack of PS every time NP jumps. However, it's possible and very likely that NP will lose stacks as it jumps, or all adds with it die. So LK can show 30 stacks of PS, but you've never gone over 10 stacks of NP.
I believe that in order to see the Necrotic Plague stacks on the adds, you might need to check 'show all buffs/debuffs' in your interface settings.
This achievement is pretty easy if you have a healer buddy and somebody who can tank without damaging. Everybody stacks along with the adds, and just let the thing jump around. My recommendation is to complete this achievement on normal and go back to heroic farming after you've gotten it - much easier to not have to watch for traps.

Here's how I solo'd this on my brewmaster monk.
First, I set my black oxe statue a bit in front of the stairs.
Second, started the event. Waited for LK to attack (he should go right for the statue). Waited for his first batch of adds, then used Jade Lightning to aggro LK only (leaving adds on statue).
From that point on it's just kiting LK around and running by the statue when he summons new adds so the statue will aggro the new adds.
Once the adds get up to about 24 or so, then use Xuen to attack LK. He should immediately throw the necrotic plague on you. Probably best to dismiss Xuen here. Run over by all the adds who are attacking your statue and dispel NP so it jumps to one of the adds. Then just continue kiting LK around until NP stacks to 30+ on the adds. As others have noted it's Necrotic Plague on the adds you need to check for stack count, not the buff on LK.
From this point on it's just normal mechanics.
Just a few things that may cause problems... You don't want to do too much damage to LK since you don't want to push next phase, so it's possible the statue might pull him off of you. If that happens you can pull him back on to yourself or you might want to wait and see if he gives you NP at that point. If there's enough adds up you may just give it to them then instead of using Xuen. Xuen can push him into next phase, so in any case you may want to dismiss him quick if you use him on LK. You're mainly just using Xuen for the taunt.

This achievement can be done solo by a priest, i just did it on my shadowpriest with a tactic that only took 2-3 tries to perfect. Gear is probably irrelevant here, my priest is using ilvl 446 healing gear from normal instances, not being able to dps or heal enough is unlikely to be a problem.
The only thing that is a must for this achievement is speccing into Angelic Feather, since you will have to kite your way through most of phase 1. I would also recommend taking the mind bender talent. Make sure you are at over 90% at all times so you dont have to stop to heal if you get hit by Infest.
1) Start off by dpsing the Lich King down to somewhere around 75-80%, close enough to 70% that you can quickly push him into phase 2 but not so close that your shadow fiend might accidentally push him. Once you've reached that health start using Angelic Feather to kite him and the adds around the platform, a simple kiting technique is to just continuously go around the edges of the four pillars.
2) Count the adds that he spawns, everytime he summons ghouls thats three adds, every shambling horror cast is one add, just have Lich King on focus or targeted at all times and do simple addition whenever you see him summoning more adds.
3) At around 20+ adds is when you want to put the plague into action, summon your Mindbender/Shadow Fiend onto The Lich King and use Fade, it is important that your fiend grabs boss aggro since the plague never gets casted on the target Lich King has aggro on. Once you have the plague on you, let the adds get in range, drop it onto them and then resume kiting around the four pillars. During this kiting you want to bring Lich King as close to 70% as you can and then as soon as you reach your desired number of stacks push him into phase 2.
4) Once you have your stacks 30 or higher, push him into phase 2. This is the roughest part, you have to keep your distance to the adds to make sure the plague does not jump to you. You also have to keep an eye out for the ice orbs that spawn and kill them at range before they get to you. Use whatever spells you can to kill off the remaining adds at range as well as the ice orbs, once they are all dead you can breath out and simply finish the rest of the fight as usual.
As others have said, once you've brought him to 10%, STOP THE DPS and let the RP happen, otherwise you risk not getting the achievement.

Basically you let your pet tank everything until 4 sets of ghouls have been spawned and then you spread the plague among them. Then you just tab between the adds until you see that Necrotic Plague has stacked 30 times. I did have some issues with my pet's hp dipping below 90% but casting Mend Pet and then Feign Death worked pretty well. Also be sure once you done that part you don't burn the Lich King down to 1hp until after the RP event is done or you might not get this achievement.

Reposting this I wrote for the 25 man achievement since it should work the same.
As a Hunter (I'm 566 at this time), this is extremely easy to get.
Put your pet on
stay
and
passive
, pretty much by LK, and
Misdirect
some aggro onto it. Then
Feign Death
a little bit away from them. Remain "dead" until maybe 3
Shambling Horrors
are up, or 4 if you want to be safe. Once you have enough adds up, run back up and stack on them until you get

again, stay down and just tab around until you see 30 stacks on one add. I personally let it go for a bit more just to ensure the achievement ticks it off. Once you have your 30+, nuke LK and there's your achievement.

just soloed this today on my guardian druid, brought him him down to 75% and then just waited for adds to spawn. once there were around 30 adds, summoned treant and got the necrotic plague. once it weared of and reached the adds, just kept running around in cat form kiting them using dash and stampeding roar whenever up along with feline swiftness talent.
kept kiting them till eventually necrotic plague weared off and didnt pass onto anyone, managed to get 61 stacks of plague siphon on lich king. once that weared off, just killed any remaining adds and resumed dps on boss.

Did this today with no problems. Apparently after the item-squish, this is no real issue. Just have all party members stack on top of the adds and Lich King and let the disease pass to each target. Might be best to have a healer just in case, but we didn't really need one. Filter through enemies and players to see how many stacks the plague is up to and burn after it hits 30.
Side-note: Apparently we don't die from Lich King toward the end of the fight anymore.

I spent a few hours farming out the terracotta fragment, but found out that our buddy Arthas here just one shots them at the start of the fight.

Did this today solo as a brewmaster monk ilvl 542, LK 10M Normal.
1. Summon black ox statue
2. Stand clear and do nothing but watch as LK and his adds focus on the ox.
3. When there's maybe not less than 20 adds on the ox, run in and detox necrotic plague then run out.
4. Resummon black ox when its health runs low.
4. Stand clear again and watch and wait til necrotic plague stacks to 30 .
5. Pop defensive CD's and keg smash and spinning crane kick them all down.
6. Burst LK to death
Very very easy. Will try it after reset on 25M Normal and see if the same works.

Managed to solo this on my ilvl 528 balance druid.
As soon as the fight started I simply let Arthas and the adds hit me until there were a sizable amount of adds. Then I used
on him causing him to temporarily switch targets allowing him to cast it on me. Then it was simply waiting as it bounced from target to target getting stacks, respawning trees to add more plagues that would bounce around and eventually merge. Continue for 30 stacks or until golden brown. Healing through the damage if it bounces to you might be hard but somehow after getting to 30+ stacks i aoed the adds down and somehow it didnt bounce to me. Still got the acheive tho

Tried this as a ilvl ret pala, but if i just stay still after engaging LK, he only summons three ghouls who keep hitting me for no damage at all. But i didnt get any Necrotic Plague at all through the whole fight! I tried both dps´ing LK and just kiting him around, but still, no plague!
So, what am i doing wrong? Please Obi-Wan Kenobi, you´re my only hope!

I solo'ed this achieve tonight on my elemental shaman. I just popped my earth elemental totem so Arthas would start the plague (he won't cast it it seems if you are alone) and then let it jig-jaw back and forth between me and the adds without damaging the LK at all until it got to 30 stacks on me. It actually got to 35 before I pushed him into phase two and it was definitely ticking HARD and I had to use almost all my mana on healing surge to stay alive along with my stone bulwark totem and healing stream totem. It also helped to put a glyphed flame shock on the occasional add for that heal as well. After I was infected with the 35 stack of it, I simply went into nuke phase and pushed him right to the end easily.
I would not try to solo this unless you are able to summon some kind of pet as I don't believe he will start the plague if you are alone. Also- make darn sure you have some healing abilities, etc. because- as I stated above- even in 6.0 it ticks hard.

I can confirm it is still possible to do this achievement solo 10 man HC as Priest.
The tactic is actually rather easy. At the beginning don't do any damage to LK, wait till he spawns enough adds (I waited between 3 to 4 minutes), then summon Shadowfiend and use Fade, this way LK will cast Necrotic Plague, now you just kite them around till the debuff reaches 30, now you can kill all remaining adds and finish LK.

Also Elemental Shaman - 90 level (491itlv).
Some hints form me- use Glyph of Ghostly Speed and Glyph of Flame Shock.
Before start change into Ghost Wolf. Glyphed wolf will cause you will be faster than Lich King unless you stop use that form and get hit.
Move circular, avoid being hit by Lich King and wait until he spawn 24-28 adds. After that summon Earth Elemental , make him taunt Lich King. When you get Necrotic Plague debuff - use Totemic Recall to get away your elemental. Get close to adds to jump plague from you to adds. Then kite adds nearby using Earthbind Totem. Don't just run far away - I think plague have some range when it jump to another add. From time to time shot Lich King or some add Flame Shock to heal (don't get below 90% health - you can die by Infest). Kite adds and watch Lich King Plague Siphon buff - it should gains stack after Necrotic Plague jump on adds and they die, so after that buff stack 30+ start killing adds in range. Earthquake is very good with chain Lightning and of course Flame Shock to not get below 90% health (use Spiritwalker's Grace or/ and Ascendance to make it faster). Then just kill Lich King but not nuke him to death when he Roleplay in last phase - wait and after that kill him to get your achievement.

I just noticed it is no longer available but any trinket that spawns a pet that can take aggro should work since you only need it for a small amount of time.
What I did is that I waited about 4-5 minutes during phase 1 so that the Lich Kings spawned in a good number of adds for the plague to get passed around on During this time I made sure not to attack him at all. Once I saw that I had enough adds I popped my trinket

Okay, After spending nearly 1.5 hours trying to complete this solo as a Combat Rogue, i felt it only fair to post my tactics.
First off, you NEED a aggro companion. Otherwise it cannot be done without a friend. I personally used the "Horseman's Sinester Slicer". See 'Guardian pet' page on wowpedia for others.

- Same as any other class, let the LK and adds just hit you for a couple of minutes (i waited around 4) or kite them around. They won't do much dmg. Do NO dmg whatsoever to the LK. (turn off Blade Furry, and leech poison or any other AoE)
Once you have a bunch of them, comes the tricky part. The difficuly part is that your companion pet needs to pick out the LK as his target. Most of the times it will choose the mob closest to you, which isn't always LK.
To solve this, run up the stairs and wait for the LK and adds to catch up. There should be NO adds on the platform anymore. Now wait until LK as summoned a new batch of adds. This is important! I'll explain later. After the summon, use Sprint and run to the other side of the platform. LK and adds will follow, but the LK is much faster! However, if you did not let him summon, he might stop halfway and the adds will overtake him. So, again, first summon, then run.
Once you've reached the other side, LK will be close to you, the adds far away. Now summon your guardian and they will attack LK. Provided you did NO dmg, LK will now attack them and whoop! you have the Plague.
Now just wait for the adds to surround you and let the Plague run its course. After you have transferred the Plague, target the LK and kite the mobs around. Watch for the counter in LK debuff bar. Once it reaches 30+, go and kill all the adds with all the AoE back to full and finish the fight with LK as you normally would.
I hope this helps! Keep trying, took me 4 to 5 wipes to get it done but it IS possible, even as a rogue

Start the fight. Do not do any damage. Healing is okay if you're worried about Infest. This step will take a while, so you have time to make a sandwich.
Wait until 20+ adds are spawned. Count them as they spawn. All the unit frames can get a little overwhelming if you don't.
Cast a few low-damage abilites until the trinket procs. Don't auto-attack. Stay in the same phase.
As soon as you notice the guardian spawn, cast your threat-dropping ability. You'll get Necrotic Plague.
Wait again. Eat the sandwich. Necrotic Plague will bounce around. You'll see the Lich King gets a buff stack every time.
Once he has 30+ stacks, this is an indication that Necrotic Plague has jumped targets the appropriate number of times for the achievement. If you're neurotic like me, wait for 35-40

Kill kill kill. Achievement will pop, and the cutscene will play.

You can solo this achievement, I just did it today with my level 99 hunter! Here is how you do it.
1. Have you pet gain agro by attacking the Lich King then put him on passive. You don't need to kill anything yet.
2. Stay away from your pet letting him take all the damage (I used a Direhorn but any good pet tank should work) and allow the Lich King to spawn undead add ons. Make sure you are counting every new guy spawned, because it gets really confusing when you have ten plus guys attacking your pet. You want a total of 30 guys attacking your pet for this to work.
3. Once you have 30 undead adds attacking your pet wait till you get the necrotic plague, you should have been getting it as you run around avoiding the shadow traps that launch you off the platform. Right before the timer is up for the plague run up to the mob of guys attacking your pet, don't engage them just run past and the plague should jump from you to them. You'll see a green line jump from your character to the mob.
4. Run away from the mob and just wait, the plague should start jumping from guy to guy stacking and killing them as it goes. When the stack reaches 30 unload your hunter arsenal on the group and kill them all. Then kill the Lich King and you'll get the achievement.
My pet never got the necrotic plague when I did it, let me know if any of you hunters experience anything different.

Lich King casted Necrotic Plague almost immediately for me (Haven't done this on 25 man yet, so not sure if this was a fluke for speed).
From there control adds (no need to kill them on 10man) and just make sure you cleanse/dispel whenever you get NP.
Wait until 30, then nuke Lich King.
I did this as a level 100 Ret Pally, ilvl 640. Was never below 95% health the entire time.

Ok so this is how i did it with my Feral Druid lvl100:
You need to select
FORCE OF NATURE
talent!
1) Engage Lich King by speaking with Highlord but
DO NOT ATTACK HIM
2) Wait at least 3-4mins so he cant spawn enough mobs.
3) When you are sure the are enough mobs just left click on Lich King to target him and use
Force of Nature
4) Now you get Necrotic plague - kill one random mob so NecP can start to jump.
5) Lich King gains his
OWN DEBUFF
each time Necrotic Plague jumps - don't confuse it with NP stacks
6) If you think that you lost track about Necrotic Plague stacks just cast Force of Nature again and repeat steps 3-4
7) When u have at least 30 stacks you can nuke Lich King down.
Notes: Lich King own debuff (Plague Siphon) stacks each time Necrotic Plague jumps so when he has like 35-45 stacks it
SHOULD
be safe to kill him. But I am not sure about this!

Got this achievement today as a solo Blood DK. 10-man normal. Even though I had 31 stacks of NP.
Just tab-switched between mobs constantly to keep track of Necrotic Plague.
On the 30th stack I aoe'd all mobs (should've done that on 29th stack), and then I myself got infected with the 31st stack of NP, that's when I quickly downed Lich King to the 2nd phase and killed him.
Note that I didn't (and couldn't) dispel anything. I summoned my army of the dead in the beginning, they managed to kill some mobs before vanishing - I guess that's what triggered the first NP.
I will also try this next week on 25-man normal solo, but then I think the damage from Lich King's Plague Siphon on me will be too high.

For a brewmaster monk, this is quite easy to do. I would suggest getting the "raidachievement" addon for this, as it informs you when you reach the required 30 stacks. I had failed this multiple times on different characters as I was looking at the plague siphon stacks on Lich King. While this is a good indicator, it is not what you are counting on.
Anyways, I started the encounter and dropped my ox statue at the bottom of the stairs, and then went and stood some distance away behind one of the pillars. Far enough away so that when I detoxed the necrotic plague it wouldn't jump to anything yet, but close enough I could roll up quickly when needed.
I simply just stood there until there were 4 shambling horrors and countless ghouls up, and then rolled up and detoxed the plague off of me onto them, and then rolled back to my original position while the stacks ticked away on them.
I would have to periodically refresh my statue, and if they agro'd me I just cast Leer of the Ox and attract them back to the statue..
After awhile (I didn't time it), the addon finally announced that there were 30 stacks and to dps LK down (note; there is no visible counter, it just pops up on the screen when ready).
Once the LK goes into his RP phase, just wait it out. I thought I had bugged the encounter as he wouldn't stop casting on me, and Tirion had not yet escaped from his ice. I finally realized that there was still one shambling horror up that was quietly flailing away on my ox statue behind me. Once he was dead the encounter finished correctly and I finally have that cool dragon mount!

I did this today on my Brewmaster Monk in the 10-man version and I have some clarification that doesn't seem to be noted anywhere else. The reason why you cannot simply count the Lich King's stacks up to 30 is because he gains an additional stack every time he casts Necrotic Plague, period. If there is no Plague bouncing between the adds, the stack will simply fall off between casts, but as soon as you drop your DoT into the group of adds, it will keep rising
while also adding every stack he gains from brand new casts.
On the 10-man version, he targets you with a new Necrotic Plague 15 times before the DoT you dropped in the add group bounces to 30 stacks.
This means that, as long as you only drop
one
Necrotic Plague in the add group, you have to wait until
46 stacks
of the Lich King's buff before the achievement's requirements will be fulfilled and you can continue to Phase 2.
I haven't tested this in 25-man, but given that his buff doesn't fall off for 30 seconds, I can't imagine that number would change.

There is a terrible annoying bug to this if you try doing it with a pet and a DK friend. Maybe I just haven't played DK's enough but no matter what your pet is doing, all the Drudge Ghouls will go after the DK. And it doesn't matter if the DK is Blood, Unholy, or Frost. It just aggros them all. It's super annoying and if it's a natural DK trait then my mistake, but right now it seems like a bug.
I have to have my friend die in order for him to stop aggroing everyone to him.
Need confirmation though or else I'm just crazy.

100 priest and 100 pally did this takes about 5- 10 minutes.
We initially tried the running around the platform spawning stuff, this ended up failing as all the ghouls started dying off when we got a ton of them. so then we just stood next to each other where the ghouls spawned. I dispelled two times. then the stacks started hopping between us, did a little bit a healing to stay alive til it hit 30 stacks, I died(priest) pally beat on LK for a bit, then there was the rezz from the instance, wherein we both got the achievement.

Soloed this (finally) as lvl 100 ilevel 660 Ret Pally, after what seems like a million attempts. I used

-- as others have said, you must have some kind of pet or you will never get the Necrotic Plague. This takes a LOT of patience. This is what finally worked: I got up on the stairs that lead up to the throne with my back to it and did not move at all during the encounter (I got thrown off of the platform to my death a bunch of times before I figured that part out). I started the stopwatch when the Lich King first started beating on me and waited 6 minutes doing nothing but healing a little bit in order to let him generate lots of adds. You need a lot of mobs because you have to do some damage to proc the trink, and they are very easy to kill. When I started damage too soon, I ran out of adds and all of the boss's stacks dropped off. I set my aura to Seal of Insight and I only judged the Shambling Horrors because they had the most health. No autoattacks, no other attacks, and no attacking the boss. It might take a while for the wolf to come out but you will get the Necrotic Plague when it does, then cleanse. I used the addon Raid Achievement, it politely applauds you when you have finally got enough stacks for the achievement and flashes a big message on your screen that it is now OK to kill the boss. The entire encounter took almost 18 minutes.
I did try several trinkets with an on-use pet, but they all got one-shotted before I got the disease. I just kept switching out pet trinkets til one worked for me, and the wolf came through for me. Good Luck!
Edit: Just did this on 25 man and had to change strategy a bit to get this to work. When the wolf appeared, I started attacking the Lich King to get the wolf to attack him, then I used Hand of Protection on myself. Got the plague immediately.

This is how you solo it as a Death Knight.
Go Blood Spec since it will be easier to tank all the damage that you recieve while you wait for the Necrotic Plague to stack.
But start with Unholy Presence because you'll gain 15% movement speed that will be useful in order to handle the Necrotic Plague stacking and Unholy doesn't give the threat generation which you need to stay away from.
Also make sure that you DON'T have
since you need your army to be able to taunt successfully in order for this to work. Would be also useful to have Gorefiend's Grasp talent ability so you can keep the adds together in a bunch since it will be easier to handle what you're about to do.
1. Start the encounter. But DO NOT LAND A SINGLE ATTACK ON THE LICH KING. If you do, you will taunt him and then he won't throw Necrotic Plague. Necrotic Plague lands first on a target that currently doesn't tank the boss.
2. Just run around the platform without attacking him and make sure that your back is facing him so that you don't accidentally attack him. Wait for him to Summon his army 3 times. Don't attack his army either. They need to be alive for this. Make sure your HP is over 90% at all times so you don't die to his Infest. You may Death Strike the biggest adds for heal. Avoid his Shadowtrapss.
3. After he Summons his third army (which means that you, at this point, are being constantly chased by the lich king and his 3 armies simultaniously while you havent attacked any of em even once) you summon your Army of the Dead.
4. Let only your army attack the boss. This is kind of RGN because sometimes the army doesnt attack the boss but his army instead. If they don't attack the boss and taunt him, then you have to restart the whole encounter until he's taunted by them.
5. A few seconds later, you should recieve a Necrotic Plague debuff. This debuff will expire after 15 seconds and as soon as you have it, you need to run over to the boss and taunt him. If you don't taunt him he will throw another Necrotic Plague that stacks individually but the lich kings armies wont be enough to stack them both to 30. So make you that only one Necrotic Plague is running around by taunting the boss imediatelly after you recieve the first Necrotic Plague debuff. You will go Blood Presence from here on.
6. After the debuff expires it will jump over to the next closest target that isn't the lich king (Yes, it jumps to the Lich Kings adds too). Make sure that you are near the armies when it expires so it can jump over to them. It will also jump if the debuffed target dies during the effect. With each jump it will stack by +1.
7. After the one of the adds has the Necrotic Plague just run around the platform again while you're once again being chased by The Lich King and the Armies. Do not attack the armies unless you need to heal to stay above 90% HP and when you do, Death Strike only the big ones. If you can Death Strike them without killing them, Perfect. Keep them in a bunch with Gorefiend's Grasp and keep a close eye on the Necrotic Plague debuff. You can do this with a Addon called Tidyplates. It will show the debuff right above the nameplates hp bar. But you need to mouseover them in order for them to show the active debuffs. Make sure that you arent the next closest target to the add that has the Necrotic Plague debuff.
7.5 If you ever f*ck up anywhere here, just jump off the platform and die and restart for if you kill the Lich King, you will need to wait another week before you can try again.
8. Once the Necrotic Plague has reached 30 stacks or higher, slow the target that has the buff in any way you can and kill him from range with any means necessary. Using Gorefiend's Grasp, Aphyxiate or Chains of Ice should work fine. Kill all the adds in range of the debuffed add if you can from afar. Stay at least 10y from them. It is also fine if you only slow the add with the debuff and drag the other adds away from it and then kill it. It will have nothing to jump to when the add dies and it will just dissapear.
9. Once you've ridden the Necrotic Plague from the fight start dpsing the boss. At one point the boss will run over to the middle of the room and start spawning some stuff with frost wind. He will point his blade towards you while he does it. Ice spheres are one of the adds that spawn and they look like whitebluish wisps (balls). As soon as they do, KILL THEM ASAP. If you don't kill them in the short time that you have, they will detonate and you will fly of the platform and die. Rest of the fight is a piece of cake. Continue as usual and kill The Lich King.
10. Enjoy your Frost Wyrm mount.

Shadow Traps are not used on normal (non-heroic) mode
, and this achievement
does not require Heroic mode
, making this achievement much, much easier.
You can also just switch between modes before the fight without leaving the instance and coming back.

This 'plagued' us over and over again as we voluntarily wiped every time we went to the next phase because we didn't think we had gotten enough stacks.
The thing is:
Maybe we did get the 30 stacks and didn't know it.
Somehow it may be cumulative.
We decided to install the
addon RaidAchievement
along with the
RaidAchievement_OldModules
addon as well. You load Ice Crown in the options typing /rach into the chat box.
I did this heroic 10 with my son, a 99 rogue. (I'm a 690 mage) I ran in frost so I could use the water elemental to tag the Lich King. I followed the solo frost mage guide in the comments below, and let my son stand on the steps. He would get hit with necrotic plague but it just wore off. I ran in circles. I finally got my stack of necrotic plague. It equaled x 0. I ran through the mobs but subsequent Necrotic Plague stacks only registered at most 6 stacks 8 stacks... why wasn't it going any higher? I was down to just a few ghouls when my son died. How were we going to get 30 stacks? It seemed impossible.
I stayed in the ghouls trying to get stacks which never went above 8 as far as I can remember. I was watching the debuff like a hawk. All of a sudden, I heard applause: The RaidAchievement audio warning that we met the requirements for the achievement.
I was wondering how is this possible? I only remember seeing 8 stacks!
But suddenly, after the applause, my buff bar showed I had 37 stacks! I cleared the remaining mobs and hit the Lich King in his next phase, careful not to kill him. Thank God I was standing in front of the steps because a blast threw me back (total luck on positioning - Remember: stand with your back to the steps ). The Lich King got down in health and suddenly the RP started. I was wondering whether to trust the add on or not. I had never used it before this run and this must have been our 12th unsuccessful run. Did I really have 37 stacks? If I went through with it and trusted the addon it would be another week before we could try again. But I was sick of it. I decided to go through with it and live with the heat I would get from my boy if I killed Arthas before we got the stacks. My son got rezzed, I let him kill the Lich King and...
TAH DAH! We got the achievement!
Bottom line: I don't know how the stacks really work. I swear there were only 3 to 4 mobs present when my stacks jumped past 30. Are they cumulative? Shared? Weird Math?
Might as well get the addon to help you know when to say when
.

Just did it with my elemental shaman :)
Waited for him to spawn 20+ adds without doing any damage at all, then popped my earth elemental total. Got necrotic plague instantly , waited for it to stack to 35 (just to be sure xD) and nuked him. Easy!

Soloed this as an Affliction Warlock, after reading through the many helpful tips on here. My process :
-Do this on normal 10 or 25. Heroic makes a mess of things.
- switched to grimoire of sacrifice and sacced my voidwalker before starting the fight. Just for the heals, I had an unhealthy fear of Defile.
-let the fight start, and do nothing to the Lich King. Just run around, waiting for adds to gather. I waited for maybe 10-15 to spawn.
- get some distance from the pack, either running with burning rush, or perhaps a well placed gateway.
-summon voidwalker, then sic him on the Lich King. You should very quickly get necrotic plague.
-as soon as you do get the plague, sac your voidwalker again.
- just move around, monitoring the stacks (I recommend the addon raid achievements. Tells you when you get enough stacks.) keep the pack of adds close, and don't worry when the plague jumps to you, doesn't do much damage with the heals from GoS.
-once you have the stacks, burn the king down and enjoy your achievement!

I just solo'd this on 10-man normal and got the achievement & mount.
1) Let Lich King summon lots of mobs. Just let them beat you up. I stood with my back against the throne.
2) Once he has 20-30 of his mob on you, hit the lich king
3) You'll get necrotic plague...just wait and it will time out and transfer to a nearby mob.
4) Let the plague bounce around from mob to mob killing them as it gains stacks.
5) At 30 stacks, kill the Lich King (this will coincide roughly with his Siphon stacks, but I would wait till siphon is 31 or 32 to be safe)

Just a note...pretty much works the way people are saying...need something for Arthas to agro onto in order to get the plague rolling. I used Cardboard Assassin. Took several attempts to get enough stacks and I used RaidAchievement to help track it. Lvl 100 Prot Warrior ilvl 715. Really the two things that I had to watch for aside from letting enough adds spawn were my health, keep it above 90% and knockback from one of his abilities. Stairs helped with that.

I wanted to help a friend today, so i did what i had done the last many times when helping people with my hunter.
1. Start the encounter
2. Have the one getting help jump over the edge
3. Wait til i had a decent amount of mobs
4. FD
5. Well what the heck, now LK resets, encounter ends.
So you can no longer just do the FD thing as hunter, LK will then reset you now have to be up fighting, so after some try and had problem with the UI, found a way to do this as hunter then.

Do not try to 'solo' this achievement if:
- You do

The rest is really simple...
To clarify something,
死疽
and
热病虹吸
do have a relationship and can be used for tracking if managed correctly. This is why you
CAN
track the
热病虹吸
buff on the Lich King for this achievement purpose.
How?
The relationship is simple: every time
死疽
jumps from one person to another (this could be you/ghouls/big ghouls) the Lich King will get an extra stack of
热病虹吸
, his debuff.
The Lich King will regularly cast
死疽
on
YOU
but will
NEVER
cast it directly on his minions (ghouls/big ghouls). It will also only start if at least 2 characters are in the area. So if you want to reach 30+ stacks of
死疽
you will need it to jump at least once from you to the other minions. From there on, it will only jump to the next if that minion dies.
Why easy
Whatever you do during the fight... only have
死疽
jump
ONE
time from you to another minion. So i repeat, only have
死疽
jump
ONE
time from you to another minion!
That's all... if you can manage to do that, you can kill the Lich King when he has 31+ stacks of
热病虹吸
And again, i will repeat. If by any chance you have a minion coming to you because of 'threat' and you are not able to redirect it to your pet... KILL IT ASAP!
Once you have the necrotic plague on you and it ends when the minion is close enough near you... it will jump to that minion and you can no longer trace the Lich King's stacks.
To think about it... maybe if you get the Lich King until +32 stacks cause it jumped one extra time...? This is something for debate but after al lthe tries... i'm not bored to test that again :-) If somebody else wants... please go ahead.

Slow but steady tactic to do this solo with a warlock:
Send in Voidwalker to tank

. After VW gets a couple of hits in, command it to stop attacking, to prevent Lich King going into phase 2 (at 70% hp)
Stand at some distance, so that when

is cast on you, it will wear off without jumping to a ghoul.
Lich King will summon

in groups of three. It seems that randomly they will attack you or the VW. I killed the ones that attacked me. Note that
灵魂碎裂
does not seem to stop them aggroing you, because the VW is not building any new aggro neither.
Keep a mental count of how many ghouls are up; by the time you get to around 30 it will be impossible to count them!
At no time heal your voidwalker! You will aggro all adds onto you.
Sometimes the Lich King summons
蹒跚的血僵尸
, bigger adds. I killed those too, just to keep things simple.
When 30 or more ghouls are up, wait until the next necrotic plague is cast on you. When it is about to wear off, walk into the group of ghouls to make it jump to one of them.
Immediately stand back to prevent the plague from jumping back to you or to start a new chain of necrotic plague from yourself.
Continue killing newly summoned ghouls that aggro you.
When the plagues has jumped 30 times (use Raidachiever addon or check
热病虹吸
stacks on Lich King) kill all ghouls and then kill Lich King.
Profit!
If done correctly, this takes about fifteen minutes.

Seems to have gotten easier as of May 2016. Have completed twice with level 100's. Stack on stairs and just wait til stacks hit 30, then kill Lich King. Need a second person or pet (Treants, Voidwalker, or player) to get the chain going. Never had to move from bottom of stairs in 10 man and 25 man. He'll keep summoning.
Completed successfully when necrotic plague showed at 30 stacks, and another time with 30 stacks of Plague siphon showing on Lich King.

Noticed I didnt get hc10 achi done. Did hc 25 version on my 553ilvl disc priest before Wod. So I gave this a shot again on hc10.
My gear is from 625 - 710 ilvl - total 690 all pvp - spec disc.
1. Run to half mid of stairs after i've started the event and didn't dpsed LK. Used

For the people that wanna solo this achievement.
I made an easy video of how to do it.
What you must have:
A pet like a hunter/lock/mage/DK etc.
Once you have 7 adds, start let your pet attack the boss for lich king, just a littel dmg so he has aggro, be sure to stay before the add spawn with your back to the mountain, after adds spawned and pet has aggro, you'll get debuffs.
those debuffs will change every 3-7 seconds from you and the monsters.
Till it stacks 30debuffs (so TAB alot to see it) you kill lich king on easy mode.
same counts for 25 man :)

Ill keep it short for everyone. THIS WORKS FOR 25 and 10 MAN!
1. Do not solo, unless you have a pet that cant tank. If so read another comment.
2. Grab a healer/dispeller
3. Get to the LK
4. Start Fight
5. Facetank the LK. Dont attack. Have your healer stand on the other side of the room
6. Avoid the black circular spawns. They will begin to spawn on you 3 seconds before they conjure at your location.
7. The healer will automatically get the first "Necrotic Plague" debuff. Dispell it AWAY from the group.
8. Wait untill the healer dispells a total of 3 "Necrotic Plague" debuffs AWAY from the group.
9. All further "Necrotic Plague" debuffs should be Dispelled within the group of mobs.
10. Continue to dispell "Necrotic Plague" untill 1 mob has 30 stacks or more. (I did it at 45 stacks because it starts stacking really fast)
11. Kill Lich King

Remember to Avoid the black circles. THEY WILL knock you off the platform, and deal a fair amount of damage to the triggerer. 2 Players must be alive for necrotic plague to be applied. Do not heal your facetanker. Youll get full agro. And most of all, enjoy getting the achievement.

on you. Be shure you have 30+ stacks of Necrotic Plague on you.
6. Start killing everything

Did this with my brother, I was a Restoration Druid and he was a Vengeance Demonhunter. No healing was required. You may note I did this on NORMAL 10, this way we didn't get the annoying ''dark traps'' (the black circles which throws you off the platform)
After reading a lot of comments, tried some tactics, and failed several times (probably due to the fact that I didn't do it properly) I gave it one last try. I encountered the Lich King again and this time, we just stood here, doing nothing at all. After waiting for about 3 minutes we BOTH reached 30+ at some point. When I had around 40+, we decided to kill him. Got the achievement after the dialogue :)
I never expected this would work too. Goodluck with yours.

For those wondering, you DO NOT need to maintain 30+ stacks for the whole fight. You just have to have had 30 stacks or more at some point in the fight. I know it's obvious and is impossible anyway. Just wanted to clarify for those who may be paranoid.

As a shadowpriest this is quite easy now (I was lvl 102 atm with 709 ilvl).
Or another class who can summon a pet and even with a trinket that summons one.
Just start with taking hits from the Lich King. Let him summon a minimum of 30 mobs, keep shield up to help with dmg and if you get the plague.
After he summons 30+ mobs use 'Shadowfiend' and 'Fade' on the LK. Then he will cast the plague on Shadowfiend, and it will bounce randomly from mob to mob (sometimes it will hit you as well).
Keep LK targeted so that you can see the stacks of plague, and after it reaches 30 it's just pew pew on the LK and the achie is yours.
This macro is excellent with counting the mobs, it counts all the mobs including LK:

Did this as unholy DK today and all I can say is:
- To do this solo, you need to be able to summon a pet/guardian etc.
- Being able to dismiss makes this A LOT easier.
1:
Engage but do not attack. If you sit on over ilvl 700 you can semi-afk til you reach the number 40 with the:

5:
Once you reach 30 stacks of NP or more, zerg everything down and go face.

Rejoice and watch the RP before dealing the finishing blow.

Necrotic Plague was failing to stack in patch 8.1.5 (preventing soloing of this achievement) but it's been confirmed fixed in 8.3.7. If you were having issues during that time, give it a try now! Thanks to everyone who reported/confirmed!

solo strategy
for
all classes
by combining info.
TL;DR:
Stand on stairs, wait for him to summon 9+ Drudge Ghouls, summon yourself a helper mob to attack him so that he'll cast Necrotic Plague on you, let Necrotic Plague jump around until 30 stacks, burn him down.
SHADOWLANDS WARNING:
Now that everyone is super overpowered and such, you could be
too
mighty depending on your class. Your helper mob could do too much damage and put Lichy into phase 2 prematurely, or your iLvl/weapon could affect plague damage to ghouls, etc., etc. So, be careful out there! Use weaker helpers if you can and don't be afraid to remove your gear if you need to. And don't forget that
死疽
scales to your level. Good luck!
Pre-Fight
Bring a helper mob. This mob will eventually be used to attack

This can be class spells like
暗影魔
or a Hunter pet.
Or it's possible to use a trinket that summons a mob to fight for you, like

It jumps between mobs (including you) every 15 seconds. The stack count increases with every jump. After it jumps from a ghoul that ghoul will die, so don't attack them yourself and let

summon them all he wants.
You will have to tab around the mobs to keep track how many stacks it has, which can get a bit tricky. If you have an addon that helps, great, but you can also keep an eye on
巫妖王
, whom you should have focused before the fight. It's likely that his stack of
热病虹吸
will be similar to the
死疽
stack count. It's usually one or two behind (or can be way off) so don't rely on it, but it may let you know when you're getting close.
The plague debuff can be especially hard to find if you have a lot of ghouls. Be patient! If you're really sure that it's completely gone (either because you ran out of ghouls or you moved, etc.) then you can start over from Step 3 on the same attempt. No need to restart the fight. (Just make sure it's really gone! If you start a second
死疽
, there's a chance you could end up without the achievement because math.)
By the way,
寄生
can get nasty, especially if the plague jumps to you again. Try to keep yourself above 90% health while you wait.
After you're sure there are
at least
30 stacks of
死疽
(more than 30 is fine), burn down
巫妖王
. You can stop worrying about
死疽
now.
Should be relatively simple to ignore the other mobs/phases/mechanics at this point if you're level 100+. Just stay on the stairs and focus DPS on him. You should be fine.
If you're having trouble killing him without dying once you have the requisite number of
死疽
stacks, try the kiting method
mentioned by liquefry
to avoid plague damage. This method did not work for me -- I had to stay still -- but it may work for you if you're having trouble.
He'll go on his 1:30+ roleplay rant and then you can get that final killing blow.
Achievement's all yours!

So this is easy now. level 110 Windwalker Monk. Ilvl 834. Start fight at bottom of steps. Have your back to the throne. Auto attack once or twice. Stand there until 20 plus adds pop. Drop Black ox statue right next to you. Wait for over 30 stacks destroy Lich king. Hardest part is waiting for stacks. I took no damage at all. You're welcome.

Cradboard Assassin Tinker works for engineers if you are doing it solo. Also, you need to face away from the stairs so you don't get thrown away, meaning your back should be facing the throne.

I'm not sure why, but I'm having trouble getting this one. I just tried it on my 110 Guardian Druid, and couldn't seem to get it. I tried it against the stairs, on the stairs, and even roaming around the platform. Most of the time one of those damn shadow traps catches me off guard and I'm flung to my death. I'm not dealing any damage, and he's summoning plenty of adds, but I'm also not seeing any plague stacks pop up either. At least the other day when I ran it with my Prot Paladin and my sister's WW Monk, we were able to see some plague stacks. Flying off the platform to our deaths was still frustrating and eventually we just opted to straight up kill him. This is the last achievement I need for Glory of the Icecrown Raider (10 Man), and it's vexing.

How I did this on 10 man with my Holy Priest:
Preparation:
1. Since I don't have a pet, I used

2. I installed Tidy Plates so I could see the Necrotic Plague debuff on the ghouls. Maybe I had it set up wrong, though, because I would only see the first ghoul it jumped to, then the debuff appeared to disappear, although it hadn't really.
3. Focus the Lich King.
Excecution:
1. Start the encounter. Wait for the Lich King to summon 9 ghouls.
2. Target Lich King. Use

4. After a little bit, the Lich King would cast

I was having a hard time with this on a 110 Druid, because I so out-geared this fight. One round of treants would kick the LK into the next phase really easily and he would stop spawning new ghouls. I tried doing it naked but he killed me with his infest, it started getting too strong for a naked cow. I was also having a hard time getting the plague to display properly on the mobs so I tried something different.
I started the fight geared up but with no weapon equipped. I just let him hit me and spam a crapton of ghouls. I lost count of how many because I actually dc'd during the fight and had to log back in.
When he had what seemed like enough ghouls I summoned the treants, he put plague on me and ran to the center, he was like 72% I think.
Plague seemed to be bouncing around, there were ghouls dying. I re-equipped my artifact and waited. I never saw the plague again until it jumped back to me at 33 stacks. 2 or 3 moonfires had LK at 0% and then it was just waiting for the roleplay and then one more moonfire at the end and I got the achievement.

I found that after the BFA stat squish I kept dying from the LK's attacks once the NP stack reached the mid-20s... equipping
Prydaz, Xavaric's Magnum Opus
mitigated all the damage entirely.

How to do this achievement easy mode.
1) Set the raid to normal 10/25 player. Doing this on Heroic is not necessary and only makes it harder.
2) Have two people, one must have a cleanse spell/ability.
3) Take off all clothes so you don't kill the boss or adds.
4) One person agros the boss and adds, the other stays on the other side of the map. It's easier to do this with a healer as they can cast heal on themselves to agro the boss and adds without causing damage.
5) Wait until the boss has spawned about 12-15 adds.
6) The person furtherest away from the boss/adds will eventually get Necrotic Plague, run up to the other player and cleanse yourself.
The Necrotic Plague will now infect the adds and jump around the adds as they die. If you get the plague again then simply cleanse it off yourself and it will jump back onto the adds. Once the plague is up to 30+ stacks dps the boss into the next phaze.

I don't know why people write essays and overcomplicate the tactics to solo...
I just did it on my spriest first try.
All you gotta do now is to
stand on the stairs
; wait for about
9-12 ghouls to summon
; then
pop your pet, let it attack Lich King for 1 sec
and
disable
your pet.
Stand still do nothing
but keep an eye on the plague debuff on the adds aka
tab
(the one LK has also similar count with it).
Once
you see 30 plague on an add
,
kill LK immediately
. You don't need to allow him kill you and get ressed and all that crap. Just wait for stacks and finish him off lol.
aaand.. Achievement is yours

Just did this with a Frost Mage. Like has already been said, just stand on the steps facing into the circle (or else you get blasted off the edge) have your water elemental attack the LK once or twice and then once you have a fair few mobs just cancel his attack and wait.
You can use Ice Barrier to stop getting damage (the first time I tried it I got DoT to death).
You can either tab around the mobs or just wait until it lands on you to see how many stacks and then just nuke the LK.

For anyone wanting to do this 10 player, I'd recommend not doing it in HC. The Shadow Trap mechanism in HC keeps you moving every 10 seconds or so, and while they do clear after a time, you can still accidentally run over one and get thrown off the edge.

can be used for any class that does not have a pet to initiate plague after having the required adds necessary to solo. Only downside is the cooldown, but can confirm this works (2/12/2019).

I've been trying to get this achievement solo (110 Druid, Balance with Force of Nature) but the plague just won't spread. I'll use force of nature to aggro the LK to get the initial debuff, then it'll jump to an add after 15 seconds, but when that add dies the debuff just disappears. LK isn't getting stacks either, so I definitely didn't just lose track of it. I've tried both killing the adds manually and letting the plague kill them, and it doesn't seem to change anything. Other miscellaneous information that might help: I tried mostly on Heroic, but switched to Normal for a try. I'm not damaging LK besides the damage the treants do. The adds are dying in one hit from the plague, but pre-damaging them a bit doesn't seem to change things. I've tried with full gear and without any gear, and even with res sickness. I've tried both with and without the Hellscream buff. The LK ends up with 2 stacks: one when the plague initially drops off of me, and one when the add dies, but no more. I've tried standing still at the base of the stairs, and trying to kite everything once the plague goes to the first add (this separated LK from the adds too due to different run speeds). Any help or advice would be appreciated because I've been trying for hours and this is driving me insane.

As of 8.1.5 I can confirm this achievement is bugged. I tried all classes with pets and the debuff gets cast on you, it expires, jumps to the first ghoul, the ghoul dies and so does the debuff. No clue why, made a post on reddit and quite a few people confirmed it wasn't working as of this patch. GM's just send you a link to the guide, but that's the guide people are using to try it in the first place.

As some commenters said before, this achievement is bugged. Tried it with a mage frost, making my pet tank the LK while I was going to get stacks, got one stack of the debuff, it expired on me, passed it to a ghoul, and then it died and the debuff just disappeared. The only way to get this achievement now is coming two people to the encounter, and killing every mob that spawns so the debuff doesn't get passed to them. Good luck everyone!

Got this achievement on my 120 priest 8th June 2019 and confirmed my method again on the 13th with a 120 warlock.
I'd been having issues stacking plague via the ghoul method and couldn't get past one or two stacks. Honestly, not sure why this is an issue but whatever. Figured that I could get a friend to come along and we could stack between us rather than the ghouls, and we got it first try, no drama. All you need for this method is a friend to bring with you. Mine was a 120 mage.
So here's the method:
Start the encounter, give the lich king a little love tap right away (no need to wait for ghouls to spawn).
Hold hands with your friend, skip around, kill any ghouls that spawn, and avoid circles of black flingy death.
We waited until we had 2 or 3 stacks (Not sure this is entirely necessary, though), then nuked him down so he would go into his rp. (We found this method best as it means we can just monitor plague stacks without having to worry about killing ghouls or avoiding hazards.)
While the rp is going on, sit nice and close to your friend and watch it play out. You'll continue stacking while it does.
Wait until you have your 30 stacks, then deal the finishing blow.
Ta-da! You have your achievement. Easiest method I've found, and least stressful.

Finally got this achievement yesterday after many failed attempts. Following the suggestion of everyone else, I knew I had to have a 2nd person to get the plague to stack successfully. Unfortunately, none of my friends are still playing this far into the expansion. So I thought maybe I could log into my spouse's account on his computer and afk his character on the steps in front of the throne. Obviously, this just led to multiple resets due to him being flung to his death. After a lot of trial and error though, I found that if I placed him on the actual throne, facing forward, the knock back didn't affect him. So I stacked our characters there, waited for the plague, and took out any adds that spawned to keep the plague on us.
Unfortunately, I was struggling to get enough stacks and keep the adds down without damaging LK with this method when suddenly....the fight reset. Just when I was about to give up for the day, I noticed that one of us still had the plague on us and it was still ticking away! So I just left our characters side by side on the throne until the plague passed between us enough to get to 30. No adds bothering me, no LK to knock us off. Once it hit safely above 30, I ran down and started the fight again. Finished off the encounter and bam! Got the achievement finally.

Anyone have tips on soloing this as a 120 druid? I've tried tanking at the stairs and summoning treants, I was infected with necrotic plague but then it went away. I think the treants took the Lich too low HP so the plague couldn't stack? Please help thanks!

The Necrotic Plague seems to be bugged at the moment, once it gets on a Ghoul and the Ghoul die, it doesn't spread to others Ghouls, you can only get 2 stack max because of it.
According to a reddit post, it's been bugged for a while now (a GM confirmed it), so right now the only way to get the achievement is to do it with another player.

YOU CAN DO IT WITH 3 PLAYERS (OR 2 +1 PET)
OR YOU CAN PUSH LK, if neither you or your friend got a pet
Basically wait untill one of you gets the plague, then push Lich King, kill the ghouls, you will have
14 seconds to do it all, push AT LEAST 90% of LK's life and kill ALL THE ADDS
, then just sit back, next to your friend and let the plague hop between you two while Lich King floats in the air, Once you got the 30, I would wait untill 31 or 32, to be sure, deliver the killing blow.

Just did it with a lvl 120 bm hunter and a lvl 100 resto shaman, the shamy had 19k health, the debuff ticks about 5k at 30 stacks so he was able to keep himself alive but i had a spirit beast just in case.
I pulled LK, stood in his face, when one of us got the debuff quickly killed everyone, boss started his rp, we stood on the stairs with the debuff going back and forth while LK was hanging in the air with 1hp, at 33 stacks killed him, got the achi.

I registered with my Bnet account (I know, I know) just to post this. Alright, so just did this as a 120 late to the mount chasing game. This was the working strategy my friend and I used and it was simple, easy and only required time and some coordination.
Composition:
Monk w/ Black Ox Totem
Hunter
Drop the Ox totem at the base of the stairs. Stand with your buddy almost stacking, 1 or 2 yds apart at most, further than 40yards away. This way, The Lich King does not cast the purple shadow thingies (technical term). When one of you gets Necrotic Plague simply allow it to expire and move to your group mate. The plague will continue to stack slowly. The only coordination required is the following: Occasionally the Lich King will cast Necrotic Plague again - and you will have one player with original stacks of the Plague and the other player will have a new stack of Necrotic Plague. When the original stack of Necrotic plague hops back to the first player (who will at this point have 2 seperate buffs of Necrotic Plague) the second player (in this case the monk) would use his spin to get away from me before the single stack ofNecrotic Plague expired. There was usually about a 1 to 2 second window before it expired allowing the monk plenty of time to cast. This coordination is required every other stack of Necrotic Plague between players. However we were able to get the stack up to 30 without a problem between ourselves and did not need to perform acrobatics with the adds.
Hopefully this helps someone. I did a bunch of digging and asking around while doing this, and while got a few tips, was left struggling after a bunch of tries.

Since the ghoul stacking method no longer works in getting this achievement solo as the debuff doesn't spread when the Ghoul dies to the debuff (bugged since 8.1.0?), I decided to figure out another way to do this achievement by myself only. But this requires having 2 characters on your account though, I did it with a 120 Shadow Priest and a 120 Arcane Mage.
I'd first like to point out that this method is a bit tricky and if you do have a friend that can help you get the achievement, I'd highly recommended it as it will be much less troublesome and more reliable. But if you are like me and no friends are no longer playing, then you can try this.
This method is to essentially juggle the debuff between the two of your own characters. The trick that makes this work is by keeping both characters active in the game using ALT-F4 to shutdown the WoW client, which will keep the character still active in the game for 1 minute before fading out. I'd estimate that you have about 30 seconds of character usage time before you use ALT-F4 again to switch to the other character, this takes into account, the time needed to load the game up again and also some leeway. Its better to ALT-F4 earlier if you are unsure. If you miss one timing, you will likely drop the debuff and need to restart. I'll refer to this as login juggle.
Prerequisite steps
Login char A and clear up until The Lich King. This is the char you want the achievement on.
Make a custom Premade group in the group finder window
Login char B, who will find the group and apply
Login char A, who will invite
Login char B, who will accept
The method:
Login char A and make your way to the Lich King stairs, facing your back to it. (so you will be less likely to get flung off later on).
Close the client using ALT-F4 and login char B, accept the lockout and run to the Lich King stairs also, facing your back to it.
Start the encounter with any char (A or B)
You will need to start login juggle between the 2 characters here
, since the talking is quite lengthy and you want both characters active for all future steps
When the talking ends and the fight has started, while you are doing the login juggle between the two characters, do the following when you have control of a character in-game:
Check your debuffs for Necrotic Plague, once you see it, go to Step 6
Try to kill as many adds as you can without damaging the Lich King with single target abilities, you don't want Necrotic Plague to jump to anything else due to the bug
Note: You may get flung off here due to the Shadow Traps, if a character dies, you will need to reset the fight and bring both characters back. The Lich King casts Necrotic Plague quite early, so you can hopefully reach the next step before any char gets flung off and dies
When you see that you have Necrotic Plague, quickly nuke the Lich King down and wipe out all adds, so that he will start the final roleplay where you can freely stack up the debuff without any interruptions (Shadow Traps or Ghouls)
The hardest part is over now as you have no risk of being flung off. Now just continue login juggle between the two characters until you have 30 Necrotic Plague stacks, should take about 7.5 minutes
Kill the Lich King with the char that you need the achievement on when you have the stacked debuff (might not be necessary to have the debuff on this char)

I created a Wowhead account just to comment on this.
You now need 2 people to do the achievement, but it's pretty easy. (you need 2 because the necrotic plague will insta kill the ghoul and not jump)
Step 1: Start the event
Step 2: Wait for Necrotic plague to land on one of you
Step 3: Stand on top of each other, and kill all adds
Step 4: Confirm you still have a stack, if you want to be super careful make sure it jumps to the 2nd person
Step 5: Kill Arthas, or at least get the RP to start
Step 6: Once the RP is done, Arthas will be stationary and have 1HP
Step 8: continue to stand stacked ontop of each other waiting for the stacks to reach 30 (this will take 14 seconds per stack so paitence is required
Step 9: At 30 stacks kill him.
Someone else posted how you can do this solo, which requires a lot of ALT F4 work, if you dont get a friend to help

I just completed both the H-10 and the H-25 versions of this achievement completely solo (last night and today). I have 5 level 120's I tried this on, and I used my main Prot Pally for the H-25 version, Vengeance Demon Hunter for the H-10 version, but the tactics for both achievements remain the same. FINALLY (after
years
), I got both the Bloodbathed and the Icebound Frostbrood Vanquishers in my mount collections! So I wanted to pay it forward here.
First, I'd like to mention that you do
not
necessarily have to have a pet or trinket to complete this. Just some patience and special attention.
I was not able to achieve this on my Mage, Warrior, or Rogue as the

stacks would just die off and disappear and the Lich King would just progress to Stage 2 and
污染
and I wouldn't progress with NP stacks. I tried going in as Frost Mage (normally I'm Arcane spec) and using my water elemental pet on passive, and it just did not work for me. But even when I went in as Arcane, I still wasn't able to achieve it with just counter-spelling and putting my shield up. My Vanish on my Rogue just reset everything, and her Assassination poisons were way too much to handle for the Lich King. On my Warrior, I tried using the

trinket (non-corrupted), and it only has a 1-min CD, but Mayor Striggs doesn't take any Necrotic Plague stacks at all in order for them to jump to you and stack. :-/ But my Prot. Pally main and Vengeance Demon Hunter both used a combo of offensive/defensive tactics and completed the achievement

Remove ALL corrupted gear
before the fight and make sure all defensive spells are off their CD's. You don't want corrupted gear proccing and killing ads or the LK prematurely! I replaced all my corrupted gear with 120 Heirlooms. Heart of Azeroth has no impact, but

can end up influencing the fight with it's corruption resistance, so I replaced it with an heirloom cloak.
If you have DBM, type /dbm in your chat, expand the Lich King, Icecrown Citadel, and The Frozen Throne, and click on the Lich King. Make sure to check off warnings for all Necrotic Plague jump targets, announcements, and other warnings for Necrotic Plague. This helps you know and track when it is on you (and if it expires), as well as being able to see it jump from ghoul to ghoul.
Stand on the 3rd or 4th step after talking to Fordring with your back to the throne and stairs. This will prevent you from accidentally being caught in

which will get you thrown off the platform.
After the initial RP, just use Taunt on the Lich King and then

use Divine Shield or Demon Spikes

), just stand there patiently and pop Taunt and Divine Shield or Demon Spikes over and over when it's off CD. The ghouls will take damage from being affected by stacks of

and passing those stacks on to you (which is another reason why you do not want to budge off that 3rd or 4th step), and they will eventually die, but don't worry. The Lich King will keep summoning more ghouls. If one of the big ghouls threatens to kick you off the stairs, you can slightly turn just enough to hit it and kill it. You don't want to be budged off those stairs! At level 120

Lich King will keep summoning ghouls over and over, especially when you taunt and then use Divine Shield/Demon Spikes right after, and eventually you will get 30+ stacks of

. Took about 5-10 mins for me (shorter time on my Pally, longer time on my DH, and I'm not sure iLvl makes a difference). On my DH, Lich King did run over to Tirion Fordring for a minute, outside of my taunt distance, but he came back over to me where I taunted him again, popped Demon Spikes, and he summoned even more ghouls and that's where I really got those Necrotic Plague stacks flowing. On my Pally, he always stayed in front of me until I had all my Necrotic Plague stacks (
maybe threat tables are higher on Pally's than DH's?
). Once you have seen 30+ stacks of
死疽
on you, feel free to start nuking the Lich King down. Don't follow him to Tirion Fordring though, stay on the steps until Fordring does his final blow after the RP, then run over and end the fight with either Avenger's Shield or Throw Glaive.
That's pretty much it. Just requires some patience, maybe turning your volume down some, and paying close attention to those
死疽
stacks and reacting with both offensive, non-damaging spells followed by defensive, non-damaging spells.
Hope this helps many of you trying to solo this
without
having to make a group, or trying to perform an Alt-F4 juggle in and out of the game with your alts. Enjoy your bony dragon mounts! :)

120 warrior here. Soloed by taking off both weapons and Jar to ensure no accidental insta-kills. Walked to steps and just stood about 3 steps up facing the center. Pulled him with a taunt and just sat there 2-3 minutes. Every once in a while I'd kill an add somehow, so I'd just sit down infrequently. Stacks went from 3->9->12->19->28->36. The only other thing I did was taunt him when it was up, and use the non-damaging slow PBAOE piercing howl. No idea if either of those are necessary, but a previous poster did it. The RP took longer than the actual fight, and I was able to get the achievements "Been Waiting A Long Time For This" and "Neck-Deep in Vile" at the same time.

Some people are REALLY over complicating this.
I was 459 ilvl enhancement shaman.
1: remove azerite neck and trinkets.
2: Stood at base of steps
3: Let him continuously summon mobs and watch the debuff bounce around.
NOTE: some mobs will die from it which is fine because I saw the debuff be on more than one mob at
the same time.
4: kill when ur at 30+ stacks. I HIGHLY suggest the addon Instance Achievement Tracker as it will count the stacks for you in general chat or party chat depending on how you set it up (very easy to do)

The easiest way to do this achievement:
1. Start the encounter.
2. Watch out for shadow traps (if you are doing it on 10hc).
3. You have to kite all the adds away from you because necrotic plague jumps to the nearest target when it ends.
4. Wait him to summon atleast 4-5 Shambling Horrors (because if all the adds die with the necrotic it resets).
5. After enough adds are summoned you can watch for the necrotic stacks. (IAT addon is great for it but not mandatory).
6. After you get 30 stacks of necrotic just kill him.

I just did this today on my level 60 druid. Was in Guardian spec - started the encounter and went like 4 steps up and faced the middle - used taunt on CD and just stood there for like 3-4 mins until the stacks went up to like 43. I didn't have my weapon on but just didn't attack anything. Didn't need to heal or use defensive CDs at all. Didn't really use any special add ons or anything.

This used to be pretty straight forward-- just wait and let it stack 30+, but it was more difficult for me on my lvl 60 protection Paladin today. My add on that measures damage detected a high amount of damage being done to the Lich King by "No One". It seems the necrotic plague was damaging him and not me. He was being killed so fast, then it moved on to the next stage of the fight, and my necrotic plague never stacked up high enough. I would jump off the ledge to reset the fight, kept trying, no luck. The way I finally got it to stack to 31 was to go up the stairs high enough so that the Lich King was on the stairs. This seemed to slow down the damage. I got the achievement finally! If anyone else is struggling with this, I hope this helps. :)

Level 60 fire mage
I started off by standing between him and the stairs before the fight even started. This was old instructions but not even sure if it's necessary. Then I just stood there and did nothing until I saw a single debuff of 30 or more. Sometimes there would be two debuffs. I recommend waiting until a single one says 30+ rather than taking your chances and adding them.
The way the debuff works is it'll take a while to show one, then it'll go away, then it'll show a few, then go away, then stack a few more the next debuff, etc., so keep in mind when it's doing that that everything's working as it should.

As a prot pally with a 250ilvl the only way this was possible was with rez sickness 3rd step back to the stairs.

Soloed this as a level 60 Pally the other day (current as of patch 9.2)
*Be sure to not have Holy Shield talent on (if prot), or just do what I did --- switch to holy, take off almost all of your gear. Definitely weapons/shield*
1) Start encounter.
2) Don't hit the boss.
3) Walk backwards and kite around in circles, don't lose LoS
4) Watch for shadow traps. You step in one, you get launched and die :(
5) If you can't tab target yet, you will learn thru prac app now.
6) Disregard infest.
7) Let him continually summon adds. The lil dudes and the big guys. I forget their names.
8) You'll get the debuff at some point. Cleanse it off of you as long as there are mobs around. As it stacks higher, it will 1-2 shot the mobs and keep transferring. Just keep kiting. This took me a good 5-10 mins of running around. If you lose your stack, just keep going and try again. Just don't hit the boss or step in a shadow trap.
9) Once the Necrotic Plague reaches 30 (I like to go higher, but I'm just an overachiever like that), smack the boss with a holy shock/judgement/slap/consecrate...doesn't matter at this point.
10) He dead. Yay.
I'm sure I missed something, or there may be some confusion. I suppose it could be broken down further in Barney style, but yeah. Enjoy your nifty, severely malnourished dragon mount(s).

Still soloable at 10.0.5. At H10 you have to move around to avoid the Shadow Traps.
It might not seem like the Necrotic Plague is stacking at first, but after a few minutes kiting around the debuff will appear stacking to 30.

I did this on Normal and sat there stacking. There's no traps to avoid or LoS spell to break. I got 32 stacks and killed him. After all the talking, achievement popped.
