I'm assuming this is an INCREDIBLY difficult achievement since progression sites show it worth 7 times as much as just killing Anub himself. Also, it's probably noteworthy that they do give a nasty debuff which probably makes this so difficult.

Yes, it's true. You have to kitte them around. My guild and i still can't figure how they go berserk ignoring stun and being not affected by the slow debuff of ice on the floor. Does anyone have an idea ? (A rogue or a hunter is a must have in order to dispell this buff).
They go berserk when you slow,snare them. Mage's Frost nova 100% drive them into berserk mode.

Just did it using 3 tanks 4 healers 3 dps ( 1 of them rogue using Fan of Knives with almost no gear {only hit parts } and lvl1 daggers ).
We had 2 NR tanks for adds, pala with non specific NR gear just the 2 rings from Onyxia and Warrior with max NR gear, the paladin was keeping like 6-10 adds on him and the warrior was keeping Vigilance on the pala so he could basicly taunt non-stop every add that spawns or decides to go off to the healers. The third tank was tanking Anub and the spawning add. We had 1 healer each tank and 1 druid to hot everything. Basicly tanks were getting max 25k ticks not more.

did this achievement today with
3 tanks: 1 feral tanking anub
2 prot warris giving each other vigilance who were tanking the adds at 2 positions, one to the left one to the right, the raid was standing in the middle
4 healers: 2 restoshamys with earth shield on each tank + diszi on the one warri and resto durid on the other warri
1 moonkin to soothe the enrage off of the scarabs
2 dds: mage + dk
did 4 borrow-phases and killed the scarabs after the last ones in p4 spawned
first we tried it with kiting the adds and used the earth-elementals but it worked out for us with spottanking (and power healing on the tanks) + we didn't use the earth-elementals because they killed the scarabs too soon.

This achievement has been nerfed. Instead of needing 40, you only need 25 now. A big step down from 40 for sure!

The key to this fight which doesn't seem to be mentioned anywhere is that the debuff from the little bugs maxes out at 40 stacks. No one in our guild knew this because you normally kill the bugs off fast as possible when downing the boss. So with an OT picking them up, the debuff is pretty easy to heal through with a couple healers.
Also at 85, you may need to have your hunter or rogue that is taking care of tranq shot/poison use a lower dps weapon. Otherwise their auto-attacks when soothing can kill the bugs.
VallonNz's video above is really good, though the first time I watched it I had no idea how the tank survived because I didn't know the stacks maxed out! :)

We just completed this for several people who didn't do it back in the "old days".
Currently, 14 scarabs spawn per "down" phase. The timer will reset if you accidentally kill one, so don't feel a need to wipe it up if one or two die early. The scarabs slowly spawn during the "down" phase, so wait until he's about to pop up at the end of the second then annihilate them.
We used a shaman's NR totem thingy along with a +90 resistance potion on a pally tank (plus another +8 for broiled bloodfin). This mitigated his damage somewhat, and the encounter was very easily done with three healers. The most difficult part was not killing scarabs by accident.

By far the best method must be using druids Soothe and then kiting them over a sea of them frost orb thingys; that way you won't have to worry about you killing them with a hunter.
I just did this with five other, a total of six for you who can't count, and it went pretty good. We made it, but only barley since we only had one healer. But with a full group this should be really easy.

Just 3-manned it. Frost mage, blood dk, holy pala. Tried different strats a bit with mage being fire and pala with RF activated kiting everything while me (blood dk) kept myself alive. Both strats worked fine and we were close to getting the achievement (missed the 25th one with 2 sec once). Basically the healing got atrocious for the paladin to cope, especially if me or the mage got the debuff stacked from the adds, and sooner or later someone died, mostly the pala as he had aggro on 95% of the adds.
So we tried a different strategy, with me taunting as many adds as possible and got it on the first try. Just make sure you don't damage the adds before due time. I was saving AMS/mirror and IBF for the last bit with 15+ adds. Having a prot warrior with vigilance I think makes it even easier for the healer.

Did this again to help guildies out. This time we used disc priest, ret paladin and blood dk, with the dk taunting in as many adds as possible. This is more than doable with 3 players, but it requires a very competent healer. Two healers would make it a lot safer.

My partner and myself just duo'd this yesterday (19 June 2012). Resto Druid 402 ilvl and prot paladin 403 ilvl.
What you have to do as a Resto Druid first, run around and get those Frost Orbs down, as many as possible. Don't slack, keep getting them down, you'll need them. There isn't much healing so throwing a Rejuvenation/Lifebloom on the tank is enough to keep him up (incl. his self healing). You'll need those to be able to kite the adds as much as possible over the ice patches that they leave on the ground, and to prevent getting hit by the boss' underground spikey attack at all times.
As the boss submerges, the adds begin to spawn. They will attack the healer as well as the tank, so try staying close to the tank so he can pick up as many as possible, the more you have on you the less likely you will survive.When the adds start showing up, the damage goes up significantly. Now you need to find the right balance between healing/getting a few more orbs down/and using Soothe on the enraged adds <-- This is quite important. When you feel you're taking too much damage, try popping either Tree Form for the extra healing and armor or go cat and use a Stampeding Roar to get some distance between you and the adds, so you can kite them better. Throughout the whole fight you will need to move and kite the adds as much as possible.

The Prot Pala in this fight tanks the boss as well as the big adds spawning at all times and dps as much as possible, those adds HAVE to die cause they apply a debuff on you which increases damage done to you by quite a lot, so you don't want that on you.
When he submerges, start collecting the adds and get as many as possible off your healer, make sure you don't get hit by the spikes and run around the room in a big circle, kiting the adds over the ice patches to slow them down. The less they hit you, the better! Also makes it easier for the healer to target the ones that are enraged, when there's many of them piled up it's difficult to select the exact one that has the enrage effect on them.
The boss will come up again, and you'll need to tank him yet again + adds, don't forget to keep kiting as much as you can though, while dpsing the boss and big adds, and wait for him to submerge a second time to collect the rest of the adds (you need 2 submerge phases to be able to collect 25).

The last bit is extremely difficult for both the healer and the tank, watch your health, use your cooldowns wisely. The tank will most likely need to LoH either the healer or himself, to buy the extra time that is needed to collect 25. As soon as there are 25, you need to be fast. It isn't bad at that point for the healer to die if he has any adds on him, cause when he dies, they will all automatically go to the tank which makes aoeing them down in time alot easier. I myself died and 3 seconds later I got the achievement. The tank needs to focus alot on surviving as well as doing as much AoE damage as possible, and will shortly die after.
It took us only 2 tries, but we've been duoing content for ages and know our classes pretty well. It requires alot of awareness/focus/coordination/timing, it's not an easy achievement.

We did this as Affliction Warlock + Holy Paladin in heroic 5.1 gear. Doing it in 10H mode was slightly inconvenient, but not a big challenge.
Lessons learned:
Don't DPS the boss at all. If you have the Voidlord tank him, disable his Void Shield ability, or he will push the boss to P3 before you get enough of the scarabs.
It took us 3 burrow phases before we had enough scarabs. One of the comments said there were 14 scarabs per burrow, so we first tried killing them after boss returned from the second burrow; there were only 21.

Soloed as an ilvl 517 Blood DK in tank gear. Was actually pretty close towards the end sitting at ~40 stacks of the nature debuff, but managed to barely pull it off through rotating cooldowns. Highly suggest Conversion as your tier 4 talent should you attempt this. Also counted 9 scarabs after the first burrow, and less than 25 after the second, so you appear to need 3 burrows.

Tracking on both this and the Ulduar opening gauntlet dwarf kill count seems broken. Unable to get either achievement, despite killing excessive numbers of mobs in far less than the required time limit.

Spent quite some time trying to solo this as a hunter, but haven't succeded yet. Some important points:
You can dispell the enrage from the adds with Tranq Shot, without killing them. But you have to be really fast with turning so an auto-shoot doesn't get the chance to go off.
As long as you don't keep getting hit by enraged adds all the time, Spirit Bond will easily keep you up.
Pet won't need much healing at all, Spirit Bond and an occasional Mend Pet is more than enough.
I tried both as Survival and BM. I don't think it matters what you choose really, but Survival does have Entrapment which makes it slightly easier doing the kiting (shouldn't be needed though).
In the end I didn't get the achievement today. From what I'm reading, it's because I accidentally killed one of the Scarabs early (a failed Tranq where the auto-shot got through). The counter doesn't seem to reset. I'm going back next week with a grey low lvl ranged weapon, which hopefully won't 1-shot the scarabs if I accidentally hit one of them.
Edit: This one is really bugged. I went back today with a low dps ranged weapon. This was enough that an auto-shoot wouldn't kill a scarab. Did everything just fine, dispelled the enrages, kited the scarabs, waited for 3 burrows. When 3rd burrow was finished, I let them all gather up and killed them in a few seconds. No achievement.

5.3 with 2 ret pallies, both ~518 ilvl. I tanked the boss (in ret gear) and the only problem we had was the nature debuff from the adds, just sprinted around everywhere. you can Bubble off the stacks, and during the 2nd burrow we both bubbled, so LoH was up for the 3rd burrow. once that happened, we very quickly killed the adds, and yay achieve!
Also, Leeching Swarm hurt a bit, but, was easy enough.

This achievement is unsoloable by Monks... since we deal damage every time we parry, and with that many adds, we parry a lot, and so scarabs keep dying, and they're almost all dead before the boss goes into the next burrow phase.

In 5.x, this is still a hard achievement, although you can see from other comments here that some (mostly tanking?) specs have been able to solo it.
For me, I found it difficult even in a group consisting of 3 dps and 1 healer. The problem is that by the time you get enough adds (which is usually during the 3rd submerge phase), the stacking damage debuff will have become crippling. What worked for us was not so much to get more healers to compensate but to get a tank in the group. Once we had a tank to soak all the damage and debuffs from the adds, the healer had a more manageable job, and we got it easily from there.
Notes:
(1) The boss alternates between above-ground and submerge phases. The swarm scarabs only spawn during the submerge phase. Rather than trying to count the adds, I recommend just counting phases. You should have enough adds at some point during the 3rd submerge phase to get the achievement, so just wait until the end of the 3rd submerge phase to be safe.
(2) At 30% health, the boss will no longer do any submerge phases. It's safest not to dps him at all until you've gotten the achievement, which does not require you to kill the boss along with the adds.
(3) Other adds will spawn besides the swarm scarabs. These are safe to kill, but be careful not to cleave by mistake and kill the swarm scarabs. It's safest to remove any gear with cleave effects (such as the legendary dps cloak) prior to the fight.

Don't dps the boss, just heal to full health until the boss submerges.
Kite the adds (they will eventually get speed buffs however). You will start to get stacks so keep healing yourself to full.
When you reach high stacks use cooldowns as the debuff will hurt a lot (max 40 stacks = 40000 nature damage per 3 sec) on top of boss auto attack which is rather nasty. Pop bubble if you are about to die (I used it during boss stun). It will wipe any stacks.
When you have enough adds (you have to see 3rd submerge) wait until adds gather around and drop

Solo'd it as a 577 Resto Druid. Wasn't really difficult at all. you can just stand in place and heal yourself lightly, I was keeping up Lifebloom at the start but I found it wasn't necessary. I moonfired lots of orbs for the ice patches and avoided getting hit during pursuit and after the 3rd submerge I popped heart of the wild and hurricane'd the mobs, they died in 1 tick then I wrath spammed the boss. You get the cheeve even if you don't kill the boss.

Soloing as a prot warrior - Pro Prot warriors probably already know how to do this, but if you're a Prot Warrior and having problems, I just did 10m normal with 574 ilvl, I'm sure other have done it much sooner, but for those of us who aren't stupidly good, here are a few hints/tidbits
1. Second Wind is a must, outside of that glyph: Death From Above is very useful for moving to avoid damage. Glyph:Enraged Speed would probably help too to increase your movement when running from the spikes (not 100% sure, didn't use it).
2. DPS burrowers only - doesn't matter if you kill them or not as when they're hurt they submerge. The 40 stacks plus melee damage is completely manageable with Second Wind as long as you don't have the armor debuff those guys throw out. So be careful but attack them.
3. I found Shield Barrier > Shield Block. The incoming damage was so low that barrier lasted for a while, block didn't seem to help much.
4. I didn't kite except to run from the spikes to minimize damage intake. Just back up slowly so you're getting the most out of dodge/parry/block and in Second Wind we trust.
5. If you're needing more health, flask/food/command shout.
Took me 3 tries, 1st time I dps'd him and never got a second submerge (then had to wait for the enrage as I couldn't die). 2nd attempt found out the burrower's armor debuff was a bad thing, especially when you get ice blocked right before the 3rd submerge (4 burrowers ~18 scarabs, plus anub was just a little too much without dodge/parry/block). And 3rd time was win.

Solo'ed this evening, 473 BM Hunter.
As hunters go, I really don't think there is any other way you'd solo it other than as BM. The fight is so healing intensive as you are waiting for the number of Scarabs to increase. I have the 8 second speed increase after disengage on my talent tree in order to continually stay ahead of the mobs.
I did have to reset the encounter, as I dropped boss down under 30 percent. Waiting for him, her, whatever that thing is, to get healed back up doesn't matter, it wouldn't submerge after going below a certain percentage. So I feigned death and restarted.
I waited for 3 submerges, without a lot of trouble, but found it is very important to kill the Nerubian Burrowers as soon as they pop up or they will down your hp very fast. I did well with my strategy till boss came out of the ground after the third submerge. There were so many adds and I was trying to kill two Burrowers but targeting them was next to impossible. I ended up using both my deterrence and even then I was under 5% health, I kept disengaging, popped Beastial Wrath and spammed multi shot to down all the adds, got the achievement, then downed the boss.
It is not easy, but it is doable.

Soloed as a shadow priest. It is very important that you remove any multistrike trinkets/armors, such as the legendary cloak. Drop lots of ice, use speed boosts, and make sure and kill the spiders as they spawn. You can use Divine Star for healing if you are far enough ahead of the scarabs, but do be aware.

this really useful macro that can be used to count the mobs. The way this works is to count the visible nameplates. So, if there are spiders up (or the boss) they will also be counted. Also, the kiting aspect of this achievement means that not all of the mob nameplates might be visible, so you should attempt to clump them up a bit to ensure more accurate counting.

Soloed as a 547 (506 without the cloak) warlock destruction. Both

are needed since the difficuly is about surviving until you have enough scarabs.
I used my voidlord to get aggro on the boss and healed it through the whole fight. I used the ice orbs to gain burning embers to heal myself, the fight is mostly about running around the place hitting as many orbs as you can to build up embers and kite the scarabs. Keep your healthstone for critical moment, you'll eventually need to stack it up with an ember tap to stay alive while healing your pet.
You can't stun the scarabs so I would recommand placing a

to use if you get surrounded.
If you don't accidently kill any scarabs in the process, you should have enough right at the end of the 2nd bury phase. Don't forget to put you pet on passive and to watch for his health, he can be brought down rather quickly. Good luck !

Breakdown:
- Need 25 small adds to die at once. At 14 adds per phase, it requires 2 submerge phases. This means that the adds from the first submerge need to be kept alive for the second submerge.
- Small adds enrage eventually and start to apply a stacking DoT, up to 40x. This starts to hurt so it requires a lot of healing.
- Large adds are almost inconsequential.
- Anub just provides annoyances in the form of stuns and more damage to heal through.
My strat:
1. To start, do as little damage as possible to Anub and wait for the submerge.
- When you start to have adds on you later, you will need Holy Power to heal through it, and you don't want to accidentally push into Phase 3
2. Kill large adds as necessary
3. Have 3+ Holy Power before submerge
- This makes you quite quick with Pursuit of Justice
4. Run from small adds and Anub's spikes, kill big adds
5. Keep yourself alive during the emerge and the second submerge
- Word of Glory, Execution Sentence, and Selfless Healer
- Hand of Purity on CD after 25 stacks
- Bubble or HoP when you have 40 stacks
- Build Holy Power off of Anub
- Killing big adds procs
so take advantage of that
- The damage is healable without too much stress, though I used all my CDs including LoH and a bubble
6. Kill 25 small adds, kill Anub

With patch 6.0.2 in and the stat squish and all I wanted to see how easy this was now.
574 warlock, glyphs/talents etc not important, my voidlord was tanking the boss without Void Shield on auto cast. Do remember to have it stop its attack, because it will kill the boss fast even with just melee attack.
I ran around tanking - not kiting - all the adds and made patches of ice on the ground. I never got below 98 % health even with a full 40 stacks of the nature debuff from the adds and their enraged melee.
Laughable now.
Macro that Gerbera posted is very helpful. I unfortunatly needed three burrows =/

Trivial now as a level 100 Ret Paladin with iLevel 615. Just stood in one place waiting for enough scarabs to appear, then AOE'd them down. Forty stacks of debuff were not a problem, a few quick heals here and there.

Just soloed with a shadowpriest level 100!
I couldn't do it on 90 because the damage would just be too high to counterheal.
On 100 it is easy.
I pulled Anub hit him under 70% (not required but its what I did...)
He will summon 2x 2 spiders which you can/should kill right away. There is no reason to keep them around
Wait for him to submerge and then kill the spiders that spawn but keep the scarabs!
Rinse and repeat this 3 times! On every submerge 12 scarabs will spawn - so for 25 total you need 3 submerges and you may kill 11 of them if you need to
I took very litlle damage from the debuff, the boss and the spikes during submerge. it never got me below 95% of my life. But then again a shadow can shield himself so you might need some healing on other classes. Remember that you can kill 3 scarabs and you can kill all the spiders that spawn. So there is something to leech life on from time to time.
Kill some of the ice balls floating above you that will slow the annoying adds a bit and mostly slow the spike during the submerge phase. Because they produce a lot of noice.
TL:DR:
pull boss, kill all spiders, wait for 3 submerges (12 scarabs on each), kill scarabs -> win. (kill boss optional)

Make sure any passive damage talents or abilities are off. e.g. If you're a prot paladin, make sure you're Holy Shield talent is not active. It will kill the mobs almost immediately and the boss will go down as well saving you to the instance for the week.
Also, to count the "Swarm Scarab" ONLY, use this macro:

I believe he is bugged or something. I tried doing it in 10M Heroic and he just wouldn't submerge. I stood for 10 minutes just hitting him with white attacks sometimes with my level 100 paladin. He spawned the big spiders, but didn't submerge, or spawn any scarabs. So... bugged. :(

Easy in Shadowlands. I just didn't attack at all and waited for him to submerge. On the first submerge there were 9/25 Swarm Scarabs, on the second submerge there were 24/25, after the third I had 25/25 and one shot AoE'd all of them down while the boss was submerged. Achievement complete, then just waited for the boss to come back up so I could finish the fight.

Did this during Shadowlands. Get a raid achievement tracker. It will count these scarabs for you during the fight. You don't need to do any damage at all to the boss. Just stand there after aggroing him and wait. Once you have enough, nuke them all and you'll get the achievement.
When I did this on 25-man, the addon wasn't working, and I just waited until I was sure I had enough. It was working during 10-man.
